AbstractIn an attempt to establish whether extended immuno‐phenotyping allows more accurate definition of subgroups of B‐cell non‐Hodgkin's lymphoma (NHL) we have stained a series of 145 cases with a large panel of monoclonal antibodies that recognize B‐cell differentiation and activation antigens. No antigen was expressed by all cases. The B‐cell histogenesis in many cases could be confirmed only by using a panel of immunoglobulin and pan B‐cell markers. There was marked phenotypic heterogeneity within and between major groups of B‐cell NHL as delineated by the Kiel classification although the differentiation antigens CD5 (lymphocytic and centrocytic NHL) and OKT10 (plasma cell tumours) were more often expressed by certain morphological groups. The activation antigens 4F2 and transferrin receptor were expressed more strongly and more often by high grade NHL but other activation antigens (CD23 and CD25) were not more frequently associated with these tumours. Extended phenotyping may be of value in improving the understanding of biological abnormalities and processes involved in B‐cell NHL, but we conclude that a limited panel of markers (CD3, CD5, CD22, CD45, IgM, kappa, and lambda) should be sufficient for routine diagnosis and classificatio

AbstractThe monoclonal antibody Ki67 recognizes an antigen expressed in all phases of the cell cycle except Go. It has been used in 141 biopsies from 138 patients with non‐Hodgkin's lymphoma to identify proliferating cells in histological sections. A Ki67 index (the number of Ki67 positive tumour cells divided by the sum of Ki67 positive and negative tumour cells) has been derived by counting 1000 cells in each case. A correction for the presence of non‐tumour cells has been incorporated by counting non‐tumour cells in serial sections stained with a panel of other antibodies. A very strong correlation between a low Ki67 index (5 per cent) had a worse survival than those with an index of<5 per cent (p80 per cent) to have a better survival than those with a lower index (<80 per cent). The patients with high grade disease who achieved complete remission or good partial remission and had a Ki67 index of less than 80 per cent were more likely to relapse than those with an index of greater than 80 per cent (p<0·04). These findings could not be explained by the effect of other prognostic factors such as age, stage, or serum albumin. AbstractThe nuclear DNA content of cells from 45 malignant lymphomas and from 60 benign lymph nodes obtained by fine needle aspiration was analysed to investigate the diagnostic value of DNA flow cytometry combined with routine diagnostic cytology in lymphomas.DNA aneuploidy was found in 43 per cent of lymphomas of high grade malignancy (NCI Working Formulation) but only rarely in lymphomas of intermediate‐ or low‐grade malignancy or in Hodgkin's disease, and never in benign lymph nodes. The median percentage of proliferative cells (S+G2/M) was 22·6 per cent in diploid high‐grade lymphomas, 15·3 per cent in intermediate‐, and 8·1 per cent in low‐grade lymphomas, as compared with 4·9 per cent in benign lymph nodes (P<0·0001). If the presence of DNA aneuploidy or more than 12 per cent of proliferative cells is used as a criterion for malignancy, the diagnostic accuracy of DNA flow cytometry in detecting lymphoma is 81 per cent. DNA flow cytometry suggested correct diagnosis in 10 of the 19 false positive, false negative, or indeterminate cytological findings encountered during the study. AbstractA silver colloid technique to identify nucleolar organizer region associated protein (AgNOR) has been applied to 68 cutaneous tumours. Basal cell carcinomas, eccrine tumours, apocrine tumours, and hair follicle tumours had differences in their numbers of AgNORs; these appeared as small black dots in their nuclei. Dermatofibromas and squamous cell carcinomas showed a degree of variability in the number of AgNORs depending on the cellularity of the former and differentiation of the latter. Basal cell carcinomas possessed significantly many more AgNORs per nucleus than the other neoplasms. It is suggested that this technique, previously the province of the cytogeneticist, may be of use in the diagnosis of cutaneous neoplasms.

AbstractThe 3‐fucosylN‐acetyllactosamine residue is the antigen recognized by the monoclonal antibody MC2. Using MC2, we demonstrated the distribution of this antigen in a variety of squamous epithelia. The antigen is expressed to a variable degree on supra‐basal cells in most normal non‐keratinizing squamous mucosae, with a similar distribution in metaplastic squamous epithelia; antibody‐labelled latex microspheres and immunogold electron microscopy show the antigen to form part of the glycocalyx. In dysplastic and neoplastic squamous lesions, expression is reduced or absent except in cells around areas of differentiation. Prior neuraminidase treatment of sections had little effect on the amount or distribution of demonstrable antigen.Expression of this antigen by cells in non‐keratinizing squamous epithelia gives an indication of cell maturity and may provide a histological marker for the grading of dysplastic and malignant squamous mucosal lesions. AbstractThe binding of peroxidase‐conjugatedDolichos biflorus(DBA),Triticum vulgaris(WGA),Canavalia ensiformis(Con A),Arachis hypogaea(PNA),Lotus tetragonolobus, andBandeiraea simplicifoliaI (BSAI) to gastrointestinal carcinoid tumours was studied. The results indicate that carcinoid tumour cells express certain carbohydrates similar to those present in the adjacent surface epithelium. The differences in the lectin‐binding properties of carcinoid tumours of different sites of the gastrointestinal tract are closely related to the regional differences in the lectin binding of adjacent surface epithelium. AbstractHepatocellular carcinoma was induced in rats by administering aflatoxin B1(AFB1) for 6 weeks. Malignant tumours were preceded by foci and nodules of altered hepatocytes of three histological types, composed of basophilic, eosinophilic, and vacuolated cells. In addition, there were areas of altered hepatocytes that were considered as hyperplastic.Lectins were used as histochemical markers to compare the expression of membrane glycoproteins in hepatocellular carcinomas and hepatic nodules with non‐nodular or control hepatocytes. There were marked changes in the lectin‐binding patterns of the hepatocellular carcinoma cells and the eosinophilic nodules. The lectin‐binding patterns of basophilic nodules, vacuolated nodules, and hyperplastic areas were similar to non‐nodular or untreated hepatocytes. The similarity in the lectin‐binding changes of the eosinophilic nodules and hepatocellular carcinomas suggests that the eosinophilic nodules may be an early stage in the development of

AbstractThe lead salt method of Wachstein and Meisel15has been applied using incubation media containing polyvinyl alcohol for the localization and quantification of 5′‐nucleotidase (E.C. 3.1.3.5) activity in cryostat sections from rat liver after ischaemiain vitroand ischaemiain vivofollowed by different periods of re‐perfusion.5′‐Nucleotidase activity at the bile canaliculi, especially in the pericentral areas, had already decreased after 60 min of ischaemiain vitro, although the total activity as measured densitometrically was not changed. After 120–240 min of ischaemia, a significant decrease of the total 5′‐nucleotidase activity was found. At that stage, signs of irreversible cell damage were recognized.Short periods of re‐perfusion (1 h) after ischaemiain vivoinduced a decreased bile canalicular 5′‐nucleotidase activity throughout the entire liver, but a restoration after longer periods of re‐perfusion was observed (5, 24, and 48 h). Necrotic areas recognized by a decreased lactate dehydrogenase activity after all periods of re‐perfusion showed decreased total 5′‐nucleotidase activities.A correlation was observed between the decrease in bile canalicular 5′‐nucleotidase activity and the disappearance of microvilli of the bile canaliculi.It is concluded that a decrease in the bile canalicular 5′‐nucleotidase activity can be used as a very sensitive marker for ischaemic liver cell damage.Assessment of the irreversibility of the cell injury has to be determined using additional parameters such as a dec
