摘要:
Cryoprecipitated factor VIII/von Willebrand factor (FVIII/vWF), freed of fibrinogen by clotting with calcium and Defibrase®, was chromatographed on Sepharose CL-2B. Fractions containing lower-molecular-weight forms of FVIII/vWF comprised coprecipitated plasma proteins of similar molecular weights. The major contaminants, fibronectin and IgM, were removed by affinity chromatography on gelatin- and anti-IgM-agarose, respectively. Finally, pure low-molecular-weight FVIII/vWF protein was harvested in the void volume fraction of a Sepharose CL-6B column. The smallest multimers had the size of the tetramer of the basic subunit chain of FVIII/vWF

The recovery and initial half-disappearance rate of factor VIII procoagulant activity (VIIIC) and procoagulant antigen (VIIICAg) were studied in 9 haemophilia A patients following infusion of factor VIII concentrates to varying plasma VIIIC levels. While VIIIC recovery was independent of dose, the VIIICAg recovery varied in a dose-dependent fashion. The excess of VIIICAg relative to VIIIC found in the factor VIII concentrates (VIIICAg/VIIIC ratios of 1.9–3.1) was not observed in plasma samples taken after low level infusions (plasma VIIIC < 1 U/ml) but a significant excess of VIIICAg was observed in higher level infusions. The VIIICAg recovery of post-infusion plasma was not increased by treatment with phospholipase

A 59-year-old male patient with Graves’ disease and severe hereditary Fletcher factor deficiency is described. PKK clotting activity as well as the activity by a chromogenic substrate method (Chromozym PK®) was less then 0.01 U/ml. In contrast to functional tests, the immunological assay (Laurell method) showed a PKK antigen concentration of 0.25 U/ml, indicating the presence of an abnormal nonfunctional PKK molecule (CRM+ variant). An inhibitor was excluded since the patient plasma did not inactivate partially purified PKK. Investigation of 11 family members revealed a reduction of the PKK clotting activity in 9 relatives of the patient. Since Graves’ disease is considered an autoimmune disease, our case represents an example of an association of a severe hereditary deficiency of a contact factor and an autoimmune dis

摘要:
The performance of a commercially available assay for the determination of prothrombin by a chromogenic substrate method has been evaluated for use on an automated clinical analyser. The method was rapid and simple; accuracy, precision and sensitivity were satisfactory, while carry-over was negligible. Determinations were performed on 170 plasma samples from patients on long-term oral anticoagulant therapy, using the automated amidolytic method; comparison of the prothrombin levels with results obtained by the prothrombin time and Thrombotest methods provided highly significant correlations.

Blood samples were drawn from 129 randomly selected young adults. Intake of acetylsalicylic acid (ASA), contraceptive drugs, smoking habits and health state were registered. Males had significantly higher systolic blood pressure, shorter bleeding time and lower VIII:C. Smoking was only correlated to some variables assessed in the female group. Users of oral contraceptives smoked more, had a shorter bleeding time and higher fibrinogen levels. Factor VIIIR;Ag was elevated only in female smokers with blood group non-O. Non-secretors had shorter bleeding times and a tendency towards higher VIIIR:Ag.

摘要:
Prostacyclin is able to inhibit the development of platelet prothrombinase activity. This inhibition, which also occurs with dibutyryl cAMP, is presumably due to the ability of prostacyclin to prevent the formation of a negatively charged phospholipid surface at the exterior half of the platelet membrane. Generation of this procoagulant surface, as induced by platelet activation with collagen plus thrombin, does not depend on thromboxane A2 formation.

Low molecular weight (LMW) heparin has been shown to prevent experimental venous thrombosis. In order to investigate its biological action and its potential use in open heart surgery, we have conducted an experimental study using extracorporeal circulation (ECC) in sheep as an experimental model. 18 sheep were randomly selected to receive either LMW heparin (2 mg/kg), high dose heparin (HD; 160 U/kg), or low dose heparin (LD; 60 U/kg). The HD heparin yielded the same circulating anti-activated factor X (anti-Xa) activities as the LMW heparin and the LD heparin yielded the same anti-thrombin activity. LMW heparin and HD heparin were both effective in preventing blood clotting in the ECC circuit, demonstrating the antithrombotic activity of the LMW heparin. Clotting in the circuit was observed following LD heparin administration showing that the efficacy of LMW heparin does not only rely upon its weak anti-activated partial thromboplastin time (APTT) and antithrombin activity. Fibrinogen, fibrinogen degradation products (FDP), factor V (F V), platelet count, antithrombin III (AT III), fast acting antiplasmin (AP) were evaluated during and after ECC. The defects in hemostasis were similar in the three groups. Conclusions were: (1) heparin chain depolymerization diminishes the anti-APTT activity without altering the antithrombotic property; (2) LMW heparin is an effective alternative to heparin in cardiac surgery; (3) the absence of postoperative circulating anti-APTT activity might be associated with a reduced incidence of hemorrhagic complication, but we were not able to demonstrate it.

The function ex vivo of autologous platelets, labelled with 111In, was evaluated at various times following injection by comparing the retention on foreign surfaces of radioactivity with the retention of total platelets (represented almost entirely by unlabelled and therefore unmanipulated platelets) after the application of fresh un-anticoagulated whole blood to (a) filter paper and (b) glass bead columns. Relative retentions were similar for the two materials, with labelled platelet retention being about 80% that of native platelet retention. Prior to injection, when labelled platelets were returned to an excess of freshly drawn whole blood, retention of radioactivity was greater than that of native platelets with a ratio significantly greater than that seen with labelled platelets tested ex vivo. Filter paper retention provides a simple technique which may be useful for the evaluation of labelled platelet function as a function of platelet age and also, with further standardisation, as an inexpensive rapid test of unlabelled platelet function.

Possible effects of an ex vivo fluorescent labeling procedure of blood platelets on the platelet viability were examined by different function tests. The degree of the impairment of the platelet function proved to be principally correlated with the dye concentration used during the labeling procedure. However, the physiological properties of the labeled platelets obviously remain unrestricted by using intraplatelet fluorescein isothiocyanate (FITC) concentrations below 0.55 mg/1011 platelets, corresponding to 8.5 × 106 FITC molecules bound to one platelet. A procedure for fluorescent labeling of blood platelets is described which is considered to be preferably suitable to prepare platelets for vital microscopic platelet observations

Investigations on the platelet function in diabetes mellitus were performed on 28 patients with insulin-dependent diabetes and in 33 healthy controls of similar age. In the diabetic patients it was possible to induce 50% of maximal aggregation by lower concentrations of adenosine diphosphate or arachidonic acid than in the controls. In the presence of N-ethyl maleimide, platelets from diabetic patients produced significantly more malondialdehyde than those from normal controls. After addition of arachidonic acid the platelets from the diabetic patients also synthesized more thromboxane B2. This synthesis of thromboxane was inversely correlated to the minimal concentration of arachidonic acid necessary to induce 50% platelet aggregation. Circulating platelet aggregates were more common in the diabetic patients than in the controls. Plasma levels of β-thromboglobulin and platelet factor 4 were raised in parallel in the diabetic patients and correlated with the increased production of thromboxane B2 by the platelets from the same patients. Platelets from patients with diabetes thus demonstrated signs of hyperreactivity both in vivo and in vitro. This may be of clinical importance for the development of vascular complications in this disease
