Lactoferrin is present in pancreatic juice, and greatly increased concentrations are found in the pancreatic juice of patients with chronic pancreatitis. It is not known whether these high levels of lactoferrin represent a genetically determined defect predisposing to the later development of chronic pancreatitis or are simply a consequence of the disease. In view of the morphological and functional similarities between the pancreatic and parotid glands, we have measured the immunoreactive lactoferrin concentration in pure parotid saliva of 30 patients with chronic calcific pancreatitis, 26 controls, 5 patients with proven pancreatic cancer, 2 patients with Sjögren’s disease and 2 patients with chronic recurrent parotitis. No difference in the lactoferrin concentration was detected between control subjects and patients with chronic pancreatitis or pancreatic cancer. Raised levels were found in the 4 patients with parotid gland disease. These findings suggest that increased lactoferrin secretion is confined to the exocrine pancreas in patients with chronic pancreatitis and is thus probably a phenomenon secondary to the disea

In dispersed acini from dog pancreas, exogenous derivatives of cAMP stimulated the amylase release; 8-Br-cAMP was a more potent stimulant than Bt2-cAMP, and induced a potentiation of the amylase release stimulated by cerulein. Both secretin and vasoactive intestinal peptide (VIP), increased cellular cAMP, with a greater potency for secretin, and supra-maximal concentration of secretin plus VIP increased cellular cAMP to the same degree as that obtained with secretin alone. 125I-VIP-binding studies showed that in dog pancreatic acini there is a 95 % decrease in the number of VIP-preferring receptors with minor changes in receptor affinity as compared to guinea-pig pancreas. The absence of effect between secretin or VIP and cerulein in stimulating amylase release could likely be explained by the low number in VIP-preferring receptors on dog pancreatic acini.

We studied the frequency and composition of circulating immune complexe (CIC) in patients with chronic persistent non-A, non-B (NANB) hepatitis and in convalescen persons after an apparently normal recovery from acute NANB hepatitis. 10 of 16 patient with chronic NANB hepatitis and 5 of 11 convalescent persons after acute NANB hepatiti had CIC as detected by the Raji cell technique. CIC in chronic NANB hepatitis were com posed of IgG, C3, and in 7 of 10 cases also IgM, while in CIC from convalescent persons Ig( and C3 were present, too, but IgM was detected in only 1 case. Viral antigens within the CI( were not detectable in any case while 14 of 16 chronic NANB hepatitis patients were found t < have free virus-associated antigen in serum.

A comparison between cimetidine and tripotassium dicitrato bismuthate liquid (TDB) in the treatment of endoscopically diagnosed duodenal ulcer by single-blind randomized trial in 48 patients has shown 18 (75%) out of 24 patients receiving TDB and 13 (54%) out of 24 patients receiving cimetidine healed after 4 weeks’ therapy. Symptomatic improvement within 1 month was seen in 19 patients on TDB and 18 patients on cimetidine, but did not only occur in patients whose ulcers healed. On follow-up, endoscopically proven relapse occurred within 1 year in 47 % of patients whose ulcers had healed during treatment with TDB in comparison with 60% of those healing on cimetidin

24 gallstone patients were treated with cicloxilic acid, an agent endowed with choleretic activity, at the dose of 240 mg/day for 1 month. 24 comparable patients on placebo treatment acted as controls. Bile lipid composition was determined and the saturation index calculated before and after treatment, on samples collected by duodenal siphonage after caerulein stimulation, in both groups. In the cicloxilic group there was little or no change in bile salts and phospholipids, whereas biliary cholesterol concentration was significantly reduced (p < 0.05) and consequently the lithogenic index lowered (from 1.5 to 1.2, p < 0.01). Cicloxilic acid can have a place in gallstone disease therapy in association with the litholytic bile acids or in the prevention of gallstone formation in high-risk populations.

In an attempt to elucidate the pathophysiological mechanisms of gastric anaphylactic ulcer we measured tritiated thymidine incorporation in gastric cells, their mitotic rate and the degranulation of mast cells in the preulcerous phase. The results showed that there is a highly significant increase in cell turnover and mast cell degranulation in the mucosa of sensitized animals. Changes were found at the site of ovalbumin challenge, where point ulceration occurred within 48–72 h. The tissue culture studies demonstrated that the mucosa of sensitized animals produces significantly more histamine when challenged with ovalbumin than the mucosa of nonsensitized animals. Addition of histamine to the tissue culture medium in which normal gastric mucosa was cultured led to an increase in [3H]-thymidine uptake at low concentration, and a decrease at high concentration. Serotonin added to the culture medium had no significant effect on [3H]-thymidine uptake, but heparin at high concentration was found to have a stimulatory effect. These studies show that anaphylactic gastric ulceration is associated with an increase in mucosal cell turnover, and that the mast cell mediator responsible for this increase may be histamin

Serum total amylase, pancreatic and salivary isoamylase, lipase and trypsin-like immunoreactivity (TLI) were measured in 16 patients with acute pancreatitis, 37 patients with chronic pancreatitis, 11 patients with pancreatic cancer, and 53 control subjects in order to evaluate the relative value of these tests in the diagnosis of pancreatic disease. In acute pancreatitis patients studied within 2 days from the onset of pain all pancreatic enzymes were abnormally high. In chronic pancreatitis patients serum pancreatic isoamylase and TLI were abnormally low in 8 out of 10 patients with severely impaired pancreatic exocrine function, while lipase was abnormally low in 6 patients. During acute exacerbations of the disease elevated levels of pancreatic isoamylase and lipase, but not of TLI, were found in about one third of cases. In patients with pancreatic cancer the pattern of changes in serum pancreatic enzymes was variable since levels within, below and above the normal range were found. The results demonstrate that in acute pancreatitis all serum pancreatic enzymes had the same diagnostic sensitivity, however serum lipase determination is the most convenient because of its simplicity and low cost. In chronic pancreatitis serum pancreatic isoamylase and TLI may be useful in detecting severe pancreatic insufficiency. In pancreatic cancer serum pancreatic enzymes lack diagnostic specificity.

We have measured plasma fibronectin in 28 patients with well-compensated chronic liver disease, 4 patients with non-cirrhotic portal hypertension and 6 patients who have undergone shunt surgery for the relief of portal hypertension and splenectomy. 17 patients with portal hypertension had significantly lower levels of fibronectin (207 ± 54; mean ± SD) compared with 15 patients without portal hypertension (315 ± 75, p < 0.001) and the 6 patients who had undergone shunting and splenectomy (330 ± 66, p < 0.001). This study was designed to determine the influence of acetazolamide, a potent inhibitor of carbonic anhydrase, on the formation of gastric mucosal lesions induced by acidified aspirin (ASA) or absolute ethanol and on gastric cytoprotection induced by prostaglandin E2 (PGE2). Acetazolamide prevented dose-dependently ethanol-induced gastric lesions and this effect was accompanied by an increased biosynthesis of mucosal PGs, indicating that endogenous PGs may be involved in cytoprotection by acetazolamide. This is supported by the finding that acetazolamide failed to affect gastric ulcerations produced by acidified ASA when mucosal PG biosynthesis was almost completely suppressed. Pretreatment with acetazolamide did not influence the protective action of PGE2 on ethanol-induced mucosal lesions and only slightly inhibited the protective effect of PGE2 on ASA-induced gastric ulcerations. This study indicates that: (1) acetazolamide prevents ethanol- but not ASA-induced gastric mucosal lesions probably via stimulation of PG biosynthesis and (2) gastric alkaline secretion, mediated by carbonic anhydrase, is probably not an essential mechanism responsible for this cytoprotection induced by PGE2.

A sulfotransferase activity which catalyzes the transfer of sulfate group from 3´-phosphoadenosine-5´-phosphosulfate to galactosylceramide and triglucosyl monoalkylmonoacylglycerol has been demonstrated in antral and fundic mucosa of normal human stomach. With both types of mucosa maximum activity for sulfation of galactosylceramide was obtained at pH 6.8, whereas the pH optimum for sulfation of triglucosyl monoalkylmonoacylglycerol was 7.8. The reactions were stimulated by the addition of Triton X-100, Mg2+ and F1-. The sulfotransferase activity of fundic mucosa for the synthesis of sulfated glyceroglucolipid was about two times higher than that of antral mucosa, while the enzyme activity for sulfation of glycosphingolipid was similar in both areas of the stomac
