Evidence mounts that Tai Chi Chuan (Taijiquan, T'ai Ch'i Ch'uan) and Qigong (Chi Gung, Chi Gong, Chi Kung), a system of physical and mental practice, is a preventative and restorative therapy for elders seeking to reverse or prevent frailty by preventing falls, improving balance, strengthening, enhancing cardiorespiratory fitness, treating arthritis, and eliciting a relaxation response. Knowledge of Chinese internal martial arts, together with clinical biomechanics and psychophysiology, suggests that Tai Chi and Qigong should also benefit some elders who have incontinence or low back pain. The combination of direct and indirect evidence can serve as a springboard for further evaluation of this system by researchers and clinicians.

Experience derived from working as a physical therapist in a prosthetics facility provides the basis of determining whether an older adult with transfemoral amputation is a candidate for a prosthesis. In addition to the client's physical status, the clinician should consider the client's emotional status and previous activity level. Prosthetic selection is based on the client's K level, the functional prognosis defined by Medicare. Training techniques within the clinic and at home are aimed at helping the client meet personal goals as well as the therapist's goals of enabling the client to accomplish daily activities as independently as possible, and become proficient in care of the residual limb and the sound foot.

A mobility training program based on motor control and motor learning theories helps the older adult regain independence at the highest possible level regardless of disability. The concepts underlying this approach and the key elements for individuals with a variety of mobility and stability difficulties are described. The program is goal oriented and the therapist facilitates the patient to select and try various solutions to accomplish a stability or mobility task in a smooth and integrated manner. Prosthetic training exemplifies a mobility training program based on motor control and motor learning theories.

Falls among adults aged 65 and older are a major health problem. Approximately 35% to 40% of community-living elders fall annually. Falling is associated with considerable mortality and morbidity. Falls generally result from an interaction of multiple intrinsic and extrinsic factors. The ability to reduce falls depends on understanding (1) fall preventive strategies, particularly evidence-based strategies, and, most important, on (2) turning evidence-based strategies into “best practices” under real world conditions.

A modified version of the Timed-Up-and-Go Test (TUAG) was tried on a sample of community-based elders with mental retardation and developmental disabilities. Eighteen subjects aged 55 and older were tested under 3 conditions: free walking, walking with a rolling walker, and walking with a personal assistant. Trials were done on a 10-ft walkway; each subject performed 3 trials of each condition. The test failed to distinguish fallers from persons who did not have a history of falls. In addition, subjects had great difficulty using the rolling walker and 14 subjects walked slower with the personal assistant than when walking freely. The Modified Up and Go Test (MUAG) has good reliability but poor validity for this sample. The use of traditional compensatory strategies, such as rolling walkers, should be reconsidered for this population.

In an effort to understand the multifactorial etiology of falls among the well elderly, the interrelationships among anxiety, depression, worry about falling, and falls self-efficacy were explored. A sample of convenience of 61 elders attending senior centers was interviewed to obtain the information necessary to explore the extent to which depression and anxiety were associated with worry about falling and concerns about falling while performing specific activities of daily living. Results indicate a need for clinicians to inquire into the mood states of elderly persons in their care. Variables of anxiety and depression should be included in every falls history, with in-depth follow-up when indicated. Secondary prevention measures should include referral for all appropriate interventions in order to reduce the substantial public health burden that falls and worry about falling imposes.

Accurate documentation of gait training is essential for physical therapists and physical therapist assistants who provide services under Medicare. Gait refers to the manner of walking, whereas ambulation is the ability to walk. Recent government publications note the ways services do not meet Medicare guidelines and are thus not reimbursed as physical therapy. Skilled services may be provided when nonskilled services would be more appropriate. Skilled services may not be medically necessary. Staff purportedly providing skilled services may not have appropriate skill or experience. Maintenance therapy is not skilled therapy. Physical therapy services are considered reasonable and necessary when 4 requirements are met, namely, suitability for the particular diagnosis, need for skilled services, expectation that the condition will improve, and occurring in a reasonable time.
