A sulfated glycoprotein (SGP) was isolated from each sample of the saliva collected during a gastric secretory test in 18 patients in two successive periods of 60 and 120 min. The concentration of the SGP in saliva was estimated from the appreciation of three of its constituents; sulfate as an index of the electronegativity of the molecule, fucose and galactose as an index of the carbohydrate moiety. In the first group of patients, the saliva was collected while the sole stimulant was the permanent presence of the gastric tube in the pharynx. A decrease in the concentration of the SGP was observed during the second period indicating exhaustion of the glands under the prolonged action of the stimulant sound. In the second group of patients, stimulation by histamine was superimposed on the influence of the tube during the second period. In this group the decrease in the concentration of the SGP was more marked and associated with an alteration in the molar ratios of sulfate to fucose and galactose pointing to a relative desulfatation.

Formal metabolic studies of deglycyrrhizinised liquorice (2.28 g/day for 21 days) have been performed in two women with gastric ulcer. In neither patient were there any appreciable changes in weight, blood pressure, serum levels or external balance of sodium, potassium and chloride. Both patients showed slight nitrogen retention. These results suggest that if deglycyrrhizinised liquorice is therapeutically active, it must contain an ulcer healing compound without the aldosterone-like activity of liquorice, glycyrrhizinic acid and carbenoxolone.

By means of an original method, the electrical activity of different regions of the digestive tract is determined in dogs, following various types of vagotomies. The recordings were performed in basic conditions and after pharmacodynamic stimulation with acetylcholine, atropine and adrenaline. The researches were completed by biochemical studies of gallbladder bile (dry residues, content in biliary pigments and cholesterol), y by enzymatic studies of pancreatic secretion, stimulated with physostigmine (amylase, trypsin and lipase) and by following the intestinal bacterian germs. The obtained data attest, that only bilateral selective vagotomies reduce to a minimum the postoperative disturbances occurring after total vagotomies (biliary and pancreatic troubles, and intestinal dysbactery). The advantages of using the selective vagotomies as an election method in surgical treatment of the duodenal ulcer disease are thus substantiated.

The acid and pepsin responses to insulin stimulation were studied during 22 insulin tests on 21 vagotomized patients. In 9 tests, no rise in acid or pepsin concentration was observed within the two first hours following insulin stimulation. In 5 tests, whereas the acid response remained negative, a rise in pepsin concentration was observed. In 3 of these tests, pepsin output was also enhanced after insulin. In 8 tests, both acid and pepsin response became positive after the insulin injection. Except in those patients in whom an acid response occurred within 30 min, pepsin response was usually observed quite sooner than the acid response. These results suggest that pepsin secretion in response to insulin provides a more sensitive index of completeness of vagotomy than does acid secretion.

The serum vitamin B12 level was estimated in 100 patients with jaundice due to various causes. The overall incidence of abnormally high values, when compared with values from 100 control subjects, was 16% and there was little difference between the proportion of abnormally high values in the main diagnostic categories. No diagnostic or prognostic value could be attached to an abnormally high result or to a normal result.

Lipase activity, bile salt concentration and intraluminal phase of lipid resorption are investigated in 4 different regions of the dog small intestine. Lipase activity remains constant throughout the first half of the small intestine but drops to low values in the distal small gut. Conjugated bile salts concentration rises progressively from the duodenum until the upper limit of the distal quarter of the small bowel is reached, where it commences to drop until zero is reached at the end of the small bowel. In the duodenum the low pH ( < 6.3) often limits solubilization of hydrolyzed lipids. More optimal conditions occur in the proximal jejunum where most of the lipids are absorbed. The factors governing the intraluminal phase of lipid resorption cover a greater length of intestine than would be necessary for a normal resorption.

In contrast to its effect in animal experiments, biguanide is shown to have no effect on gastric emptying in man. Gastric emptying was assessed by the double sampling test meal 2 h after placebo or 1 g of dimethylbiguanide (Metformin). The test meal consisted of 750 ml of water, either pure or containing 50 or 100 g of glucose. The data confirm that osmolality prevents gastric emptying of water and demonstrate that Metformin does notinfluence this emptying.
