AbstractAffinity purified antibodies to human collagen type I and III and antihuman fibronectin, were employed immuno‐histochemically on formalin‐fixed, paraffin‐embedded human tissue containing schistosome granulomata. The distribution of the three connective tissue proteins was studied in 45 biopsies in which there wasSchistosoma haematobium(30) andSchistosoma mansoni(15) infestation. The granulomata were divided into early, intermediate and late according to the proportion of inflammatory cell types and the degree of fibrous tissue deposition. Only fibronectin was detected in the early granuloma where macrophages are the main cell type. In the intermediate granuloma both fibronectin and collagen type III were localized along reticulin positive fibres; spindle cells, (presumptive fibroblasts) and macrophages were the cell types present at this stage. Collagen type I was mainly localized in the late granuloma. Hyalinized connective tissue was negative for the three proteins. The loose connective tissue and walls of blood vessels were constantly stained for fibronectin and collagen type III in normal areas distant from granulomata. AbstractSpecific antisera to the human laminin P1 fragment and the 7S domain of type IV collagen were used to investigate the ultrastructural location of these main basement membrane (BM) components in a dermal cylindroma. Thick frozen sections were treated by the immunoperoxidase method, postfixed, embedded in epon and then sectioned for electron microscopic examination. Laminin and type IV collagen were detected in all layers of the cylindroma BM, together with non‐specifically stained, diffusely distributed fibrillar structures, possible microfibrils and anchoring fibrils or residual stromal collagen fibres. The mixed presence of the BM components indicates that these substances codistribute rather than occurring as separate layers even when forming neoplastic basement membrane

AbstractA series of breast carcinomas of known steroid receptor status have been examined for evidence of binding of the lectins peanut agglutinin, soy bean agglutinin and wheat germ agglutinin. Correlations were found between oestrogen receptor status and reactivity of carcinomas to peanut agglutinin and soy bean agglutinin but these were not absolute. Wheat germ agglutinin binding was unrelated to the presence of oestrogen receptors. No relationship was evident between progestogen receptors and the binding of any lectin. It therefore seems unlikely that lectin histochemistry can replace steroid receptors as markers of hormone dependence in breast carcinomas.

AbstractLung biopsies from 17 patients with cryptogenic fibrosing alveolitis of a cellular rather than fibrotic pattern were examined by transmission electron microscopy in the hope that such cases would show features of pathogenetic significance. Further selection was made by choosing minimally affected areas. There was no ultrastructural evidence of immune complex deposition but alveolar epithelial and capillary damage was frequently found (17 and 14 of the 17 cases respectively). Alveolar epithelial injury consisted of patchy necrosis and regenerative hyperplasia. Alveolar capillary injury consisted of cytoplasmic swelling and basement membrane thickening and reduplication. Many of these features have not been emphasized in previous reports and their prominence in early stages of the disease suggest that they may have pathogenetic significance, possible mechanisms of which are discussed. Similar findings identified during the course of this study in 8 asbestos workers suggest that similar pathogenetic mechanisms may operate in asbestosis.

AbstractRenal biopsy material was examined from 13 patients who had a history of pre‐eclampsia and who were biopsied after delivery. The characteristic ‘endotheliosis’ associated with pre‐eclampsia appeared to resolve at varying rates and was not seen in biopsies taken more than 1 month after delivery. Four of the biopsies showed residual focal segmental glomerular lesions which are described. Focal global glomerular sclerosis (eight biopsies). focal interstitial scarring (seven biopsies) and arteriolo‐sclerosis (nine biopsies) were also identified. AbstractLysozyme is a bacteriocidal enzyme which is a major stable secretory product of mononuclear phagocytes, including hepatic sinusoidal macrophages (HSM), and serves as a good marker for these cells. Patients with alcoholic liver disease (ALD) have decreased HSM function which is reflected in reduced clearance of microorganisms and endotoxin derived from the gut. The HSM population in 54 liver blopsies from patients with ALD was studied using immunoperoxidase staining of lysozyme and was compared with 15 histologically normal controls. In both groups lysozyme positive HSM were more numerous in periportal than perivenous parenchyma. In each zone there were significantly fewer positive HSM in cases of ALD than in controls, in alcoholic hepatitis than in ALD without hepatitis, and in cirrhosis than in ALD without cirrhosis. These findings suggest a decreased population of functionally active HSM in ALD which correlates with severity of liver damage. This might be due to decreased lysozyme content of the entire HSM population or to the existence of two populations, one positive and one negative for lysozyme. The observed decrease in HSM function explains many of the phenomena observed in ALD.

AbstractThe grading of gastric epithelial dysplasia has been studied by means of computer aided morphometry. Measurements of histological features were made on segments of epithelium from 38 selected cases of dysplasia graded by subjective assessment. The measurements were statistically compared with the subjective scores using discriminant analysis. The measurements were found to provide significant discrimination between all groups. Nuclear size proved to be the main discriminating variable. Prediction of the likely group membership of individual cases was possible using classification equations derived from the discriminant analysis. Classification of the original data set revealed prediction errors which suggested a bias against diagnosis of dysplasia. As a result of this study reliable, repeatable and objective gradings of gastric epithelial dysplasia can be obtained by inexperienced persons with an accuracy approaching that of a skilled pathologist.

AbstractMonoclonal antibodies to A, B and H blood group isoantigens (BGI) were used in an indirect immunoperoxidase technique to compare the distribution of BGI expression as a marker of vascular endothelial cells with that of coagulation factor VIII related antigen (F VIII‐RAg), a well established marker for vascular endothelium. Formalin‐fixed and paraffin‐embedded material from 70 specimens representing a wide range of reactive and benign or malignant neoplastic states of the vascular endothelium was used to provide adjacent serial tissue sections to compare directly the tissue expression of BGI and of F VIII‐RAg. In reactive and benign neoplastic conditions of the vascular endothelium, cell membrane expression of the BGI appropriate to the patient's blood group was readily detected. In the more cellular and proliferative areas of some haemangiomas, pyogenic granulomas and cases of angiolymphoid hyperplasia with eosinophilia, BGI proved a superior marker of the endothelial cells compared with F VIII‐RAg. The malignant endothelial cells of 27 specimens of angiosarcoma showed deletion of BGI expression in 24 and of F VIII‐RAg in 19. Expression of neither antigen showed any significant correlation with the degree of tumour differentiation. Some cases of lymphangioma, uniformly negative for F VIII‐RAg staining, showed strong expression of BGI. The possible use of BGI expression as a marker of vascular endothelium is discussed in the light of the known tissue expression
