With the aging of our population there is an ever increasing number of elderly who need some type of assistance, which is usually provided by caring family members. With cost-cutting measures in health care resulting in shortened terms of treatment, sicker elders are being sent home to already overtaxed families. This means that health professionals who have less time to spend with each patient must, in addition to working with their older patients, also evaluate and support caregivers to care for themselves as well as for their family members. Issues that make caregivers of older adults hidden patients as well as hidden team members are discussed and suggestions for health professionals are offered.

When treating the frail elderly population, clinicians must be cognizant of each of their patients' abilities and limitations. Critical to this awareness is exactly how these patients will function when discharged from their care. In determining exactly how patients will function once discharged, clinicians must determine the state of the caregiving dyad and be aware of its multidimensional nature. An insight into the caregiving dyad is offered, along with frequently used screening/assessment tools, such that astute clinicians may direct the dyad to essential services and education.

It is important for health providers to identify stress in caregivers to geriatric rehabilitation patients in order to prevent or reduce potential adverse physical and emotional health consequences. This article presents results from studies of caregivers to older rehabilitation patients recruited from a rehabilitation hospital, a comprehensive geriatric assessment program, and a social service agency. Results suggest that practitioners may identify at-risk caregivers by assessing stress-related behaviors (eg, psychotropic medication use, weight change). Implications for training health professionals, incorporating assessment and treatment into the clinical assessment of the patient-caregiver dyad, and evaluating the effects of this assessment are discussed.

The more caregivers are involved in caregiving, the more likely they are to experience adverse consequences that may endanger the patient-caregiver dyad. The Patient-Caregiver Functional Unit Scale (PCFUS) measures the stability of the patient-caregiver dyad. Patient dependence in each of 14 ADLs/IADLs and caregiver emotional and/or physical difficulty assisting with each of the ADLs/IADLs is simultaneously assessed. The scale is a reliable and valid instrument and can be used in multiple settings for both clinical and research purposes. The PCFUS also can be used to quantify the stability of the patient-caregiver dyad and can be measured prospectively over time to study change.

Caregivers are often under a great deal of stress while caring for spouses with dementia. When stress increases and becomes overwhelming, caregivers are at risk for developing depression. The primary objective of the present study was to determine which cognitive and behavioral coping strategies are associated with lower levels of depression; once these strategies are identified, interventions can be established to educate these caregivers. Thirty-two spousal caregivers participated in this study. Results of this study indicate that problem-focused coping strategies were associated with a lesser degree of depressive symptomatology, whereas most of the emotion-focused strategies were associated with a greater degree of depressive symptomatology among the present sample of spousal caregivers.

The research examines racial differences in the predictors of caregiver burden among informal caregivers of impaired elders in the African-American and Anglo-American populations. While these populations have several predictors of caregiver burden in common, findings suggest that African-American caregivers perceive less burden while providing more care to impaired elders than do Anglo-American caregivers. In addition to rehabilitative interventions, implications for health professionals include evaluating caregivers for the presence of predictors of caregiver burden; supporting successful strategies observed in each family caregiving network; and assisting caregivers by providing community resources or respite with IADL.

A study of caregiving issues for 51 caregivers of stroke patients in Taiwan investigated the relationship among caregiving burden, functional status, cognitive level of patients, perceived social supports and attitudes toward providing long-term care, gender and type of caregiver, and selected demographic variables. No significant correlations were found between the caregiver burden, functional status, or cognitive level of the patient, although results were in the desired direction. Significant differences were found between spousal and adult child caregivers, between female and male caregivers, and between some pairs of variables. Findings that suggest further research about the dynamics of the Taiwanese family caregiving network are needed.
