The complex nature of urinary incontinence in older adults demands an interdisciplinary approach to treatment. Social and psychological consequences to incontinent behavior and its treatment require the knowledge and specialized skills of several disciplines to achieve maximal patient outcomes. Urinary incontinence in the long-term care setting is a particularly stubborn problem; however, interdisciplinary teams working together using a continence philosophy can help change professional norms and institutional cultures to those of continence promotion. This article addresses several principles in the interdisciplinary treatment of urinary incontinence in older adults and offers an updated continence promotion model.

This article describes the continuing research that expands on the concept of the pelvic floor and treatment of incontinence. The expansion of concept includes the pelvic muscle force field in contrast to the pelvic floor as it relates to treatment of chronic incontinence; the breathing diaphragm, trunk musculature, and intrinsic foot musculature in relation to treatment of chronic incontinence in older adults; and the autonomic nervous system control of smooth and striated muscle in the urogenital and pelvic muscle systems as it relates to treatment of chronic incontinence.

Urinary incontinence is underreported and has tremendous financial, emotional, and physical effects on older adults. Older adults with urinary incontinence who have adequate mobility and mental functioning can be cured or significantly improved with behavioral treatments. Physical therapists are an excellent source for providing treatment for this population. Physical therapy treatment may consist of biofeedback, therapeutic exercise, neuromuscular reeducation, therapeutic activity, or gait training. Physical therapists require specialized training and equipment to perform the treatments.

Urinary incontinence is described with specific reference to distinctions in the older man. An overview of the male urogenital anatomy and the functional pathophysiology of the lower urinary tract are described. Emphasis is placed on voiding dysfunction and incontinence as it relates to prostate disease, its surgical sequellae, and the effects of aging. Treatment focus is specific to behavioral methods to regain control, with particular emphasis on exercise prescription for pelvic floor strengthening.

This article reviews the most common medications currently used in pharmacologic management of urinary incontinence. Medications should be used when necessary after behavioral interventions have been instituted. In cases of overactive bladder or urge incontinence, two new medications have recently been introduced and may be used effectively even in the geriatric population.

This article seeks to explore possible explanations for the effects that dietary and nutritional substances may have on urinary incontinence. A brief discussion of old traditions and new perspectives is provided as a basis for subsequent discussion of the use of nutritional interventions. Changes associated with aging of the kidneys and bladder are reviewed. The importance of hydration is emphasized. Pharmacologic causes of urinary incontinence are discussed, followed by a discussion of nutritional deficiencies and systemic changes that may lead to or exacerbate the condition of incontinence. Food substances that can trigger urinary incontinence are presented with rationale for possible causes of urinary incontinence. Lastly, modifications in the diet and the use of herbal remedies for the treatment of urinary incontinence are presented.

Activities programming in long-term care can be an extension of geriatric rehabilitation if activities use rehabilitation principles. Twenty-five residents of a dementia unit participated in a study comparing effects of Montessori-based activities to regular activities programming. Montessori programming uses rehabilitation principles such as task breakdown, guided repetition, and progressing from simple to complex. It also utilizes principles of dementia interventions such as external cue usage and reliance on implicit memory. Montessori programming elicited more constructive engagement and pleasure and less passive engagement and negative affect than other programming. The implications for collaboration among rehabilitation, recreational, and nursing staff are discussed.
