Abstract:Experimental and clinical data suggest that Ph‐negative myeloid progenitor cells are present, albeit suppressed, in the bone marrow of chronic myeloid leukemia (CML) patients. These residual Ph‐negative cells might, in certain circumstances, regain their proliferative advantage over the leukemic Ph‐positive clone. Treating CML patients with intensive chemotherapy might allow the harvest, in the early phase of recovery, of Ph‐negative stem cells to be used as graft after myeloablative regimen. In our study, 6 CML patients were admitted to a program of autograft with circulating stem cells (CSC) collected after high‐dose (5 or 7 g/m2) cyclophosphamide (HD‐CY) mobilization. All were autografted, using busulphan 16 mg/kg and melphalan 60 mg/m2. As graft, 4 patients received CSC only, while 2 patients were also given bone marrow, as their peripheral blood CFU‐GM yield was unsatisfactory. Two previously alpha‐IFN‐responding patients showed a slow hematologic recovery, but achieved a marked and further reduction of their Ph‐positive metaphases post‐graft. Moreover, in one of them, cytogenetic analyses performed on apheresis product showed a more pronounced reduction of his Ph‐positive metaphases, as compared to bone marrow samples, suggesting a potential purging effect of the

Abstract:Laboratory experiments have demonstrated that tetra‐ and triiodothyronine (T4, T3) enhance hypoxia‐induced erythropoietin (Epo) production. In the present study serum immunoreactive Epo was measured in 29 patients with hyperthyroidism and in 10 patients with hypothyroidism. Epo levels were inversely correlated to the blood haemoglobin concentration [Hb] in both groups of patients. However, Epo levels at given [Hb]were significantly higher in the hyperthyroid state.In vitrostudies confirmed that T4and T3stimulate Epo synthesis in the human liver cell line HepG2. This stimulating effect persisted for at least 1 day after the removal of T4and T3from the cultures. Thus, while thyroidal disorders affect steady‐state levels of circulating Epo, it seems unlikely that thyroid hormones play a major role in abrupt adjustments of Epo production, such as the diurnal ch

Abstract:The reticulocyte count is a good index of erythropoietic activity. Automated methods, mainly flow cytometry, have made the counting easier, more accurate and reproducible. The data presented describe the counting of reticulocytes in 1219 apparently healthy subjects of both sexes who came for a periodic health examination. Their ages varied from 4 to over 60 years. The reticulocytes were counted with a flow cytometer (Sysmex R‐3000) with Auramine‐O as fluorochrome. There was no statistical difference between boys and girls aged 4–19 years. However, the reticulocyte count was significantly higher in men than in women over 20 years of age. The reticulocyte count was affected neither by the menstrual cycle in girls at puberty, nor by oral contraceptives and the menopause in women. No effect of moderate smoking was observed in either sex. Abstract:Iron stores were assessed by measurement of serum ferritin in a population survey comprising 436 randomly selected urban Danish schoolchildren (219 boys, 217 girls) 7 to 17 years old. Median ferritin values were similar in boys and girls 7 to 13 years old, while 14‐ to 17‐year‐old boys had significantly higher values than girls of similar age (p<0.0001). Boys had stable median ferritin values from 7 to 15 years of age, after which a significant rise was found from 16 to 17 years of age (p<0.01). Girls had unchanged median ferritin values from 7 to 11 years of age, after which a significant decrease was found from 12 to 17 years of age (p<0.01). The frequencies of low ferritin values<16 μg/1 (indicating absent iron stores) were: Children 7 to 11 years, 1.0%. Boys 12 to 15 years, 4.5%; 16 to 17 years, 0%. Girls 12 to 15 years, 7.6%; 16 to 17 years, 16.7%. Ferritin levels in the reported 1986 survey were significantly higher than those obtained in a similar survey in the same area in 1979. Abstract:Erythrocytosis is sometimes caused by the presence of haemoglobin (Hb) variants with increased oxygen affinity. Here we describe a β‐globin variant found by isoelectric focusing (IEF) of Hb from a 23‐year‐old Swedish male with moderate erythrocytosis. Amplification of DNA corresponding to the β‐globin gene and subsequent nucleotide sequencing revealed heterozygosity for a GTG&RR; GAG mutation at codon 20, thus suggesting a Val&RR; Glu substitution, which was confirmed at the protein level. This mutation occurs at the same position as Hb Olympia [β20(B2)Val&RR; Met], another variant associated with erythrocytosis due to increased oxygen affinity. Abstract:Anemia is a frequent manifestation of rheumatoid arthritis, with a probably multifactorial etiology. We investigated the effect of peripheral blood mononuclear cell supernatants (PBMCS) from rheumatoid arthritis (RA) patients on hematopoietic colony formationin vitro, by using a methylcellulose assay. PBMCS from patients suppressin vitroerythroid (BFU‐E), mixed‐lineage (CFU‐GEMM) and to a lesser degree granulocyte‐macrophage (CFU‐GM) progenitors. PBMCS from anemic RA patients were more suppressive for BFU‐E than those from non‐anemic patients. Addition of antibodies to tumor‐necrosis factor alpha (TNFα) almost completely reversed the inhibition of BFU‐E and CFU‐GEMM, but had little effect on the CFU‐GM colony formation. Antibodies to interferon‐gamma (IFNγ) and interleukin‐1 (IL‐1) were not effective. Abstract:A novel human myeloma cell line, OH‐2, was established from pleural fluid of a myeloma patient in end stage of the disease. Effects of cytokines on proliferation were analyzed by measuring uptake of3H‐thymidine. Cell surface antigens were detected by flow cytometry. The cell line is dependent on IL‐6 for growth and proliferates in response to TNF. There is synergy between the stimulatory response of TNF and IL‐6. The cells express both the p55 and p75 TNF receptors. Neutralizing anti‐IL‐6 did not inhibit TNF‐mediated proliferation, showing that TNF acts through a pathway that is independent of IL‐6. TNF was more potent than IL‐6 in stimulating the growth of primary myeloma cultures (>99% pure) from the same patient (OH‐2‐PC), indicating that TNF in selected myeloma patients has a growth‐promoting effect equal to IL‐6. Abstract:This study demonstrates that soluble interleukin‐1 receptor and tumor necrosis factor receptor modulate their corresponding cytokine‐induced DNA synthesis of acute myeloblastic leukemia (AML) blasts in a dose‐dependent, bimodal fashion; at lower concentrations they enhanced, while at high concentrations they inhibited, the cytokine‐mediated effects. Furthermore, the concentrations of endogenously produced IL‐1β and TNF‐a were found to be significantly (p<0.01) higher in supernatants of AML cells cultured in the presence of corresponding soluble receptors compared to their levels in supernatants of cells growing in the absence of these molecules. Abstract:Blood samples from 43 infants with Down's syndrome and from 67 with a normal karyotype were screened for the presence of cells positive for platelet glycoproteins GpIb and GpIIIa. There was 1 case of fatal leukaemia and 1 case of transient leukaemia among the Down's syndrome patients, both cases had increased numbers of circulating blood cells positive for platelet glycoprotein markers. These cells were present in small numbers in both the Down's and control groups, especially among stillbirths, and there was no difference in the numbers of such cells found in Down's syndrome patients in comparison with the controls.
