SummaryThe effects of a competitive inhibitor of 3β-hydroxysteroid dehydrogenase (3β-HSD), Epostane (Sterling Winthrop) on serum progesterone, oestradiol and Cortisol have been studied in twenty women who were awaiting termination of pregnancy and thirteen women in the luteal phase (days 19-21) of their menstrual cycle. In the pregnant women there was a highly significant decline in the mean serum progesterone concentration to 13-Onmol/litre (19 percent of the pretreatment value) after 4 days of treatment. A similar fall in serum oestradiol was observed. Progressive uterine activity did not occur in association with these changes. In the luteal phase study there was also a significant decline in serum progesterone to 13 per cent of the pretreatment value and this was accompanied by uterine bleeding, in 6 of 8 patients. The mean serum oestradiol concentration did not change. This treatment did not produce any adverse side effects and the mean serum Cortisol concentrations in both studies did not differ from those of the controls. Epostane is an effective inhibitor of placental and ovarian 3β-HSD, and may have an important clinical role as an interceptive agent.

SummaryAspiration biopsy is currently the most popular technique for obtaining chorionic villi for early prenatal diagnosis. Various sizes of syringes have been suggested to provide the necessary negative pressure, but no guide line is available to indicate which is an appropriate size. Differences between pressures and pressure profiles for the 2 and 20 ml syringes were examined in bothin vitroandin vivosituations.In vitrotesting showed the 2 ml syringe created one-quarter (125mmHg) the negative pressures found with the 20 m] syringe (525mmHg) in our system. Pressures during aspiration biopsy were half to two-thirds those recorded for thein vitrosituation when either the 2 or 20 ml syringe was used. The clinical implications of these findings are discussed.

SummarySecond trimester amniotic fluid myoglobin levels were determined in 50 normal, 15 bloodstained, 11 meconium stained and 7 haemoglobin pigmented samples. In addition 26 unstained amniotic fluid samples obtained in the second trimester from patients with fetal abnormalities were assayed for myoglobin levels. These comprised 5 anencephalics, 5 severe open spina bifidas, 2 small covered spina bifidas, 7 chromosome disorders, 2 skeletal malformations and 5 spontaneous abortions.

SummaryRoutine real-time ultrasound scans in early pregnancy have detected five patients in whom the fetus had cystic spaces in the cervical region. In four cases the fetus had a chromosome anomaly. Although this has been reported before, we feel that the cystic spaces are an indication for amniocentesis, to examine the karyotype and theαfetoprotein levels in the amniotic fluid.

SummaryMaternal oestriol levels in blood and urine reflect feto-placental function. Very low levels usually indicate fetal death or adrenal hypoplasia (idiopathic or due to anencephaly), placental sulphatase deficiency, maternal corticosteroid treatment or Cushing's disease. High levels are seen in multiple pregnancy and in virilising congenital adrenal hyperplasia (Cathroet al., 1969).

SummaryNinety-eight growth retarded babies born in a peripheral district general hospital without routine ultrasound screening were analysed. Routine obstetric practice failed to diagnose antenatally 65 per cent of dysmature babies. The perinatal mortality and morbidity in babies that were diagnosed antenatally was better than those not diagnosed (P<0-05). It is concluded that the current antenatal practice for detecting fetal growth retardation is inadequate.

SummaryA long birth spacing of at least 10 years has been reviewed in 48 patients delivered since 1978. Of the 99 previous pregnancies only one had been by caesarean section; 9 of the 48 current pregnancies were delivered by caesarean section. The pregnancy was desired in 37 per cent of the patients and related to lack of contraceptive advice in another 37 per cent. Antenatal, labour and delivery complications have not changed from previous reports in the literature.

SummaryHypnosis induced by their husbands was used by eight women to control pain during childbirth. The degree of pain experienced during childbirth, as assessed by a linear analogue scale, was significantly less (P<0-01) in this group (median: 6-3 arbitrary units) than in a comparative group matched for age, social class, parity and length of labour (median: 9-2 arbitrary units). We suggest this technique of using husbands as delivery room hypnotists to overcome the logistic problems inherent in other hypnotic techniques.
