SummaryPlacental development in 120 normal pregnancies was studied by a new ultrasound tissue characterisation technique using back-scatter signal analysis and amplitude processing. The placental signal was related to that from the maternal abdominal wall to standardise recordings and permit between and within patient comparisons. Colour coding was used as a visual prompt to identify change.The technique permits non-invasive study of normal and abnormal tissue development,in utero; colour coding highlights areas of placental change and pathology which may be missed on grey scale imaging. The back-scatter signal can be quantified to monitor progressive tissue development.

SummaryCervical stromal biopsies were taken in non-pregnant and pre-and postmenopausal women; and from women in early and late pregnancy. Hydroxyproline and water content were determined. There was no variation in collagen or water content during the menstrual cycle and combined oral contraceptive agents did not influence these factors. The use of depot medroxy-progesterone acetate was associated with a considerable reduction in collagen content and an increase in water content similar to that seen in late pregnancy. A lesser similar change was seen in early pregnancy and a more marked decrease in collagen in biopsies taken at the time of delivery. Postmenopausal tissue from the cervix had a higher collagen and lower water content.

SummaryDuring a 2-year period 636 pregnancies with serumα-fetoprotein at or above the 95th centile were followed to outcome in three South Wales districts. Amniocentesis was performed on 200 women and of these pregnancies, 3·0 per cent resulted in spontaneous abortion. In those women who did not have amniocentesis the spontaneous abortion rate was 4·6 per cent. From these results amniocentesis in pregnancies with raised serumα-fetoprotein does not appear to increase the risk of spontaneous abortion.

SummaryTerbutaline was infused i.v. into third trimester pregnant women for 30 minutes to relax the uterus before external cephalic version. Blood samples were drawn at 0, 30, 60, 120 and 240 min after the infusion. Plasma noradrenaline had increased markedly at the end of the terbutaline infusion and there were reductions in growth hormone and thyroid stimulating hormone. Sixty minutes after the start of the infusion, triiodothyronine (T3) concentrations were decreased. These hormone changes were reversed by the end of the observation period. No terbutaline or noradrenaline induced fluctuations in 15-keto-13, 14-dihydro-PGF2αor prolactin concentrations were found. Maternal growth hormone levels did not vary much after the initial reduction due to the infusion. The episodic release of growth hormone that is seen in non-pregnant women may be absent in pregnancy.

SummaryA 4 year retrospective review was performed of 124 consecutive cases of antepartum haemorrhage from 21974 deliveries, an incidence of 0·56 per cent. Placenta praevia and placental abruption were present in respectively 1 in 600 and 1 in 470 pregnancies, while the incidence of an indeterminate cause of bleeding was 1 in 550 deliveries. A history of induced first trimester abortion was found to be associated with placenta praevia. The higher incidence of the latter with advancing age and parity and the association of hypertensive disorders in pregnancy with an increased risk of abruption were confirmed. In cases of abruptio placentae, when vaginal delivery was not imminent, caesarean section reduced fetal loss.

SummarySerum osteocalcin was lower in pregnant mothers than in age matched non-pregnant women (mean±s.d. 2·7±0·8versus6·5±2·2 ng/ml;P<0·001). Osteocalcin levels were increased in the newborn (19·0±4·0 ng/ml;P<0·001). No correlation was found between the maternal and newborn values, between osteocalcin and the baby weight, or with gestational age between 38 and 41 weeks. There were no sex related differences in newborn osteocalcin levels. These changes cannot be the result of modification in calciotropic hormones occurring at delivery. They may he explained by differences in the bone turnover of mother and child.

SummaryIn a prospective study to assess the value of ultrasound in the prediction of secondary postpartum haemorrhage, 272 vaginally delivered patients had ultrasound examinations of the uterus on the third day postpartum. Five cases of secondary postpartum haemorrhage were correctly predicted. There was one false positive result. If the uterine cavity was empty or contained only blood clot, secondary postpartum haemorrhage requiring surgical evacuation did not occur.
