&NA;Although impairments in sensation and perception are not typically among the most prominent following stroke, they are important determinants of outcome. As such, they merit evaluation and treatment. This article briefly summarizes research literature addressing the evaluation and treatment of these impairments. Although far from adequate, the literature does provide some direction as to how these impairments can be approached.

&NA;While aphasia is not per se a geriatric disorder, risk factors for aphasia increase with age, and effective treatments for older individuals must be sensitive to age‐related needs and circumstances. This article first reviews evaluation tools, with their assessment domains and ways of using them, then moves to treatment approaches, with an eye in particular to describing experiences using advanced treatment programs with older aphasic patients, and concludes with discussions and examples of outcomes analyses that contribute to our understanding of improvements following treatment in older patients.

&NA;Gait restoration after stroke is a major concern of neurological rehabilitation. The article focuses on gait evaluation suitable for clinical purposes, newly derived principles of therapy favoring a task‐specific, repetitive approach, and novel treatment techniques. Treadmill training with partial body weight support, automated gait rehabilitation, functional electrical stimulation, rhythmic auditory cueing, and botulinum toxin treatment of focal spasticity are presented. Further, practical aspects on canes and orthoses complete the article.

&NA;The purpose of this study was to begin to obtain normative data on the physical properties of hand strength and touch‐pressure sensation among community‐dwelling elderly. Grip strength, pinch strength, and touch‐pressure sensation were tested in 82 individuals between the ages of 70 and 98 years from 2 urban senior centers in New York City. The average scores for grip and pinch were stronger for the men than the women; there was a noteworthy decline in hand strength after the age of 82 years for men and women. Touch‐pressure hand sensation scores showed diminished light touch sensation (3.61) for the majority of the subjects in both the end‐stage tests sites innervated by the median or ulnar nerves of the hand regardless of age or gender.

&NA;The purpose of this study was to describe strategies used by community dwelling elderly women to ease standing up from various household surfaces. A convenience sample of 55 community dwelling elderly women participated in this investigation. Volunteers were asked to indicate if they had difficulty rising from 6 household surfaces (dining chair, easy chair, couch, commode, bathtub, and floor) and to describe the strategies they used to facilitate rising from each surface. Their answers were coded and tabulated. The women most frequently reported using upper extremity assistance to ease the task of rising from a dining chair, easy chair, couch, or commode. Subjects revealed that getting on all fours was the primary means by which they facilitated standing up from a tub or the floor.

&NA;In a comparative literature review 51 studies representing a variety of professions and social and ethnic population diversity were analyzed. The effects of chronic pain on the functioning of older adults and the factors associated with the risk of falling were compared to determine areas of overlap. Fifty‐eight percent of the chronic pain studies identified effects that are recognized as risk factors for falls, yet few studies on falls have considered chronic pain as an independent variable. The relationship between chronic pain in the elderly and its potential as a risk factor for falls warrants further study.

&NA;Neuropsychological assessment has traditionally been used to aid in the diagnosis of diseases of the central nervous system. Within a rehabilitation context however, neuropsychological assessment is playing an increasing role in delineating patients' cognitive strengths and weaknesses in order to address applied functional issues. The majority of related research in rehabilitation neuropsychology has been conducted with a young adult population, typically patients with acquired brain injuries. Rehabilitation of the elderly, however, is a quickly growing component of professional rehabilitation services. Given the current aging trend of the population, demand for such services is expected to parallel this growth pattern. At this time, relatively little is known empirically about geriatric rehabilitation programs and the role of neuropsychological assessment forgeriatricrehabilitation applications. Within a context of Continuous Quality Improvement in clinical practice, the present study investigated patterns of neuropsychology referrals on a 36‐bed geriatric rehabilitation unit, with frail and comorbidly complex patients. The prevalence of referrals made to neuropsychology and the specific referral questions posed are outlined, subsequent to tabulation over a 12‐month period. The study further examined the test selection utilized and the extent to which recommendations made by neuropsychologists covaried with test results. Findings revealed that referrals involved 1 or more of 3 issues to be addressed. The most frequent question was related to the patient's competency to live independently. Results further revealed that varying recommendations regarding the level‐of‐care required were associated with significant differences on some test scores, but not others. Score patterns and recommendations made for each referral question are outlined. The discussion addresses the need for further research aimed at identifying predictors of instrumental daily living skills in seniors, and related functional cutoff scores on psychometric tests, in order to facilitate evidence‐based practices in geriatric neuropsychology.
