摘要:
Presbycusis, or hearing loss associated with aging, is common among older adults. It is characterized by a bilateral decrease in hearing sensitivity, particularly at higher frequencies. In addition to difficulty with sound detection, older adults frequently have difficulty understanding speech. The following article discusses changes in structure and function of the auditory system that may occur with age. Recommended clinical procedures for identification and evaluation of hearing impairment are presented. A three-part rehabilitative intervention program is described highlighting the use of sensory aids, treatment of communication problems and counseling needs.

This article provides an overview of the three major types of communication problems that occur after a stroke-aphasia, cognitive-communicative problems associated with right hemisphere damage, and dysarthria. The primary characteristics of each disorder are reviewed. Strategies for facilitating communication that can be used by the interdisciplinary team are provided.

This article reviews speech and voice disorders accompanying idiopathic Parkinson's disease. Data are summarized that document laryngeal, respiratory, articulatory, velopharyngeal, and sensory kinesthesia disorders in this population. Medical and behavioral treatments are identified. The rationale, fundamental concepts, treatment techniques. and outcome data are presented for the Lee Silverman Voice Treatment (LSVT) an effective program for treating speech and voice disorders m individuals with idiopathic Parkinson's disease.

The types of communication disorders found in the treated head and neck cancer population are reviewed. The head and neck cancer populations discussed include those surgically treated as well as those receiving organ preservation treatments of radiotherapy with or without concurrent chemotherapy. Management of speech and voice disorders is reviewed, including surgical, prosthetic, and speech intervention.

Dementia of the Alzheimer's type (DAT) is devastating to the quality of life of individuals with DAT and to the lives of their caregivers. Progressive declines in language and communication isolate individuals with DAT and contribute to caregiver stress and burden. This article describes current communication education and training programs for caregivers of individuals with DAT. A case study involving a spousal caregiver of an individual with DAT is presented to illustrate the components of a comprehensive, individualized caregiver communication enhancement education and training program. Implications for future program development are discussed.

Neurophysiology and neuropharmacology are reviewed, followed by a discussion of how drugs work in the nervous system. The effects of aging on pharmacokinetics, the study of how drugs move in the body, are discussed. Disorders that affect communication of older persons are presented along with a review of the drugs used to manage these disorders. The desired effects as well as side effects of the drugs are addressed.
