SummaryTwo hundred and fifty-six women were questioned regarding their lower urinary tract symptoms in early pregnancy. In addition 44 completed frequency charts and 30 measured their fluid intake and urine output. We found that increased voiding occurred in early pregnancy in 91 per cent of women. Black women suffered less diurnal frequency but worse nocturia than white women. Nocturia appeared to be caused by a decreased functional bladder capacity and increased number of hours spent asleep. With respect to stress incontinence, we found a greater prevalence in white parous women compared to white nulliparous women but there was no such difference in the black group.

SummaryForty consecutive cardiolipin antibody positive sera were identified and the case notes of the corresponding patients reviewed. Thirteen of these patients were females without systemic lupus erythematosus who had been pregnant at least once. Five (38 per cent) of them had had proteinuric pre-eclampsia in first pregnancy. There was also a high frequency of history of miscarriage in these patients. A review of their known clinical and laboratory features failed to reveal characteristics which could be confidently used to predict cardiolipin antibody positivity in apparently healthy patients, and we suggest that routine antenatal screening for cardiolipin antibodies may be justified.

SummaryWe have shown previously that fasting plasma glucose correlates positively with birth weight, and is higher in Asian than in European mothers. We therefore studied this further in 97 Europeans and 30 Asians at 28 weeks of pregnancy by means of an oral 75 g glucose load with measurement of glucose, hormones and related metabolites. Asian mothers had a higher plasma glucose (P<0–05 at both fasting and 2 hours) and insulin(P<0001 at both 2 and 4 hours) which suggested that they had a greater degree of insulin resistance. Also, in Asians, plasma glucose correlated with plasma non-esterified fatty acid(P<0–001) and triglyceride (P<0–01), whilst in Europeans it was plasma insulin that correlated with these lipids (p<0005 andP<0–05respectively); non-esterified fatty acid and triglyceride may therefore play a part in the development of insulin resistance. Plasma glucose and insulin were unrelated to placental lactogen.The birth weight and fatness of the Asian babies correlated significantly with 2 hour maternal glucose and insulin, fasting non-esterified fatty acid and triglyceride and 4 hour blood pyruvate, whilst Europeans’birth weight correlated with 2 hour glucose. None of the measurements proved sufficiently sensitive or specific for predicting the small baby in an individual pregnancy. The fatter Asian mother who maintained a higher blood glucose level achieved a birth weight comparable with that of the European.

SummaryBacteriological examinations were made of 100 women and their babies. Three groups were studied; those in preterm labour, and 2 control groups, threatened preterm labour and uncomplicated mature labour. Chorio-amnionitis was present in 25 per cent of the preterm labour patients, which was the only group to present with clinical signs. Unlike other reports, this study did not demonstrate a high colonisation rate ofChlamydia, Ureaplasma urealyticumandMycoplasma. This may be a reflection of the study population and the low incidence of preterm labour in this district. Sexual activity during pregnancy showed no significant correlation with preterm deliveries, premature rupture of membranes, the bacteriological findings or the presence of histological or clinical chorio-amnionitis.

SummaryA retrospective controlled study of 57 unplanned births, occurring at home,en routeor at non-maternity units is reported. Twelve per cent of the women had not been booked for hospital delivery and 20 per cent were booked at other hospitals. Similar numbers of hospital births matched for age, parity, ethnic origin, presentation and gestational age were studied as controls.There were 4 perinatal deaths out of 58 babies in the study group (69/1000) compared to 2 deaths in the controls (35/1000; 18/1000 corrected for one congenitally abnormal baby). The hospital perinatal mortality rate was 9/1000 for all patients delivered at the hospital during the study period. Breech presentation seemed to contribute an increased risk with birth before arrival.Factors contributing to unplanned births include mul-tiparity, women below age 20, unmarried, unskilled and unbooked.

SummaryWomen presenting at term with strong, regular, distressing contractions who were not established in labour (cervix>3 cm dilated) and who did not become established in labour within 12 hours were described as having‘spurious labour'. Compared with controls they were more likely to be primigravidae (P<005). Compared with other primigravidae meconium staining of the amniotic fluid was more common(P<0–001) and forceps delivery was more common (P<005). Caesarean section and low Apgar scores were no more common than in controls.

SummaryThe effect of a delay between obtaining an umbilical cord blood sample and analysing its pH, PO2anddPco2was studied. Delays were investigated both in a double clamped section of cord and in a capped syringe, and venous and arterial samples were considered separately. Venous samples were found to yield less variable results. Significant changes were found in the venous PCO2with storage in both the cord and a syringe.
