摘要:
Rehabilitation interventions for older persons are largely limited to traditional health care therapies such as physical or occupational therapy. The authors propose a broader conceptualization of geriatric rehabilitation that encompasses rehabilitation interventions that support independ and disability policies providing rehabilitation interventions are reviewed and evaluated. The differences in the continuum of long-term care and rehabilitation services through the overlapping policy objectives of the Rehabilitation Act and the Older Americans Act are contrasted. The dilemma of integrating current aging and disability policies is explored and how existing public policies might be reformed is considered.

Current research shows that for Americans today there is a high probability that both aging and disability will be facts of their personal lives. The question of how we, as a society, will accommodate to these facts is posed in the introduction to this article. It is asserted that answers to this question could be positively affected by a coalition of the aging and disability communities. The first section of the paper. “Strategies for a Coalition,” offers approaches for bringing together aging and disability groups to affect change in general. The second section focuses on the specific issue of Social Security Disability Insurance and work incentives. It offers some options for change upon which a coalition of aging and disability groups could concentrate its efforts.

The Americans with Disabilities Act (ADA) signals an important shift in national policy regarding people with disabilities. The changes required will allow people with disabilities to function more independently and enjoy an improved quality of life. The high rate and severity of communication and other disabilities associated with aging means the ADA can have a significant impact on older Americans. Rehabilitation professionals need to know how the ADA affects their clients and understand their professional roles and responsibilities in order to make the ADA a reality.

Technology and rehabilitative procedures can improve hearing function for the aging adult, thereby improving access to health care and various life activities. The areas of identification, selection, and evaluation of assistive technology are discussed in light of the impact of changes expected in aging adults. The health care provider working with aging adults can be an important source of referral for hearing health care, as well as a user of assistive technology with his or her patients.

摘要:
The Americans with Disabilities Act has given new hope to those who are physically or psychologically challenged. When thinking of accessibility issues, many think only of architectural barriers. Chemical, perceptual, auditory, visual, cognitive, and psychosocial issues as well as physical and functional barriers also demand attention. Multidisciplinary design teams, including persons with disabilities, utilizing human factors methodology offer a new model for creating accessible environments. An accessibility audit for team use is presented. Modification for vision and hearing loss, products, an accessible single-family dwelling, and multi-family federally financed housing are discussed.

Practitioners actively seek out opportunities to enhance and expand their clinical skills. Anticipated changes in the health care system make it imperative for them to expand and broaden their involvement as advocates for quality health care. No matter where they practice, clinicians have not only the knowledge but the ethical obligation to advance the needs of not only their patients but for society as a whole. Advocacy is a viable tool and mechanism that professionals can use to have a positive impact on health care services. This article explores models for proactive advocacy.

The purpose of this retrospective document analysis was to describe the association of age, function, and discharge disposition among 1055 patients (aged 60 years or more) who were referred for physical therapy over a 30-month period in an acute care hospital. Bed mobility, transfer, locomotion, and stair-climbing functions were described by the Functional Independence Measure system. Discharge disposition was described by a five-level ordinal scoring system (0 = death to 4 = home). Patients were assigned to orthopedic and other (nonorthopedic) groups. Age was associated significantly with both discharge disposition and functional scores. The correlations of age with function were higher for the orthopedic group than the other group. Functional scores had higher correlations with discharge disposition than did age. As function is potentially alterable, the findings of this study should be encouraging to professionals involved in the rehabilitation of hospitalized patients.
