SummaryThe influence of foreknowledge of gestational age on the measurement of fundal height was investigated. Measurements of fundal height without foreknowledge of gestational age were, on average, 0-5 cm less than where gestational age was known, and the correlation with expected fundal height was lower. Fundal heights measured without foreknowledge of gestational age were smaller than expected (P<0001), whereas no difference was observed when gestational age was known. The implications for screening for intra-uterine growth retardation are discussed.

SummaryThe objective was to audit the activity of the North Staffordshire Maternity Hospital Obstetric Flying Squad using a retrospective case note and register review, covering 1986-1990.There were 132 calls, a rate of 4:1000 deliveries; all were domiciliary and the majority were within 5 miles of the hospital. Only 22 calls were professionally initiated. Bleeding was the commonest problem, though this was estimated to be less than 100 ml in 80 per cent. In 92 per cent of instances no action was necessary other than setting up an intravenous infusion and transporting the patient back to hospital.The current use of the obstetric flying squad to respond to domiciliary obstetric emergencies is redundant and usually inappropriate.

SummaryThe accuracy of glucose tolerance tests, glycaemic profiles and sonographic measurements at 32 weeks in the antenatal diagnosis of fetal macrosomia was evaluated in 38 euglycaemic and 30 gestational diabetic pregnant women.The predictive value of positive tests was low (glucose tolerance 17 per cent, glycaemic profile 16 per cent, ultrasound measurements 27 per cent), but each of them was useful in excluding macrosomia, with predictive values of negative tests of 76, 74 and 81 per cent respectively.The combination of all three tests led to an increase in sensitivity (71 per cent), but had a substantially unchanged predictive value (positive 19 per cent, negative 71 per cent).

SummaryThe purpose of this study was to determine the differences in labour outcome between primigravidae and multigravidae in a twin pregnancy. The course and outcome of labour were analysed by parity in all twin pregnancies delivered in the years 1988/1989. The most dramatic difference was in the incidence of caesarean section, which was 57 per cent (n = 46) in primigravidae compared with 18 per cent (n=123) in multigravidae(P<0005).Of the 15 caesarean deliveries for dystocia, 13 were in primigravidae. Five of the 6 caesarean deliveries for a second twin were in multigravidae. The mean duration of the first stage of labour was 5-5 hours in primigravidae and 2-3 hours in multigravidae (;P<0-001). The mean duration of the second stage was 25 minutes in primigravidae and 11 minutes in multigravidae (P<0001). As in singleton pregnancies, the course of labour in twin pregnancies differs between primigravidae and multigravidae.

SummaryFifty-nine carefully supervised obstetric patients, mean age 32-3 years, range 22-44, mean parity 5-2, range 4-13, who had had between 4 and 7 previous lower segment caesarean sections (mean 4-4) had elective abdominal deliveries at 38 weeks from 1972 to 1989 inclusive. There were no complications and in particular no cases of ruptured uterus were found.During the same time period, there were 6 proved cases of ruptured uterus, 4 after one previous caesarean section and 2 after 2. Their mean age was 33-1 (range 23-38), mean parity 5-8 (range 1-11). All these patients were in labour when their scar rupture was found.
