SummaryParticles of pulmonary surfactant can be detected in minute quantities of amniotic fluid; they appear as round birefringent particles of 0.5–3μm diameter with a characteristic cross of polarisation.Two hundred and five samples of amniotic fluid of various gestational ages were examined microscopically under polarised light. The number of birefringent particles observed perμl of amniotic fluid correlated well with the percentage of phosphatidyl-choline in the sample. There was also good agreement with palmitate determinations, surface tension measurements, the lecithin:sphingomyelin ratio and the clinical data.This technique is simple and rapid and does not require counting surfactant particles.

SummaryBloody taps were obtained in 158 out of 5765 (27 per cent) middle trimester amniocenteses reviewed. Of these, 70 (44 per cent) contained measurable fetal blood, and 88 (56 per cent) only maternal blood. The next consecutive clear fluid sample was used as a control for each. Differences were found between bloody taps and controls for the incidence of anterior placentas (70versus40 per cent), more than one insertion attempt (12 per centversusnil), and failures of amniocyte culture (1 7versus6 per cent). Amniotic fluidα-fetoprotein was higher when fetal blood was present, compared with control samples or those containing only maternal blood. We found no difference in pregnancy outcome following a bloody tap at amniocentesis.

SummaryIn a retrospective study of 4300 ultrasound scans for placental localisation 505 patients were described as having‘low-lying’placentas on a scan performed at 18–20 weeks pregnant. When these were divided into two groups with the placenta covering or not covering the cervical os, it was found that only the former group were at significant risk of developing a placenta praevia (41.4 per cent) or of having an antepartum haemorrhage (27.6 per cent). It is therefore suggested that only these cases should be the subject of repeated scanning at 32 weeks to screen for placenta praevia.

SummaryMicro-albuminuria as measured by the albumin/creatinine ratio may be a practical method to detect early renal involvement in hypertensive pregnancies. This ratio was measured in 162 patients; 27 developed proteinuric pregnancy induced hypertension, 72 remained nonproteinuric pregnancy hypertensives and 63 were normotensive throughout. The three clinical categories could be separated prospectively on the basis of their albumin/ creatinine ratios (P<0.00001) although there was some overlap. Twenty-nine per cent of patients with nonproteinuric and 89 per cent of patients with proteinuric pregnancy induced hypertension had an albumin/ creatinine ratio of greater than 3.5 mg/mmol but 95 per cent of women with normotensive pregnancies had an albumin/creatinine ratios of less than this. The ratio did not predict low birth weight or intra-uterine growth retardation in hypertensive patients. No correlation was found between urate and albumin/creatinine ratio and no relationship with platelet count and liver function tests. There is a spectrum of disease activity in pregnancy induced hypertension ranging from absence of microalbuminuria to gross proteinuria.

SummaryIn order to investigate the influence of pregnancy on the activation and degranulation of polymorphonuclear leucocytesin vivo, lactoferrin levels and neutrophil elastase bound toα1-proteinase inhibitor were determined in plasma samples of pregnant women and patients with pregnancy induced hypertension. The median plasma lactoferrin andα1-proteinase inhibitor bound elastase levels were higher in pregnant than in non-pregnant females (300versus200 ng/ml for lactoferrin and 92versus70 ng/ml for the proteinase inhibitor elastase;P<0.001 in each case). The median plasma levels in pregnancy hypertension were higher than in normal pregnancy (670versus300 ng/ml for lactoferrin and 180versus92 ng/ml for the proteinase inhibitor elastase;P<0.001 in each case). These data indicate that there is activation and degranulation of polymorphonuclear leucocytes in pregnancy induced hypertension.

SummaryThe reproductive performance of 112 women with homozygous sickle cell (SS) disease, treated at the King Faisal University Hospital in Saudi Arabia, between January 1982 and December 1988 was analysed. Of the 230 pregnancies that occurred, 37 (16 per cent) ended in spontaneous abortion. The major maternal complications in continuing pregnancies were anaemia (86 per cent), sickling crisis (34 per cent), infection (37 per cent) and severe pre-eclampsia (7 per cent). Eclampsia developed in four patients. Intra-uterine growth retardation occurred in 21 per cent of babies. The perinatal mortality rate was 72.5/1000 and stillbirths accounted for 64 per cent of perinatal deaths. There was no maternal death in the series.

SummaryLiterature on the management and outcome of the macrosomic fetus is scanty and conflicting. In a retrospective review of 199 consecutive macrosomic deliveries clinical antenatal detection was poor. The incidence of caesarean section was increased but there was negligible neonatal morbidity. The results do not support a policy of elective caesarean section or routine admission to special care baby units for observation.
