Nocturnal enuresis and the use of desmopressin: is it helpful? |

SummaryWe assessed the outcome of the use of an enuretic alarm and desmopressin according to conventional guidelines and investigated the reasons for resistance to desmopressin. Children were given a 4 month course using an enuretic alarm if they had not previously used one; 12 out of 21 were dry (57%) after 4 months and one relapsed 1 month later. Those who had previously failed with an alarm or were considered poorly motivated to use it, were given a 4 month course of intranasal desmopressin. Of these 26 children, 10 (38%) were dry at the end of 4 months but only two (7%) remained dry after this was withdrawn. After the initial treatment with alarm or desmopressin, 27 children were still enuretic and attending the clinic. They were shown how to use the alarm and eight also used the dry bed training technique: 15 had become dry after a further 6 months. Of the 12 children who made no response to intranasal desmopressin, nine were given this medication under supervision in hospital; seven of these children still wet the bed despite producing small amounts of concentrated urine overnight. They also had small measured diurnal bladder capacities. We conclude that if a 4 month course with an enuretic alarm is unsuccessful, rather than using desmopressin, the alarm should be continued with relearning and consideration given to additional use of the dry bed training technique. The major factor causing nocturnal enuresis in children is likely to be a small nocturnal bladder capacity.

SummaryFindings from a postal survey of foster carers in a region of Ireland are reported. While a majority of the respondents seem broadly contented in their role, it is clear that some are experiencing difficulties of different kinds. Problems experienced by different minorities of these foster carers included concerns with the level of administrative and professional support, worries about being parted from the child, and problems in the relationship with the child's biological parents. Responses on motivations to foster suggest a high level of altruistic motivation. The possible implication of these and other findings are discussed.

SummaryThe purpose of the study was to investigate parents' experiences of diagnosis of Duchenne muscular dystrophy. Data were collected retrospectively by postal questionnaire from the parents of 158 boys diagnosed at different times and locations and, for some, after lengthy periods of uncertainty. Most parents knew nothing about Duchenne before being given the diagnosis. There was considerable variation in parents' experiences but the only independent predictors of parents' satisfaction with how the diagnosis was given were obtaining the information that they wanted and feeling that they had understood and remembered it. What distinguishes Duchenne from, say, Down syndrome is that doctors are having to deliver a death sentence on a child. Our data show that this can be done in a way that is satisfactory to parents. What is required is empathy and sensitivity to parents' informational and emotional needs.

SummaryThis study examined children's auditory perception of traffic sounds, focusing on identification of vehicle movement. Children in three age groups (5 years, 8 years and 11 years) were presented with recorded sounds of a car approaching, passing and going away from the listener. The auditory perception of movement was identified as a problem area for children, particularly younger children. The number of correctly identified vehicle sounds was found to significantly increase with age. Age differences in some aspects of auditory perception of vehicle movement were more noted than others, identification of an approaching vehicle was the most difficult task for the 5‐year‐olds. The role of the attention‐demanding characteristics of some traffic sounds and their correct identification was discussed, approaching vehicles catching the attention much later in the stimulus array than passing or receding vehicles.
