SummaryOne hundred and twenty-four patients were transferred antenatally to the regional referral centre for neonatal intensive care at Hammersmith Hospital. A total of 142 babies were born to these patients. The transfers were indicated by preterm labour, severe pre-eclampsia or antepartum fetal distress. The main obstetric problems encountered were the inefficiency ofβ-sympathomimetic drugs in arresting preterm labour in some cases, and the difficulty of timing uterine preterm delivery in cases of severe pre-eclampsia and intra-uterine fetal growth retardation. The uncorrected perinatal mortality in this high risk group was 232/1000 total births.

SummaryEvaluation of the contribution of serum HCG measurement to the diagnosis and management of early pregnancy and its disorders gave the following results:1.Estimation of gestational age. Due to the steep slope of the HCG curve in early pregnancy, the day of implantation can be extrapolated, within 95 per cent error limits ranging from 7 to 9 days, from a single HCG concentration determined before the ninth week of pregnancy.2.Threatened abortion. The predictive value of serum HCG, measured within 48 hours of the onset of vaginal bleeding, was assessed in 471 patients. The cut-off level for an abnormal value was taken at mean - 2 s.d. The positive predictive accuracy of a single serum HCG determination was 97 per cent.3.Ectopic pregnancy. In all but 2 of 45 cases of ectopic pregnancy the serum HCG concentration was below normal, at least at the time of operation.

SummaryThe improved performance offered by modern ultrasound scanners allows the secondary yolk sac, which develops during the second week of intra-uterine life, to be visualised in a high proportion of cases. The yolk sac usually presents as a strongly echogenic spherical structure, about 5 mm in diameter, which bears no constant relationship to the fetal body. Its ultrasonic appearance is typical, but it may be mistaken for part of the fetal body, for a second fetus or for a congenital malformation. The main clinical value of ultrasonic visualisation of a secondary yolk sac is that it nearly always indicates the presence within the same gestation sac of a living fetus.

SummaryThe screening programme involved routine antenatal estimation of serum a-fetoprotein, followed by a repeat estimation by an ultrasound examination, and by amniocentesis, where indicated. In no case was a normal pregnancy terminated. Thirty-two patients had pregnancies terminated for fetal neural tube defects and one for exomphalos, as a result of the programme. Four patients with normal serumα-fetoprotein had fetuses with neural tube defects.The running costs of the programme were about fl per test, using privately obtained reagents rather than commercial kits.

SummaryA study of 6 non-pregnant healthy women and 6 healthy women in their last 4 weeks of pregnancy has been undertaken to determine whether venepuncture or attendance at a hospital clinic caused any difference in the levels of prolactin in serum. Neither stimulus caused a consistent change in the hormone concentrations of any patient; in the pregnant women prolactin concentrations fluctuated during the 2-hour test period so that a single venous blood sample was not as representative of their basal status as it would be for a non-pregnant individual. It is suggested that in pregnant women for whom a prolactin determination may have clinical relevance, such as in the monitoring of those with suspected pituitary adenomas, about four venous blood samples should be obtained over 2 hours and the hormone concentration determined on the pooled serum.

SummarySpontaneous premature rupture of the membranes was associated with more than one-third of preterm deliveries in the John Radcliffe Hospital, Oxford, over an 8-month period. The prevalence of bacterial colonisation of the vagina, the placenta and the neonate and of inflammatory cell infiltration of the placenta and membranes was studied in 68 patients (39 delivered between 24 and 37 weeks and 29 after 37 weeks) and the findings were compared with those in 30 patients undergoing elective amniotomy at term. Bacterial colonisation of the neonates had occurred in 41 per cent in the preterm group compared with 28 and 23 per cent respectively in the two groups at term; inflammatory cell infiltration of the placenta was present in 33 per cent in the preterm group compared with 21 and 16 per cent in the term groups. There was poor correlation between bacterial colonisation of the vagina, placenta and neonate and chorioamnionitis. Although 7 of 42 babies in the preterm group died, only one death was associated with septicaemia. The major hazard to the neonate after spontaneous rupture of the membranes was preterm delivery, not infection. It is likely that both bacterial colonisation and inflammation of the placenta may be a consequence rather than a cause of spontaneous rupture of the membranes.

SummaryDamage can be caused to the fetal scalp by the application of a spiral electrode in routine fetal heart rate monitoring. In a prospective study of 192 randomly selected mothers with low-risk labours, complications occurred in 4 babies (2 per cent). In the neonatal period these babies had excessive scalp bruising surrounding an area of skin damage and subcutaneous necrosis where the spiral electrode had been applied. The scalp bruising had apparently contributed to severe jaundice, which was treated by phototherapy.

SummaryFive cases of conjoined twins were delivered in Coventry in the years 1960–1979. Only one case was correctly diagnosed antenatally, and one set was surgically separated and are surviving. In a further case, the twins were phenotypically of different sex, with one fetus being anencephalic.
