摘要:
Motivation is an important variable in the recovery process of any older adult, particularly regarding functional activity performance. Unfortunately, motivation is often not evaluated, and the patient is seen as noncompliant, unmotivated, and unwilling to perform. The theory of self-efficacy helps explain motivation and provides guidelines for intervention. Such interventions strengthen self-efficacy and outcome expectations and thereby improve functional activity. The Self-Efficacy for Functional Activities and Outcome Expectations for Functional Activities scales can be used to evaluate efficacy expectations and help practitioners develop a plan of care to improve motivation and behavior.

This article reviews the literature on the role of practice in learning for geriatric individuals. It concentrates on understanding how practice, an often-used method of instruction, affects learning in the older patient from a cognitive standpoint. The article also reviews the need to consider memory, cognition, and learning principles for adults. In addition, a discussion of the internal and external factors that affect practice and provide the most benefit to older patients is included, as well as strategies that support and interfere with learning.

摘要:
Cognitive impairment has long been acknowledged as one of the top three intrinsic risk factors contributing to fall risk in older populations. However, the biologic mechanisms behind declining cognitive function have yet to be determined. The development of more reliable, less invasive, and less costly monitoring devices has made the process much more attainable for evaluation in clinical outpatient settings. This article highlights the potential interrelationships between cognitive testing, cardiovascular and cerebral blood flow, physical activity, and functional status.

Residents in long-term care facilities may lose function due to the adverse effects of deconditioning and learned helplessness. The article describes five educational programs designed to promote mobility and exercise in the everyday activities of caregiving. The programs targeted residents of the facility, direct care staff, and managers. Program formats varied to meet the different needs and schedules of the target audience. The ultimate goal of the programs was to instill a cultural change in the facility that fostered individualized care, reduced learned helplessness, and maximized physical functioning.

This article discusses the role of the geriatric social worker on the inpatient medical rehabilitation unit, in a comprehensive outpatient rehabilitation program, in a geriatric medical clinic, and in the community. The social worker on the inpatient unit provides program coordination, psychosocial assessment, intervention, and patient advocacy. Case examples illustrate the social worker's ability to work with patients across treatment settings to process their losses, find sources of support, reevaluate their lives in light of physical changes, and find meaning in their lives. The importance of social support, the search for meaning, and the use of reminiscence in providing a rehabilitation of the spirit are emphasized.

Neuropsychological assessment has traditionally been used to help diagnose diseases of the central nervous system. In rehabilitation settings, however, such evaluations are conducted to estimate patients' functional abilities, such as ability to live independently. The study examined the ecological validity of neuropsychological measures in terms of their empirical ability to predict competency for independent living in geriatric patients following discharge from a geriatric rehabilitation unit. Results identified neuropsychological measures that discriminated between patients capable and incapable of safely resuming living alone following hospitalization. The article outlines the clinical implication of results, in terms of competency determinations made by psychologists.

The article presents rehabilitation utilization patterns in New York State from 1985/6 through 1998. Data from hospital discharges, certified home health agencies (CHHP), skilled nursing facilities (SNFs), and long-term home health certified programs (LTHHCP) were used to aid understanding of future rehabilitation needs of aging Baby Boomers. Hospital data indicate an 854% increase in discharges with rehabilitation codes for persons 60+. CHHPs provided 134% more rehabilitation visits; LTHHCPs recorded 394% more visits. The percentage of SNF residents receiving restorative rehabilitation increased 9.44% for restorative physical therapy, and 7.64% for restorative occupational therapy, with declines in the percentage receiving non-restorative treatment.
