Obsessive compulsive disorder (OCD) is now recognized as a relatively common disorder that often has its onset in childhood. Both behavior modification and newly available serotonin reuptake inhibitors have shown to be beneficial in reducing the primary symptoms of OCD in adults, but behavior therapy has not been well‐studied in children. This paper presents a case of a 13‐year‐old boy admitted to a child psychiatric inpatient service with severe OCD, psychotic features, and a history of Tourette's syndrome. 摘要:
The authors present a systematic evaluation of the implementation of a token economy protocol on a child psychiatric inpatient unit. Unobtrusive observations and staff responses to vignettes that depicted child behavior indicated that staff members did not always distribute tokens as scheduled and were inconsistent in their judgments about which behaviors should be rewarded. The results highlight the need for child psychiatric nurses to monitor systematically the implementation of care for which they are responsible.

摘要:
To ensure that children and adolescents receive quality care, psychiatric nurses must gather assessment data on admission that is accurate and comprehensive. This step of the nursing process is becoming more crucial as hospital stays decrease, requiring nurses to prioritize patient needs quickly and implement plans of care. The authors describe the development of a nursing admission assessment tool designed to collect data systematically on children and adolescents admitted to acute inpatient psychiatric units. Lewin's change theory (1961) is used as a framework for describing the challenges of instituting a new documentation system.
