AbstractAmyloid enhancing factor (AEF) activity has recently been demonstrated in ubiquitin purified from amyloidotic murine tissues and Alzheimer brain extract. Since AEF is known to bind to amyloid fibrils and ‘fibril‐AEF’ on passive transfer induces accelerated amyloidogenesis in the recipient animals, it was of interest to investigate whether ubiquitin binds to amyloid. Immunohistological studies were carried out on liver sections from amyloidotic mice. Biotin–strepavidin–peroxidase methods using monospecific rabbit anti‐mouse AA amyloid IgG (RAAG) and rabbit anti‐bovine ubiquitin IgG (RABU) antibodies were employed to immunostain the amyloid and ubiquitin deposits, respectively. RABU‐treated liver sections were counterstained with thioflavine S. RAAG reacted strongly with the amyloid, indicating that it is AA type, and RABU‐positive immunodeposits were found bound to the thioflavine‐S‐positive AA deposits. Treatment of the liver sections with 0·1Msodium acetate containing 0·5MNaCl, pH 4, for 2–3 h at 37°C nearly completely desorbed the AA amyloid‐bound ubiquitin. Since ubiquitin demonstrates AEF activityin vivoand binds non‐covalently to AA amyloid, we suggest that ubiquitin may indeed be ‘fibril‐AEF’ and may play a crucial role in the pathogenesis of amyloidosis. AbstractPutative sarcomas of follicular dendritic cells are extremely uncommon and only small case numbers have been clearly documented. A major difficulty in their delineation has been the persistent controversy over the phenotype and ontogeny of normal follicular dendritic cells. Two new cases arising in adult females are presented herein, both of which had initially been mistaken for metastases from a true soft tissue sarcoma. Their morphology, immunophenotype, and ultrastructure (in one case) are described in order to aid their wider recognition. In the light of these findings, the nature and origin of follicular dendritic cells are discussed.

AbstractMalignant histiocytic neoplasms of dendritic cell lineage are rare and most are derived from the dendritic cells of the T‐cell and B‐cell areas of the lymph node, respectively. Only one case of a malignant histiocytic neoplasm of Langerhans' cells, the dendritic cell of the skin, has been reported.We report the occurrence of a true histiocytic neoplasm of Langerhans' cell type in a 23‐year‐old female patient. AbstractThe eosinophils from bone granuloma, bone marrow, and peripheral blood of a patient with Hand–Schüller–Christian disease (HSCD) were studied by electron microscopy and cytochemistry. Impressive eosinophil degranulation was observed. Extracellular release of eosinophil peroxidase (EPO) and EPO binding to surrounding cells were seen in the granuloma and bone marrow. Cells with peroxidase‐positive plasma membrane were also observed in peripheral blood. The pattern of eosinophil degranulation showed quite different features from those described so far. In the granuloma, the process begins with intracytoplasmic release of the granule matrix content, as revealed by both extensive extragranular accumulation of EPO and progressive decrease of the matrix electron density. Core dissolution follows thereafter, leading to complete disappearance of the granules. At the end of the process, the cells show rupture of the plasma membrane and release of their content into the surrounding environment. This pattern if secretion was also observed in blood and marrow eosinophils of the patient. AbstractTransmission electron microscopy was used to study the ultrastructure of nucleoli in seven high‐ and seven low‐grade non‐Hodgkin's lymphomas. Nucleoli with a single large fibrillar centre were predominantly seen in the low‐grade group, while the majority of nucleoli in high‐grade lesions contained several small fibrillar centres of approximately equal size. A third type of nucleolar appearance was identified, with a large central fibrillar centre surrounded by numerous small ‘satellite’ fibrillar centres. This nucleolar configuration was the least common but was more often seen in low‐grade than in high‐grade lymphomas. The mean fibrillar centre cross‐sectional area was determined for each group and measured 278 800 nm2for low‐grade non‐Hodgkin's lymphomas and 62 507 nm2for high‐grade lymphomas (P<0·001). AbstractThere is considerable variation in the quoted mean numbers of AgNORS per nucleus for benign melanonaevi and malignant melanomas. This is partly attributable to differnt approaches to AgNOR counting. This study summarizes our experience in devising an optimal technique for counting AgNORs. We show that it is essential to count intranucleolar AgNORs in addition to those lying outside the nucleous to obtain clear separation of naevi from melanoma. Although this seems an onerous task, we further demosnstrate that a maximum of only 30 nuclei need to be counted to obtain a mean AgNOR count per nucleus which is representative of the whole lesion. This compares with the arbitary figure of 100 nuclei chosen by most workers.Only by optimizing and standardizing all aspects of the AgNOR technique including fixation, staining, and counting will mean AgNOR counts per nucleus become a useful quick, reproducible method which can be applied to lesions which pose diagnostic problems such as borderline melanocytic lesions.

AbstractWe have assessed the degree of observer variation of both Breslow depth and Clark's level in a series of 50 thin malignant melanomas. Our findings are similar to those of previous international studies in that Breslow depth is the more reproducible measure. Significant intra‐ and inter‐observer variation exists and in some cases it was up to ±0·86 mm. Even small differences will potentially affect patient management at our centre and this was analysed using kappa statistics. Good agreement was found between observers and this could be improved by comparing the mean of two or more measurements. This removes larger errors, but smaller observer errors and differences in subjective interpretation of the deepest malignant cell mean that agreement will never be more than 90 per cent. This is high compared with studies of observer variation in other pathological conditions, e.g., dysplasia of the cervix, but where surgical management is potentially disfiguring it is not high enough. AbstractA cell‐counting technique and a photographic‐based scoring system for assessing inflammatory cell infiltration were compared for agreement in 275 endoscopic gastric antral biopsies from 55 dyspeptic patients. Each biopsy was independently scored 0–5 for polymorphs and mononuclear cells using both methods, and a mean score was derived for each patient. There was a highly significant correlation between the mean patient scores obtained using each method (P<0·001). Agreement statistics showed that the mean score difference (cell‐counting minus photographic score) was negative for polymorphonuclear and mononuclear cells, indicating that the photographic method yielded higher mean patient scores. Scatterplots of difference versus mean by the two methods showed that only 3·6 per cent of scores fell outside ±2 SD of the mean difference. The use of standard photomicrographs to grade superficial antral gastritis therefore provides accurate results, comparable to those obtained by cell counting, yet is a faster and much cheaper
