SummaryIn this prospective study, abruptio placentae and associated haematological and nutritional factors were examined at three large university hospitals in Karachi from January 1990 to December 1992. The incidence of abruptio placentae was 5.0 per cent amongst poor uneducated women and 0.5 per cent amongst women of middle or high socio-economic class. Ninety-five patients with placental abruption who delivered at the Jinnah Postgraduate Medical Centre and Civil Hospital, Karachi, were compared with 150 controls matched for age and parity delivering over the same time. Antenatal anaemia (haemoglobin<10 g/dl) was found in 67 cases (71 per cent) and 52 controls (33 per cent; P<0.001). Iron deficiency anaemia predominated, with a strong association with placental abruption (P<0.001). Protein deficiency was present in 47 per cent of cases and 24 per cent of controls (P<0.05).

SummaryWe measured maternal serum placental and placental-like alkaline phosphatase (FLAP) immunoreactivity in 33 cases of Down's syndrome in the second trimester and compared levels with those from 300 normal pregnancies. Using non-linear regression to calculate the normal gestation specific median, the median for FLAP in cases was 1.2 multiples of the median compared with 1.0 for controls (P<0.001). The effect appeared to be greater with increasing gestational age. On average FLAP levels are elevated in second trimester pregnancies affected by Down's syndrome. However, the use of FLAP measurements for screening did not prove as effective as other established tests such as human chorionic gonadotrophin.

SummaryA retrospective 10 year survey (1983–1992) to determine the incidence and clinical importance of positive syphilis serology tests in pregnancy was performed in Coventry. The incidence was 1/4400. Antenatal screening identified 9 mothers of whom 5 had previously unknown acquired syphilis and 4 were known and had been previously treated. A further 2 patients were diagnosed after delivery.

SummaryEighty deliveries in which a combination of vacuum extractor and forceps procedures was used for delivery were analysed retrospectively. The previous vacuum extractor delivery and the previous forceps delivery in the record books were used as controls. The vacuum extractor—forceps deliveries were due to a vacuum extractor failure in 76 per cent of the cases and deliberate use to draw down the fetal head and permit a low forceps delivery in the remaining 24 per cent. In the vacuum extractor-forceps group there were increased incidences of post-dates pregnancies, induced labour, difficult applications and malrotations. Mean birth weight and mean newborn head circumference were higher than in the controls. The perinatal results were poorer (5 minute Apgar scores<7 in 6 per cent of cases versus 1 per cent in the control cases; mean umbilical cord pH 7.15±0.08 (s.d.) versus 7.20±0.10 and 7.22±0.08). Admissions to the neonatal unit occurred in 21 per cent of the newborn delivered with the vacuum extractor and forceps but fetal birth trauma was not increased. Maternal morbidity was low. We advise against the use of the vacuum extractor-forceps combination because of the higher perinatal risk.

SummaryTwo hundred primigravid women delivered vaginally were allocated at random to one of two groups. Patients in one group were to undergo episiotomy unless it was considered unnecessary (elective). Those in the other group were not to undergo episiotomy unless it was considered to be essential (selective). The resultant episiotomy rates were 83 and 53 per cent respectively. An intact perineum was more common among those allocated not to undergo routine episiotomy but these patients had more second degree perineal lacerations (8 per cent). No difference was found between the two groups in either paraurethral lacerations or lateral vaginal wall lacerations. We favour selective use of episiotomy in primigravid patients.
