We have developed a rabbit model for intraabdominal candidiasis by the intraperitoneal injection of Candida albicans. The most reliable early predictor of induction of disease was positive paracentesis cultures for C. albicans during the 1st week post-inoculation (p.i.). Cultures of mesenteric and renal abscesses were mean log 6.20 and 3.18 colony-forming units (CFU) per gram tissue, respectively, 1 week p.i., and remained positive for the 21-day p.i. period. Incidence of hematogenous dissemination, judged by chorioretinal cultures, was 69%. Histopathology of involved organs demonstrated suppurative abscesses at 1 week p.i.; lesions became more granulomatous at 2 and 3 weeks p.i.

The action of domperidone is studied by manometry of the lower esophageal sphincter (LES) in healthy volunteers. This antiemetic and gastrokinetic drug is devoid of central nervous side effects because it does not cross the blood-brain barrier. A low dose of this drug, 10 mg i.v., was able to increase the LES pressure to 275% above the basal pressure. This effect was rapid and prolonged and not mediated by gastrin. Because in vitro studies have shown that domperidone inhibits the dopaminergic system, this points to the role of dopaminergic receptors in the regulation of the LES pressure.

In order to test the hypothesis that chronically elevated serum gastrin levels induce hypercalcitoninaemia, we have measured serum calcitonin levels in patients with chronic hypergastrinaemia. Basal serum calcitonin concentrations were found to be elevated in 2 of 23 patients with Zollinger-Ellison syndrome, but not in 16 patients with hypergastrinaemia due to achlorhydria or a single patient with primary hypergastrinaemia of antral origin. Both Zollinger-Ellison patients with hypercalcitoninaemia (0.31 and 0.79 ng/ml) had multiple endocrine adenomatosis type 1, but no medullary carcinoma of the thyroid. The hypercalcitoninaemia in these patients does not seem to be related to multiple endocrine adenomatosis type 1, since serum calcitonin was normal in 12 normogastrinaemic patients with hyperparathyroidism from families with multiple endocrine adenomatosis type 1. We conclude that chronically elevated serum gastrin levels do not result in hypercalcitoninaemia.

Salivary secretions on mechanical stimulation with forced spitting method and on chemical stimulation with 10% citric acid and gastric acid secretions on maximal histamine stimulation were studied in 20 adults with tropical sprue and 20 adult control subjects. It was observed that the chemical method provided a better stimulus than the mechanical one in both groups. The salivary flow rates as well as salivary amylase, pH, sodium and potassium were similar in both groups. Tropical sprue patients, however, had significantly lower maximal acid output as compared to normals. It was concluded that, although tropical sprue patients are known to have widespread damage to alimentary tract mucosa, salivary glands do not seem to be involved.

Young mice aged 4–6 weeks and old mice aged 24–26 weeks were inoculated with standard, single, oral doses of two strains of human pathogenic ‘related’ campylobacters and in vitro small intestinal solute absorption and fluid transfer measured 7 days after inoculation. Young inoculated mice showed significantly lowered D-glucose and D-galactose absorption; in old inoculated mice absorption of these solutes remained unaltered. The effect of oral disodium cromoglycate (1.2 g/day) was compared with placebo in 18 patients with Crohn’s disease using a double-blind cross-over trial. No improvement occurred in symptoms, signs or Crohn’s disease activity index and no change in the frequency of relapses occurred. Na-alginate as well as guar gum inhibit the absorption of a 59Fe-labelled iron dose (360 nmol) from tied-off jejunal segments of either normal or iron-deficient rats. In order to inhibit the absorption of the iron dose by half as compared with normal rats to which ionized iron was administered 1.2–8 mg of guar gum and 8–30 mg Na-alginate was necessary. In iron-deficient rats the highest dose of Na-alginate tested, 100 mg, inhibited the absorption of iron by about 20%; the highest dose of guar gum, 30 mg, inhibited the amount of iron absorbed by about 25%. An artificial diet containing 10% of either guar gum and Na-alginate fed for 3 days inhibited the absorption of iron in normal but not in iron-deficient rats. A study was conducted on a group of 48 patients presenting with jaundice during the course of chronic pancreatitis. Two different patterns of symptoms could be opposed: (1) Short-lasting cholestatic jaundice which is accompanied by pain, but free from pruritus and fever, is linked to a medium consumption of alcohol, occurs in the course of advanced chronic pancreatitis and seems to be the consequence of a stricture of the main bile duct due to pancreatic lesions. (2) Protracted, intense jaundice which is linked to a high alcohol consumption, is encountered among less-advanced cases of chronic pancreatitis, the main bile duct being generally normal. In such a case, a direct toxic effect of alcohol on the liver must be questioned.

Chronic atrophic gastritis and a sessile polyp were diagnosed during gastroscopy of a patient with splenomegaly and portal hypertension. The polyp was removed endoscopically. Histologically, it proved to be an endocrine tumour, which was identified as gastrinoma by immunohistochemistry. Then, hypergastrinemia was found and gastrectomy was performed to ensure complete removal of the tumour. The surgical specimen showed G cell hyperplasia of the antro-pyloric mucosa and severe atrophic gastritis with scattered argyrophil micronodules in the corpus-fundus region. 3 years after this treatment the patient is well.
