摘要:
Although the stages of change model holds the promise that health care professionals will be better prepared to provide the type of help the client needs in a particular stage of readiness, I argue that the readiness stages are not ready for widespread practice, particularly with older clients. In fact, these stages may unnecessarily limit the range of strategies that professionals may successfully apply to their clients. I propose that health care professionals identify and offer a repertoire of strategies that are appropriate for most older persons—health education, social support, behavior and psychological management techniques, and referral to community resources—and allow them to choose what they find useful.

摘要:
This article discusses selected health behaviors and how they influence health in old age among Mexican Americans based on the literature and a recent large-scale study of elderly subjects. Particular focus is given to smoking, alcohol consumption, obesity and physical activity, and cancer-screening behaviors. Sociocultural factors influencing these health behaviors include socioeconomic status, language barriers, and level of acculturation into the larger society. Implications for health promotion are also discussed.

摘要:
Physicians can play a major role in improving the oral health status of older Americans. This article reports the results of a pilot project that demonstrates that physicians who incorporate oral health assessment and referral protocols into their daily office routines can increase the use of dental services among older people who have access to dental care. Replication of this effort nationwide could lead to a significant improvement in the oral health status of the elderly population.

The official health promotion goals for the year 2000 in the United States include an emphasis on behavioral and contextual changes to increase the healthy lifespan of Americans. The goals also encompass reducing disparities in health status among different groups, and many of the specific goals address issues pertinent to older people. Limited data on the health status and health behaviors of older Asians and Pacific Islanders contributed to the absence of any goals for this group of elderly people. This article uses 1993 to 1994 data on 213 Korean and 201 non-Hispanic white people age 65 and over who lived in Los Angeles County to compare, by ethnicity and gender, health behaviors that may influence the health of older people. The older Koreans practiced a somewhat higher number of healthy behaviors overall, but smoking cessation for older Korean men and increased exercise for older Korean women were specific areas needing attention. Limited English-speaking ability and educational levels of the older Koreans make it imperative that health promotion activities for older Koreans use community-based organizations that can reach the older Koreans in an effective manner.

The literature on the health behaviors of elderly African Americans rarely looks beyond gender variations. This article attempts to determine if health-related behaviors (ie, regular exercise, cigarette smoking, alcohol consumption, and health care seeking) vary by demographic, sociocultural, and mental health factors in a community sample of 311 men and women 55 years of age and older. The results indicate that over a third of the sample participated in no regular exercise, smoked cigarettes, consumed alcohol, and sought health care from hospitals and clinics. Results of the bivariate analyses revealed several significant relationships between the health behaviors and the nine independent variables. Gender, age group, church attendance, and perceived control were significantly related to three of the four health behaviors. Implications for health promotion efforts targeting elderly African Americans are discussed.

Personal definitions of health are an important link to effective health promotion, especially for elderly people. The degree to which individuals find health care advice relevant is related to their definitions of health, yet few studies exist that invite older people to discuss such definitions in their own words. This descriptive qualitative study used content analysis to examine 78 statements made in eight focus groups. These statements were in response to the question, “What does 'healthy' mean to you?” The 67 participants ranged in age from 67 to 91 years. Despite fairly similar backgrounds, participants used different personal definitions of health. Participants were most likely to rely on activity or attitude definitions of health. African Americans were more likely to discuss attitude definitions than white participants. Suggestions are made for improving the effectiveness of health promotion programs on the basis of personal definitions of health.
