Platelets from patients with acute myocardial infarction were labeled with 51Cr and reconstituted whole blood. The endothelium of rabbit aorta was removed, and segments of the inverted aorta placed on a Plexiglas rod in a perfusion chamber, and the deposition of platelets on the aorta was calculated by radioactivity measurement. In all 8 patients the deposition of platelets was reduced after ingestion of 800 mg of sulfinpyrazone daily for 4 weeks.

Platelets play an important role in the genesis of thromboembolic episodes frequently observed in patients following cardiac valve replacement. No simple and reliable method as yet exists for quantitating platelet function in vivo. Platelet-specific proteins, serum β -thromboglobulin related antigen and platelet factor 4 as well as thromboxane B2 were measured in 50 healthy subjects and in 100 patients who had cardiac valve replacement at least 6 months previously; these were related to the type, site and number of valves replaced. Highly significant differences were observed in the mean plasma β-thromboglobulin related antigen, platelet factor 4 and thromboxane B2 levels in patients compared with healthy controls. The elevated levels of platelet release products observed 6 months after cardiac valve replacement suggest a continuous activation by the prosthesis. In citrated platelet-rich plasma of eight mammalian species lithium produces species-dependent effects on ADP aggregation ranging from stimulation to inhibition. Raising the extracellular calcium concentration enhanced aggregation of both lithium-stimulated and lithium-inhibited platelets, indicating that the extracellular calcium concentration has no direct relation to the effect of lithium on platelet function. Lithium did not induce changes in total intracellular platelet calcium and magnesium contents. Aggregation of platelets with ionophore A23187 showed the same species-specific lithium effects as aggregation induced by ADP. The data suggest that lithium does not exert its effect on platelets through interference with intra- or extracellular calcium concentrations.

Antithrombin III, factor VIII, fibrinogen and fribrinolytic activity were measured in two groups of long-standing insulin-dependent diabetic subjects (24 with proliferative retinopathy, 24 without detectable retinopathy) and 24 non-diabetic controls. Mean antithrombin III ( ± 1 SD) was 115.9 ( ± 15.1), 109.8 ( ± 18.1) and 101.4% ( ± 12.5), respectively, in the retinopathy, non-retinopathy and control groups. Statistical significance was obtained when comparing the retinopathy and control groups (p < 0.001) and when comparing all 48 diabetics collectively with controls (p < 0.01). Mean factor VIII coagulant activity was 137.5 ( ± 37.0), 126.2 ( ± 58.2) and 97.0% ( ± 38.7), respectively, in the three groups. Again the differences between all diabetics and controls (p < 0.01) were statistically significant. Similar increases were observed for other modalities of factor VIII activity. Fibrinolytic activity was significantly increased in both diabetic groups but fibrinogen levels, although increased, were not statistically different from levels in controls. It is suggested that the observed changes are more likely to be secondary to the development of retinopathy and that the increase in antithrombin III activity is due to an increase in α2-macrog

Antithrombin III (At-III) activity and protein, heparin cofactor activity, anti-factor Xa activity were determined in 200 healthy adults, evenly distributed within age and sex groups, in 60 patients with cerebral thrombosis and in 20 oral contraceptive users. There was a positive correlation between At-III protein and its activities in normal subjects and in patients with cerebral thrombosis, The mean At-III protein and heparin cofactor activity values decreased with age in men and in women of fertile age. The mean values of At-III protein and its activities did not show any variation in the patients with cerebral thrombosis when compared with the normals. In oral contraceptive users a positive correlation was also found between At-III protein and its activities, antifactor Xa activity excepted. The mean antifactor Xa activity value in these women decreased during treatment, whereas the other At-III activities and At-III protein were unchanged.

The heparin-antagonizing effect of human α1-acid glycoprotein (α1-acid GP) was studied by activated partial thromboplastin time (APTT) and by a factor Xa assay for heparin using a chromogenic substrate for factor Xa. The antiheparin effect of α1-acid GP in the APTT system was similar to the effect on heparin thrombin clotting time (HTCT), and the effect in the factor Xa assay system corresponded well with the effect in a heparin cofactor assay system using a chromogenic substrate for thrombin instead of factor Xa. It is concluded that α1-acid GP contributes to the reduced anticoagulant effect of heparin observed in various acute stages of disease, when assayed not only by HTCT but also by so-called ‘global’ coagulation tests, such as the activated partial thromboplast

Factor IX activity (IX:C) and antigen (IX:Ag) as well as factor VII, prothrombin time, Normotest and Thrombotest were determined in 37 haemophiüacs B from 26 kindreds, in 30 carriers of haemophilia B and in 40 healthy subjects. In 15 obligatory carriers from 9 kindreds, in which haemophiliacs did not exhibit IX:Ag in excess of IX:C, IX:C was parallel with IX:Ag, but in 4 from 4 kindreds, in which haemophiliacs exhibit excess of IX:Ag, there was a discrepancy between IX:C and IX:Ag. Among the obligatory carriers 6 from 3 kindreds, in which haemophiliacs did not have IX:Ag in excess of IX:C, showed that the anomaly M is not a sign of genetic variability of haemophilia B
