SummaryThe purpose of the study was to assess the feasibility of making measurements of fetal adrenal glands using ultrasound and to determine whether there is any correlation between such measurements and maternal oestriol levels. Serial measurements of fetal adrenal and renal maximum transverse diameters, circumference and area, and maternal plasma and saliva oestriol and progesterone levels were made at 4 weekly intervals from 24 weeks to delivery in 32 normal pregnant women. There was an approximately linear increase in the fetal adrenal and renal measurements that were studied. There was no correlation between these measurements and either plasma or saliva oestriol or progesterone levels, or the saliva oestriol:progesterone ratio, at any specific gestation.

SummaryTestosterone levels have been determined in tissue homo-genates of 47 fetal testes and adrenals during the second trimester of pregnancy. This is at the time when high levels of testosterone are found in amniotic fluid, especially in those pregnancies associated with the presence of a male fetus. The level of testosterone was considerably higher in the testis than the adrenal, but the levels in the two organs were positively correlated (P<000l).The fetal testis seems to respond in a more marked manner to stimulation by HCG during the second trimester of pregnancy than does the fetal ovary.

SummaryThe properties of the patellar tendon reflex were investigated in pregnant patients in the last trimester of pregnancy using a computer assisted timing device. Three groups of patients were studied; normal, hypertensive and‘imminent eclamptic' patients. Delay, degree of extension, time and angular velocity of the reflex were measured in each patient and the three groups compared. No differences were found between the means of the three groups, but some individual‘imminent eclamptic' patients had faster and more exaggerated patellar reflexes which were outside the normal range.

SummaryWe studied six pregnancies occurring in four patients with chronic myeloid leukaemia, diagnosed before pregnancy in one case and during the first pregnancy in three cases. For one patient the first pregnancy ended in spontaneous abortion but a second pregnancy, during which she was treated only by leucapheresis, was essentially uneventful although the baby was born with spina bifida and a small meningomyelocele and talipes. A second patient delivered a normal baby after treatment by leucapheresis alone during pregnancy. A third patient was treated with hydroxyurea from 16 weeks in her first pregnancy and with leucapheresis and hydroxyurea during the second pregnancy. Both babies were normal. A fourth patient was treated with busulphan in the third trimester of pregnancy and subsequently by leucapheresis immediately before delivery of a normal baby. Pregnancy may proceed normally in patients with chronic myeloid leukaemia; leucapheresis may be advisable in the first or second trimester but cytotoxic drugs can usually be administered in the later stages of pregnancy without undue risk to the fetus.

SummaryPlacentas from singleton deliveries of 108 prospectively followed pregnant women, 18 with sickle cell disease (16 SS, 2 SC) 36 with AS and 54 with AA haemoglobin were examined and findings correlated with antepartum Doppler results and neonatal outcome. Systolic/diastolic ratios⩾3 were designated as abnormal. Gross and microscopical examinations of placentas were done by a pathologist blinded to the Doppler results and pregnancy outcome.Results of our study show moderate to severe villous sclerosis as well as intervillous fibrin deposits in 31 per cent (5/16) of SS placentas examined but these were absent in the placentas of SC, AS and AA patients. The same patients with sickle cell (SS) disease demonstrated both abnormal uterine and umbilical systolic/diastolic ratios. Four of these five pregnancies resulted in the births of small for dates babies. In contrast, the nine SS women with normal Doppler studies and the two patients with only an abnormal uterine Doppler had normal or minimal placental findings and normal weight babies.Our data suggest that placental villous sclerosis and intervillous fibrin deposits are associated with abnormal umbilical and abnormal uterine velocimetry in SS and SC individuals.

SummaryA prospective, random allocation study was performed to assess the effect of the length of the latent period and the efficacy of oral prostaglandin E2tablets for induction of labour in women with premature rupture of the membranes at term.Forty-nine women were allocated to early induction of labour (group 1) and 48 to delayed induction (group 2). The median interval from rupture of the membranes to induction was 3–25 hours in group I and 200 hours in group 2. The median interval from rupture of the membranes to delivery was 13–5 hours in group 1 compared to 22–9 hours in group 2. There was no difference between the two groups with respect to gestational age, birth weight and mode of delivery. Twenty-four hours after admission all of group 1 women were delivered compared to only 58 per cent of group 2 women. There were no statistical differences in the incidences of maternal or neonatal infective morbidity between the two groups.These observations suggest that there is no advantage to a prolonged latent period and that oral prostaglandin E2tablets are effective in the induction of labour in these patients.

SummaryA total of 1939 pregnant women were screened for group B p-haemolytic streptococcal colonisation. The colonisation rate was 17–2 per cent. Colonisation was not affected by age or parity and all the husbands were circumcised. The vagina and the urethra were the most commonly colonised site; rectal colonisation was much less frequent. Treatment of colonised women with benzathine-penicillin i.m. at 34 weeks of pregnancy reduced intrapartum colonisation, but had no influence on neonatal transmission. Failure of antepartum treatment to reduce neonatal transmission together with the large number of defaulters in this study makes the intrapartum fast latex agglutination test for group B streptococci desirable.

SummarySerum levels of the stress protein alpha, acid glycoprotein were measured before, during and after labour in 31 patients, 21 of whom were treated for pain with intramuscular pethidine and ten with an epidural block. Levels of the glycoprotein were increased from pre-labour values during late labour, at delivery and postpartum in patients given pethidine. In the group given epidural analgesia, no change was found in alpha, acid glycoprotein concentration during labour but on day 1 postpartum an increase comparable with that in the pethidine series was found. Levels of the glycoprotein in umbilical venous blood taken at delivery were similar in the two groups.

SummaryDelivery by caesarean section has been advocated to minimise the risk of fetal trauma associated with breech presentation. When over 2200 breech deliveries were examined, traumatic intracranial haemorrhage caused ten deaths, all occurring in association with vaginal delivery and most (80 per cent) being associated with prematurity. To reduce the risk of such trauma, caesarean delivery for all premature breech presentations is recommended. This would increase the overall caesarean section rate in our hospital by 0–5 per cent.
