AbstractThe effect of long-term therapy with natural conjugated oestrogens was studied in 119 patients exhibiting a variety of menopausal symptoms and ranging in age from 29 to 70 years. A high percentage of good or excellent response (93%), with complete relief of symptoms, was noted after 6 months on oestrogen therapy. In the patients who have participated for more than one year, 100% showed an excellent response. After 6 months, a marked increase was seen in the number of women with a superficial cell level of 60% or more. Improvement continued to be made throughout the study.A significant lowering of cholesterol and total lipid levels was seen in the 4 groups studied. Increases in phospholipid levels were significant only in the early and surgical menopause groups, and not significant in the typical and late menopausal groups. The changes in lipid levels had no correlation with the age of the subjects.Commonly noted oestrogenic adverse reactions were soreness of the breasts, hypogastric pain and breakthrough bleeding. However, these were mild in all cases and did not necessitate discontinuation of the drug.

SummaryTwo-hundred and ten patients from general practice with a mean age of 75 years were assessed for the signs and symptoms of potassium deficiency, before and after receiving a potassium supplement (‘Slow-K’) 8.06 mEq. three times a day. The assessment included potassium and haemoglobin levels, diet, and symptomatic ratings. Following treatment, 86% of patients showed symptomatic improvement, and there was a highly significant increase in mean potassium levels from 3.8 to 4.4 mEq./l. Interestingly there was a slight but significant increase in mean haemoglobin levels from 12.1 to 12.8 g./100ml. It was not possible, because of the trial design, to correlate conclusively the symptomatic improvement with the therapy.

SummaryThe effectiveness of lorazepam, a new benzodiazepine derivative, in treating Thai patients with anxiety and related neurotic symptoms was assessed in this uncontrolled study of 56 out-patients. A good to excellent improvement was noted in 69.6% of patients and only in 4 patients were the results poor. Optimal dosage appeared to be 2 to 4 mg. per day, and at this dosage lorazepam was well-tolerated by the majority of patients. The only side-effect that occurred with any frequency was initial mild drowsiness.

AbstractThe incidence of failure to take prescribed drugs was investigated in 272 psychiatric patients on amitriptyline, chlorprothixene, flupenthixol, or placebo. In the trial quinine was added to the tablets as tracer. The urinary excretion of quinine was checked by thin layer chromatography. A negative quinine reaction meant that the medication was not being observed.One hundred and eight (40%) patients had a negative quinine reaction, in the majority of cases only on one occasion. A negative reaction was demonstrated in 14% of in-patients and in 45% of out-patients. There was no correlation between a negative quinine reaction on the one hand and diagnosis, drug, duration of treatment, incidence of side-effects, sex, or age on the other. However, the quinine excretion seemed to be related to the therapeutic results. Among patients with endogenous depression on amitriptyline therapy and in patients with reactive depression on placebo there were significantly more positive quinine reactions in those who did not improve than in those who did. Patients treated with sedating drugs showed, however, a significant relationship between a positive quinine reaction and an improvement in their condition. This finding as well as the role of medication failure and the possibilities of eliminating this factor are discussed.

AbstractA new surgical dressing material in sheet form (gelatin 20%, pectin 20%, sodium carboxymethyl cellulose 20%, polyisobutylene 40%) has been tried in the treatment of varicose ulceration, to overcome the difficulties encountered in applying firm pressure continuously to the floor of a deep ulcer. Preliminary results in 15 cases seem to indicate that this may have a place as an adjunct to routine therapy. The effect of the ulcer on the material is also discussed.

SummaryOne hundred and two women were treated for approximately 2 years with a low-dose progestogen-only oral contraceptive preparation (‘Exlutori’) containing 0.5 mg. lynestrenol per tablet. Therapy was continuous, without interruption of the cycle. The contraceptive reliability during the total of 2,421 cycles was 100%; side-effects, including breakthrough bleeding and spotting, were minimal–even in the first cycles of treatment.Serum alkaline phosphatase and SGOT-determinations in each patient after a year's treatment showed that liver function was undisturbed.

SummaryThe current status of medicines regulation and legislation throughout the world is reviewed. The basic task of drug regulatory agencies is to ensure that the medicines available to the physician are sufficiently safe and effective, and that the information supplied on these medicines is adequate and reliable; the final choice of drug remains the physician's task. By examining the way in which regulatory agencies have dealt with different problems it is possible to identify many of the norms which are emerging. By and large these would seem to be reasonable, and it appears unlikely that drug regulation has, in fact, impeded the development of valuable new medicines. Such problems as do arise are due in part to marked differences in national control systems. It is suggested that these problems may be more readily resolved by informal international co-operation than by formalized international control of medicines.

AbstractA multi-centre uncontrolled assessment of tri-potassium di-citrato bismuthate in the treatment of 161 ambulatory peptic ulcer patients (26 gastric, 114 duodenal, and 21 'indefinable’) was carried out under the conditions of general practice. The intensity of pain and vomiting were assessed before and after a 28-day course of treatment, as was the degree of overall symptomatic relief.Full or great symptomatic relief was reported in 96.3% of patients, many of whom had failed previously to respond adequately to antacid or other forms of treatment.
