The duodenal aspirates were analyzed for amylase, lipase, trypsin and chymotrypsin contents in healthy patients and in subjects with chronic pancreatitis and pancreatic carcinoma. The duodenal aspirate was obtained in basal conditions and under continuous infusion of 3 U/kg/h of cholecystokinin (GIH). Lipase exhibited the largest increase under cholecystokinin stimulation and the smallest variation coefficient and degree of overlap between control subjects and patients with abnormal pancreas. healthy subjects, cholecystokinin stimulation induced significant deviations from parallelism for the enzyme ratios involving lipase. Such deviations tendedto disappear in patients with pancreatic disease.

In the course of the irritable 12 months, some 213 new patients were seen at the out-patient department of the Gastroenterological Unit of this hospital. Of these, 36 (16.9%) were considered to be suffering from bowel syndrome. The condition was suggested by a typical history of symptoms, essentially negative results of comprehensive laboratory and radiological investigations. A group of 20 was studied through the measurement of activity and pressure changes in the lumen of the distal colon using a water-filled polythene manometric system. The distribution of the abdominal pain is presented and the incidence of emotional factors is given. The essential nature of the follow-up of these cases for continuing confirmation of the diagnosis, and the management of the patient are discussed.

Pure-tone audiometric examinations of 116 non-drinking healthy veterans were compared to those of 96 subjects who consumed more than 10 ounces of pure alcohol per week. Hearing thresholds, compared by age groups, were similar in the non-drinking and drinking groups. No correlation between amount of alcohol consumed and hearing thresholds could be demonstrated in this population selected for absence of alcohol-related impairment of organ-systems other than the special senses.

Sera from patients with neoplastic and non-neoplastic diseases, as well as from full-term newborns and pregnant women were examined for the presence of α-fetoprotein (α-FP) by counterimmunoelectrophoresis. α-FP was detected in primary hepatoma (59%) and gonadal teratoblastoma. Positive results were also obtained in a case of colonic carcinoma metastatic to the liver and in 100% of cord blood sera. Since counter-immunoelectrophoresis is approximately intermediate between double diffusion and radioimmunoassay in terms of degree of sensitivity, it was proposed that this technique should be employed routinely for clinical purposes. The frequency of the hepatitis-associated antigen (HBAg) was determined in the sera of hepatoma patients and of two control groups consisting of healthy blood donors and patients with malignant tumors other than hepatoma. HBAg was present in 1 (4.5%) of the 22 hepatoma sera. By comparison, it was detected in 1 (4%) of the 25 sera belonging to the neoplastic series and in 5 (1.9%) of 950 blood donors. It is suggested that primary hepatoma is not directly related to chronic hepatitis and/or presence of HBAg. When, however, peculiar relationships occur between host and the hepatitis virus, viral hepatitis may progress to liver-cell carcinoma through the intermediate stage of postnecrotic cirrhos

The literature on benign recurrent cholestasis is reviewed, and three personal illustrative cases are briefly described. Benign recurrent cholestasis was familial in 31 % of the total of 39 known cases, and there was a significant male preponderance. The disease consisted in attacks of jaundice and pruritus separated by symptom-free intervals. The age at the time of the first episode ranged from 8 months to 59 years, but in 79% of cases it was under 20 years. The duration of the attacks ranged from 2 days to 24 months. The shortest asymptomatic interval recorded was 1 month, and the longest was 20 years (mean 15 months). During the asymptomatic periods the clinical, laboratory, and morphologic findings were normal. The cause is not known. Treatment is purely symptomatic: it consists in relieving or eliminating pruritus (chiefly by administration of cholestyramine) and in combating the effects of malabsorption. The prognosis for life is good, but there is no known way of shortening the attacks or of reducing their frequency. There is as yet no evidence that any permanent morphologic changes occur in the liver.
