Vasomotion, the rhythmical contraction and relaxation of the arterioles, is a natural property of the arteriolar microcirculation. It is observed clinically through related flow variations (flux motion) which are detected by laser Doppler flowmetry. It presents two distinct regimens: slow and fast waves, with frequencies of 1–2 and 10–20 cpm, respectively. Both activities are found in normal tissues, and their rate of incidence as well as their magnitude become significantly enhanced during abnormal conditions associated with low blood pressure and hypoperfusion. The effect of buflomedil on postischemic reperfusion injury was studied in the dorsal skin fold chamber preparation of awake hamsters. Microvascular events were investigated in the striated skin muscle by means of intravital fluorescence microscopy prior to 4 h of pressure-induced ischemia and 30 min, 2 and 24 h after reperfusion. In untreated control animals, ischemia and reperfusion provoked marked leukocyte sticking and macromolecular leakage while functional capillary density was reduced. Treatment with buflomedil (3 mg/kg b.w. in 0.3 ml saline, administered as bolus of 0.1 ml 10 min prior to release of ischemia followed by i.v. infusion of 0.2 ml during the first 20 min of reperfusion) significantly reduced leukocyte sticking and macromolecular leakage, while functional capillary density was effectively preserved. No differences in macro- and microhemodynamic parameters were observed between buflomedil-treated and untreated animals. These findings support the concept that activated leukocytes are involved in the microvascular manifestation of reperfusion injury and indicate that leukocyte sticking and its sequelae can be efficiently prevented by treatment with buflomedil.

In vitro studies on the action of buflomedil (BFL) and its derivative CRL 41034 on the polymorphonuclear cells (PMN) has been performed using functional tests and scanning electron microscopy. The two drugs exhibited the same effects. BFL does not change the in vitro chemotaxis of PMN, but exhibits a regulatory effect on ZMS-induced aggregation of these cells. BFL also appeared to decrease superoxide production of PMN, in a dose- and time-dependent way. The cytoskeleton F-actin polymerization, analyzed through the binding of rhodamin-phalloidin, was increased when the total F-actin of the cells was unchanged. When cell extensions were studied morphologically a change in the shape of the pseudopods as well as the general aspect of the PMN (cottonous aspect) was observed as compared to controls. These drug-induced modifications in the shape change may be efficient in adhesion processes. Finally this latter effect and the influence on oxygen metabolite production could be another means of BFL to protect the microvessels during ischemia, in addition to its vasomotion promoting properties.

Analysis of flux motion by the laser Doppler technique in the forefoot of healthy controls and patients with peripheral arterial occlusive disease was performed by establishing computerized frequency histograms. It was found that the low frequency components (1–10 cycles/min) are caused by local vasomotion. The prevalence of high frequency flux waves (15–25 cycles/min) is significantly increased in peripheral ischemia compared to normal conditions. The amount of oxygen actually supplied to the ischemic muscle tissue of patients with intermittent claudication was quantified before and after a standardized pedal ergometric test. Muscle tissue pO2 was measured with micro-platin needle electrodes directly in the lower limb muscles at rest and 3, 10, 20 and 60 min after a 4-min work load. Time-dependent variations in the behavior of pO2 values as well as changes in the shapes of pooled pO2 histograms make it possible to monitor the effect of therapeutic measures. In claudicants (stage lib) with ascertained occlusions or stenosis of the femoral artery or the pelvis region, it was demonstrated that the delayed increase in tissue pO2 after exercise could be improved by the infusion of 400 mg buflomedil. Comparison between the time-related pooled histograms confirmed improvement of oxygen supply under the influence of this drug.

Vasomotion is a potential target for drugs in vascular diseases. In this work, the effects on arteriolar vasomotion of norepinephrine, phentolamine and the vasoactive drug buflomedil were assessed in the hamster skinfold preparation. Results show the strong vaso-kinetic properties of alpha-sympathetic receptor activation by norepinephrine and an expected depressive effect of the alpha-blocker phentolamine. Buflomedil 10–4M enhances vasomotion and arteriolar reactivity and does not interfere with norepinephrine. The mechanism of these properties of buflomedil remains to be investigate

The aim of this work was to evaluate the effect of buflomedil on hemorheologic, hemodynamic and microcirculatory properties in diabetic patients. Patients were 20 non-insulin-dependent diabetics with distal arteriopathy characterized by chronic hypoxia (transcutaneous oxygen pressure = 25.2 ± 4.8 mm Hg). They were randomly assigned to two groups for a double-blind study versus placebo. The treatment consisted of a 4-hour intravenous perfusion of 400 mg of buflomedil or placebo as a daily dose during 7 days. Results showed significant improvements in hemodynamic parameters such as post-occlusive peak flow and time to peak flow at the end of buflomedil treatment. In the same manner, the transcutaneous oxygen pressure was found to be significantly increased after buflomedil treatment, whereas there was no modification by placebo. Indeed, red cell aggregation was significantly less important as a result of buflomedil treatment whilst red cell deformability was found to be significantly increased. These hemorheological improvements make the blood flow redistribution through the microcirculatory network easier and lead ultimately to better tissue oxygenation
