SummaryPredictive values of serum CA125 and serum progesterone were evaluated in 152 pregnant women with threatened abortion before 7 weeks menstrual age. Eighty patients carried to term and 72 aborted. The predictive values for abortion of serum CA 125 and serum progesterone were 79 and 77 per cent, respectively. When combined, the predictive value for abortion of serum CA 125 and serum progesterone was 93 per cent. The predictive values for successful outcome of pregnancy of serum CA 125 and serum progesterone were 83 and 64 per cent, respectively, and increased to 90 per cent when both measurements were considered. The prognosis of patients with threatened abortion can be predicted before 7 weeks menstrual age by using serum CA 125 and progesterone determinations.

SummaryPregnancy outcome in women with threatened abortion was compared to that in a control group. Major complications such as prematurity, premature rupture of fetal membranes, antepartum haemorrhage, pregnancy induced hypertension, asphyxia and neonatal death were more common following threatened abortion. Fetal outcome was poorer when spontaneous vaginal bleeding occurred before 14 weeks. The incidence of congenital malformations was not significantly increased. Threatened abortion is a risk factor for poor fetal outcome. In this group of patients, prematurity is the main poor outcome predicted. There is need to monitor patients after a threatened abortion to minimise these complications.

SummaryLevels of CA125 were measured in maternal serum and matched samples of amniotic fluid and extraembryonic coelomic fluid obtained by transvaginal ultrasound guided amniocentesis from 22 women 8 to i 12 weeks pregnant. The levels of CA 125 were much higher in amniotic fluid (median 496u/ml; range 207–1285 u/ml) than in extra-embryonic coelomic fluid (median 35 u/ml; range<35–239 u/ml;P<0–0001; Mann-WhitneyUtest) and maternal serum (median 35 u/ml; range<35–70 u/ml;P<0–0001). There was a linear correlation between gestational age and amniotic fluid CA 125 levels (r = 0–53;P<0–05), but no such relationship with CA 125 in coelomic fluid or maternal serum.

SummaryThe role of gonorrhoea and other sexually transmitted diseases as risk factors for adverse pregnancy outcome defined as low birth weight, stillbirth and postpartum endometritis, was examined in a case control study in Nairobi, Kenya. Maternal gonococcal infection was associated with very low birth weight babies (<1500 g; 17–1versus5–8 per cent;P<0–01; odds ratio 3–2) and with postpartum endometritis (18–2versus5–6 per cent; P =<0–001; odds ratio 41). The isolation rate ofNeisseria gonorrhoeaeincreased with declining birth weight (p<0–05).In addition, an association was found between maternal gonococcal infection and cervical mucopus postpartum (15–5versus3–9 per cent;P=<0–001; odds ratio 4–6). The sensitivity of a yellow cervical swab was 62 per cent and the specificity 73 per cent. In settings where laboratory facilities are not available, empirical treatment may be implemented based on the finding of cervical mucopus.

SummaryDuring 8 years, we managed 70 patients with eclampsia. Sixty per cent were primigravidae and the mean maternal age was 23–1±6–5 (s.d.) years. The preferred protocol for management was an infusion of diazepam with or without magnesium sulphate, intravenous hydralazine and prompt delivery by caesarean section. The perinatal mortality rate was 16 per cent and there were four maternal deaths.

SummaryMoyamoya disease is diagnosed on the angiographic findings of intracranial carotid artery stenosis or occlusion and a fine network of collateral vessels at the base of the brain. It commonly presents as a subarachnoid haemorrhage in a teenager or young adult. Of ten reports of pregnancy complicated by moyamoya disease, five patients suffered intracranial haemorrhage during pregnancy. In only two patients were there no changes in their neurological status. None of the neurological complications were associated with hypertension. The outcome for the patient appears to be the same if delivery is by caesarean section or by assisted vaoinal delivery.

SummaryFifty gram oral glucose tolerance tests performed for recognised indications in pregnancy were analysed retrospectively to assess the contribution of glucose metabolism to birth weight. Women classified as abnormal, who required a diabetic diet or insulin, were excluded. Plasma glucose results from 3 hour oral glucose tolerance tests performed on the remaining 423 pregnant women were analysed in conjunction with maternal age, parity, weight and height, gestational age at delivery and fetal sex. In these non-diabetic women, an increasing difference in blood glucose levels between the first and second hour was found to have the highest association with increasing birth weight. These findings suggest that the efficiency of the removal of the glucose from the circulation is the most important factor in glucose metabolism influencing birth weight in non-diabetic women.
