摘要:
The “aging imperative” is associated with a rising proportion of older subjects in the population and an increasing prevalence of disability. Assessment of functional status is needed to identify disability early when intervention is likely to be most effective and to monitor the effects of interventions to obtain the best outcomes. Standardized self-report assessments for elders at risk are reanalyzed in this article to produce concise linear measures from ordinal scales. This permits quantification of measurement error while apparently yielding the same clinical information in less time and with less respondent burden.

To address the full scope of work options, medical management and rehabilitation services related to the older worker, it is important to recognize that an active work role can and will extend well into one's 70's. Age discrimination, as a process, will become reversed as the Baby Boom generation becomes the “Vintage” work force. Over the next two decades, the Vintage Work Force will maintain and demand a critical role in the expanding service and information industries. Therefore, the continuation of work will be more a function of job demands accommodating worker capacities, than arriving at particular chronological age of retirement. This chapter introduces the basic issues for the employer and physician in predicting and managing disability related work performance problems for the older worker.

Multiple sclerosis (MS) is the most common progressive neurologic disease of young Americans. Most patients develop their symptoms between ages 20 and 50 years. With contemporary medical management, MS patients live an almost normal life span and can spend their geriatric years combatting their MS along with the symptoms associated with the normal aging process. Instruments such as The Medical Record of Disability have been developed to assess the functional state of the younger patient but do not address the additional problems commonly found in the geriatric population. This article explores some of these areas within the functional systems to alert responsible health care providers to the care for this expanding population of elderly MS patients.

This article discusses the relationship of neuropsychology to rehabilitation for older persons and describes the neuropsychologist as a member of a team of specialists that manages the many aspects of the rehabilitative effort. Concerns of neuropsychology and rehabilitation are compared. Different traditions raise areas of potential conflict, but practical and functional approaches make working together easy. Integration of neuropsychology into the rehabilitation team is crucial because of the prevalence of cognitive dysfunction in the aged population caused by cognitive dysfunction associated with health problems of aging.

摘要:
Objective functional skills assessment gives a rational basis for the revolution in health care. Cardinal numerical data define individual patients' needs and refocuses energy on them, specifies remediable deficits and their treatment, and then evaluates outcome. Not surprisingly, serial functional assessments correlate well with functional status at discharge, and the closely related length of hospital stay. Therefore, serial functional assessments may be used to predict inpatient and outpatient resource utilization. This tool, as sensitive as it is powerful, can change the culture of hospitals and aftercare sites, giving backbone to peer group leaders and even guiding educational reform. Because chronic disabilities are by definition cumulative, it is the fixed-income geriatric patients who will benefit most from the justice inherent in this type of approach.
