SummaryA study of post‐assessment default from a preschool child psychiatric service found that about two‐thirds of the sample defaulted (two‐thirds of these immediately after assessment) and, as in other services, most referrals were seen three times or less. Default, as predicted, was not related to sociodemographic factors. In addition, in contrast to previous studies, there was no overall relationship to type or severity of child symptoms nor to maternal mood disorder. However, default was unexpectedly associated with staff inexperience, shorter initial interview duration and type of treatment offered. SummaryEarly intervention programmes have grown rapidly over the last 30 years. Despite numerous attempts to answer the apparently simple question of whether or not early intervention works, results to date have been equivocal. This paper looks at the reasons why this question has not been answered satisfactorily and suggests that this question may not, in fact, be possible to answer in a global sense. It is suggested that questions must be addressed to the evaluation of specific services rather than to the evaluation of early intervention as a whole.

SummaryThe gait patterns of 10 obese and four normal‐weight, prepubertal children were used to evaluate characteristics of varying weight groups. Analyses of temporal and distance parameters were conducted on representative gait cycles at slow, normal and fast speeds of walking. Obese subjects showed longer cycle duration (P<0.001), lower cadence (P<0.001), lower relative velocity (statures/sec) (P<0.001) and a longer stance period (P<0.001) than normal‐weight subjects. Differences were also found in gait symmetry measures, with the obese displaying asymmetry in step length and step factor. In all cases, the right limb was favoured. SummaryStudies of parental satisfaction with the way in which diagnostic information is imparted have generally focused on conditions that are evident from birth. Diagnostic disclosure for disorders in which the onset is later, and less readily identifiable, has received less attention. The present study investigated parents' views of diagnostic procedures for children whose developmental delays only became apparent as they grew older. The degree of satisfaction was related to the type of diagnosis given but 2 weeks after the assessment 87% of parents described themselves as being very or fairly satisfied. However, levels of satisfaction were lower at a later follow‐up than they had been initially. Moreover, several specific aspects of the assessment fell short of the ideal model and parents offered a number of practical and cost‐effective suggestions as to how these could be remedied. SummaryThe diagnosis of children with a spectrum of autistic features, who do not qualify for the diagnosis of classical autism, has increased during the past decade. A case is reported of an 8‐year‐old child originally diagnosed as learning disabled with attention disorders and hyperactivity, who was later diagnosed as having an autistic spectrum disorder when abnormalities of social interaction and play activity became more obvious. The frequency of learning and attention problems in school‐age children may obscure more significant psychiatric pathologies, such as autistic disorders. Improved awareness of disorders of social functioning and play activity in the school‐age child and use of screening tools may lead to earlier detection, definitive diagnosis and treatment for these children. In addition, assessments from multiple sources, i.e. the school, the home and the clinic, are needed in the diagnostic process. Periodic re‐evaluations of the school‐age child with problem behaviour are necessary to insure that more significant pathologies, which were not clear on initial evaluation, are diagnosed at the earliest
