摘要:
This article summarizes the findings of the third review of nursing quality measurement literature commissioned by the American Nurses' Association, discusses implications of the current state of the art of nursing measurement, and suggests directions based upon lessons learned from all 3 reviews. Using both computer and hand searching methods, 326 articles describing measurement of nursing care quality were abstracted. Recommendations based on the analysis of literature include incorporating the Nursing Minimum Data Set into databases; documenting nursing hours and nursing educational preparation; determining appropriate patient outcomes within different settings; continued research identifying nurse-sensitive outcomes; and continued nursing leadership in quality improvement efforts for health care.

摘要:
Pain is a complex disease process. Pain management requires an interdisciplinary rather than a multidisciplinary clinical intervention, interdisciplinary because the factors that may affect pain intensity and duration occur simultaneously. A particular problem exits when measuring outcomes from chronic diseases, disorders, and impairments because neither a cure nor death is a likely outcome. The clinical and psychosocial determinates of pain complicate the assessment of level of pain and the measurement of pain management outcomes. The major obstacle to assessing change from a pain intervention is the lack of a gold standard. Because we have no gold standard for better functional health, our evaluation of the responsiveness of health status measures regarding pain management will continue to be plagued with the question “responsive to what”?

摘要:
Research improves nursing and patient care outcomes. However, program managers and educators search for ways to help the busy bedside nurse recognize clinical problems and use research. Despite the new evidence about pain assessment, wound care, pain-free injections, preoperative fasting, and preoperative shaving, many nurses do not apply this knowledge to their practice. Barriers to using research include a lack of readable reports and role models who support and encourage staff to demonstrate evidence-based practice. One creative solution is the research-based fact sheet with recommendations for practice. This article reviews the literature and outlines a strategy for improving evidence-based practice.

摘要:
The purpose of this project was to assess the value of using the Web to teach older consumers about their role in preventing medical errors. Consumers were recruited from a metropolitan Detroit senior community center. The majority of the participants had little or no experience with the Internet. After being taught how to use the computer and Internet, seniors completed a Web-based intervention on quality health care and medical errors. Seniors increased their knowledge about quality care and their role in preventing medical errors and highly rated the Web sites.

摘要:
When building an expert system that will be acceptable to clinicians in their practice, it is imperative that the knowledge engineer identifies, defines, and describes a clinical problem precisely. This can be accomplished by eliciting private knowledge from expert clinicians or by analyzing public knowledge available in the scientific literature. This study describes a systematic method for examining public knowledge found in health care textbooks and practice guidelines surrounding the concept of oral feeding in premature infants in a neonatal intensive care unit. It includes the development of an instrument for extracting data from these sources to standardize definitions of terminologies. Preliminary results and plans for further analysis are reported. This method can be applied to other clinical problems deemed appropriate for decision support system development.

摘要:
Ever rising patient acuity, new nursing staff practitioners, and cumbersome documentation processes potentially impact quality of care. Nurse managers are challenged to utilize creative strategies to minimize risk potential and improve patient outcomes. One 36-bed medical/surgical/telemetry unit focused on improving competence in clinical assessment and documentation associated with the determinants of STABLE. Education centered on a simple approach based on the use of an acronym to memorize concepts and trigger the recall of 6 key points of care. Evaluation of documentation after pain implementation indicated significant improvement in nursing practice.

Patient satisfaction has become an important indicator to measure the quality of care. Nursing has long used outcome measures to evaluate health care. The study assessed the patient satisfaction of patients from medical and surgical units at a teaching hospital of southern Taiwan. Of the 806 near-discharge patients from medical or surgical units approached to participate in the study, a total of 477 patients returned questionnaires for a response rate of 59%. The total mean score for all the patients' satisfaction score was 4.28 (SD = 0.53). In general it reflected that the patients were satisfied. There was no significant difference between the patient demographic variables, primary nurse's age, marriage, and total working experience on any of the subscales. Results of this study provided evidence that primary nurse's unit working experience can influence patient satisfaction (Pvalue < .05).

摘要:
The purpose of this study was to determine whether there is a relation between job satisfaction and quality of life (QOL) of nurses. The study was based on a sample of 501 nurses. Job satisfaction was measured by means of the Short Form Minnesota Questionnaire and QOL by means of WHOQOL-BRIEF. The results demostrated a positive correlation between job satisfaction and QOL. It was also found that job satisfaction and QOL scores of nurses showed a significant difference according to their age, economic level, marital status, duration of working life, and position at work.
