Continuous quality improvement has resulted in the need for the development of new and exciting skills in group dynamics and in the facilitation of small group interactions. Leaders of quality work groups must use the understanding of group dynamics and leadership to maintain high performance work teams. To be effective, the team leaders and members must be aware of what is being said (content) and how things are being said (processes). Team leaders and groups can not be guaranteed effectiveness. However, development of the necessary skills in team building will ensure the likelihood of effective group processes and positive outcomes.

The health care industry is looking to continuous quality improvement as a process to both improve patient care and promote cost effectiveness. Interdisciplinary teams are learning to work together and to use data-driven problem solving. Humor adds a creative and welcome touch to the process that makes it easier and more fun to work in teams. The team leader or facilitator who uses humor along the journey sanctions the risk-taking behavior that accompanies creative solutions to tough problems.

摘要:
Decentralization of quality assurance activity or total quality management (TQM) is a goal set by many health services. Effective and efficient use of available resources is usually the goal. This article describes the efforts of one organization to decentralize its nursing quality assurance program. Quality circles provided forums for an increasing acceptance by ward-based nurses of their role in establishing quality as central to nursing practice. This emphasis on local group processes has helped staff to work with each other to solve problems related to the work of nursing.

The authors describe a model, Quality in Daily Work (QIDW), which may be used to assist in the implementation of TQM at the unit level. QIDW is patterned after Juran's Trilogy of quality planning, quality control, and quality improvement.

Review of staff performance in the area of pressure sore prevention revealed that nursing practice was adequate. But the staff's belief that it could do a better job provided the impetus for a quality improvement project. Based on research data a planned change was implemented, and followup study reflected positive outcome results. This successful project integrated the concepts of research, cost effectiveness, and patient outcome.

Many health care facilities are developing and implementing critical paths to streamline the provision of quality care. Critical paths are a compilation of multidisciplinary input driven by specific time oriented outcomes. The purpose of this article is to illustrate critical path development, utilizing a multidisciplinary approach in a community hospital setting. The creation of a critical path will be described, from the environment that stimulated interest, to development and implementation of the path. Advantages to the patient and members of the health care team are presented. Emphasis is placed on strategies that promoted and facilitated collaboration among team members.

摘要:
This article describes the development of a quality improvement program for a clinical nurse specialist department reflecting the 10-step process described by the Joint Commission on Accreditation of Healthcare Organizations. The philosophy of continuous improvement has allowed the department to promote concurrent improvements as well as to measure and monitor selected direct and indirect patient care indicators. The department developed process indicators to verify the processes in place. The department anticipates development of outcome indicators for subsequent programs. Identified opportunities for improvement, and the changes which occurred to address those opportunities, are also described. Information regarding data collection related to resource consumption and increased collaboration with other departments, is also described.

A graduate-level course in quality management has been developed through a partnership between the San Francisco State University Department of Nursing and over 80 clinical agencies. The course, required of all master's students and open to nurses in the community, combines the most current information and research in quality management with a major project conducted in a health care setting. This article describes the course objectives, content, and methods, as well as the positive outcomes for students and agencies. The partnership offers an innovative model for agency and university collaboration in the teaching and practice of quality management.

Evaluation is an important aspect of staff development service. However, it is frequently overlooked or superflcially addressed in staff development quality management programs. This article emphasizes the importance of multilevel evaluation techniques to assure quality education and promote effective use of staff development resources. The author suggests the greatest impact in staff development evaluation lies in the ability to demonstrate transference of learning to the bedside.

A multidisciplinary documentation quality team was appointed four years ago with the charter to revise the admission nursing assessment form, eliminate the traditional handwritten nursing care plan, establish a method to utilize standards of care and standards of practice as the basis for individualizing the patient plan of care, establish flexible charting systems for specialty sections, and develop clear and concise guidelines to accompany specialty area documentation systems. This article provides an overview of the project and the outcomes realized through commitment, collaboration, and cooperation.
