摘要:
THE MANNER in which the gaseous exchange in the lung is accomplished has always been an interesting subject for experimental physiology. The naturally attractive explanation, that the exchange followed the usual gas laws of passage from a higher to a lower pressure. seemed to fail, as the determinations of Bohr showed oxygen tensions in the blood higher than those in the alveoli aid carbon dioxid tensions in the alveoli greater than those in the blood.

WE ARE ALL familiar with the more or less friendly taunt, “The operation was a success, but the patient died”. Of what did the patient die and upon whom rests the responsibility? Barring infection, in which we are not directly interested at present, the patient died of shock, and the responsibility rests largely upon the anesthetist. Infection has been reduced to a minimum and shock is less common than it was, but hundreds of lives are sacrified annually through a failure to recognize the condition of shock in its early stages and to institute prompt and efficient measures to combat it. The ultimate responsibility must always rest with the surgeon, for he is the captain of the ship, but the anesthetist is the pilot, who takes temporary charge of the patients' physical state during the operation and he must be ready to assume the immediate responsibility. If we, as anesthetists, are to receive the fees of specialists, let us see to it that we deserve them, and to deserve them we must have the knowledge to render special service. We are working for greater recognition and privilege, but privilege always means responsibility.

IN CALLING nitrous oxid-oxygen the anesthetic of choice, I shall try to consider it from the stand-points of the patient and his general welfare. I am considering, too, only inhalation anesthesia in this discussion. While local and spinal anesthesia have their definite place, and I use them, especially the latter, in my own practice, these types of anesthesia have never been as generally popular in the United States as in some parts of Europe where they have largely supplanted inhalation anesthesia.

THE NECESSITY of an abundant supply of oxygen during the induction and maintenance of general anesthesia is so patent that there is little justification in stating the fact. But since the final delivery to the cell of sufficient oxygen to supply the chemical needs of the body is subject to many modifying influences, a study of the available supply to the given demand, is both interesting and of the first importance.

THIS PAPER is based, for the most part, on the experiences of the Anesthetic Department of Stanford University Hospital. When we consider nitrous oxid as to its rapidity of action and elimination, the absence of disagreeable after-effects, the lack of degenerative changes following its use, the lessened blood changes, the avoidance of acidosis, the preservation of muscular tone and its flexibility, it would seem to more nearly answer as a safe and satisfactory anesthetic in obstetrics than any other general inhalation anesthetic.

DURING THE PAST few months I have had numerous inquiries regarding the danger of using ethylene mixtures as an anesthetic, owing to the fact that ethylene combined with oxygen or air in suitable proportions forms a mixture which is exploded by an electric spark. In order to be able to have some definite information on this subject, a number of experiments were done in the Department of Pharmacology, University of Toronto, and I will as briefly as possible give you the findings.

MAN IS ALWAYS looking for something better. It is planted in human nature to live in the future. The statesman looks for perfection in international relations as well as in national affairs. The lawyer looks to the future for laws that are not ambiguous, and that the simple man can understand. The manufacturer looks for trades relations that will absorb his output at a profit. The laborer looks for a time when he shall be paid sufficiently for his work to enable him to live and save a little (which seems to us to be in the present and not in the future). And so on throughout the various occupations. The Christian looks forward to the coming of the Lord and His rule in righteousness and justice, when sin, the cause of all man's troubles, shall be abolished. The anesthetist is no exception to this rule, for he, too, looks forward to the discovery of a perfect anesthetic agent and method for its administration. He thinks it, dreams it and works to that end. Each succeeding generation builds on the works of the preceding, and adds its efforts towards advancement—perfection being the hoped for goal. However much has been done in anesthesia, much more lies in the future as reward to the diligent and faithful thinker, observer and worker.

AN ETHER-AIR MIXTURE decreases the blood catalase more than an ether-oxygen mixture during anesthesia. Furthermore if the ether-air mixture be administered and the large decrease in the blood catalase produced, then if oxygen be substituted for air the catalase rapidly returns almost to its original amount. Or if the ether-oxygen mixture be administered first, and the small decrease in catalase produced, then if air be administered instead of oxygen a still further decrease in catalase is brought about.

摘要:
THIS WORK was undertaken to study whether some of the deviations from normal physiological conditions seen in anesthesia might not be influenced by changes in the environment. The present paper is concerned entirely with ether. While it has long been known that anesthetics exhibit a tendency to make animals poikilothermic examples are cited in a recent review of heat regulation1we are still in ignorance of the precise conditions of environment which are optimum for maintaining a normal body temperature under the influence of ether. Furthermore, little has been discovered as regards the concentration of the blood during anesthesia, or how its alteration may concern the welfare of the subject.
