A new technique, which simulates primary haemostasis in vitro was tested using blood from control persons and from patients with a defined abnormality of primary haemostasis. The device allows the reproducible measurement of in vitro bleeding time and volume in small samples of whole blood. Inhibition of platelet adhesion and aggregation can very sensitively be detected with the new technique, which allows its possible use in clinical or pharmacological applications.

Platelet adhesion was studied in whole blood using a simple stagnation flow method in patients at high risk for the development of atherosclerosis. Fourteen patients with hypercholesterolemia, 26 with hypertriglyceridemia and 25 normolipidemic hypertensive subjects were compared with 75 normal (normotensive and normolipidemic) subjects. Increased platelet adhesion was found in hypercholesterolemic (but not hypertriglyceridemic) patients only when native blood with no anticoagulants was used. In hypertensive patients, platelet adhesion was signficantly elevated, but remarkably reduced by beta-blocker drugs. Propranolol and atenolol significantly reduced platelet adhesion, but this effect was found to take a longer time than that required for significant blood pressure reduction.

Platelet sensitivity to adenosine diphosphate (ADP) and to prostacyclin (PGI2) was studied in normal and diabetic pregnant women. The threshold concentrations of ADP inducing the second phase of aggregation were used to determine the platelet sensitivity to PGI2. The sensitivity of platelets to ADP increased in both groups in the second trimester, thereafter it decreased both in normal and diabetic pregnancies. In contrast, sensitivity to PGI2 increased in the last trimester of pregnancy. No difference could be observed between diabetic and normal groups. The similarity of the results between the two groups could be explained by the normoglycaemic state of well-controlled diabetic pregnant women.

Plasma antithrombin III (AT III) and fibrinogen were measured in 25 Saudi children with nephrotic syndrome during both relapse and remission. During relapse the mean AT III level was 46.6% (range 7–84%) and mean fibrinogen was 992 mg/dl (range 413–1,433), while during remission, AT III levels increased remarkably to a mean value of 120.6% (range 81–160) and plasma fibrinogen dropped to a mean level of 431 mg/dl (range 230–693). In 10 of these patients AT III was measured simultaneously in urine. During relapse urine AT III levels were of the same magnitude as plasma AT III, while during remission no AT III was detected in urine. These findings confirm the hypercoagulable state during the relapse of nephrotic syndrome and the quick recovery during remission. None of our patients showed any clinical evidence of thromboembolism. Postoperative changes related to platelets and their correlation with the incidence of deep venous thrombosis (DVT) were studied in 30 patients undergoing total hip replacement. Levels of platelet count, platelet-crit, mean platelet volume, spontaneous platelet aggregation, platelet factor 4 (PF4) and β-thromboglobulin (β-TG) were measured before operation and on the 1st, 3rd and 7th postoperative days. DVT was detected by 125I-fibrinogen leg scanning in 11 of the patients. After the operation there was a significant and progressive increase (p < 0.01) in PF4 and-TG, and the presence of circulating platelet aggregates was demonstrated. Platelet count levels and platelet-crit were decreased on the 1 st and 3rd postoperative days followed by recovery on the 7th day. An unusual thrombin inhibitor is described in a patient suffering from liver cirrhosis, associated with a polyclonal gammopathy. In all screening tests that are based on thrombin action, the plasma clotting time was prolonged; in contrast, a test using the thrombin-like enzyme thrombin coagulase was not influenced. Tooth extraction was carried out in 405 patients with various hemostatic disorders due to thrombocytopenia, chronic liver disease, hemophilia A and B, von Willebrand’s disease and oral anticoagulants.
