SummarySummaryThe aims of this study were to examine the presence of abnormal detrusor activity in pregnancy and to document changes which occurred after the pregnancy was over. Pregnant women were assessed urodynamically during pregnancy and postpartum. The changing prevalence of abnormal detrusor activity was assessed. We found a significant difference antenatally and postpartum. The data presented here demonstrate that abnormal detrusor activity occurring in pregnancy tends to recover postpartum.

SummarySummaryResidual bladder volumes in 216 women, following spontaneous vertex delivery, were measured by ultrasound. Of these cases, 39 women had asymptomatic residual bladder volumes exceeding 50 ml on the first or second postpartum day. The factors associated with significant residual bladder volume were evaluated using multiple logistic regression analysis. Maternal age. height, parity, gestation, duration of first stage of labour, fetal macrosomia, and the presence or absence of perineal trauma did not contribute significantly to the logistic model. A longer duration of second stage of labour increased the odds of having residual volume>50 ml (odds ratio = 2–63; 95 per cent confidence interval = 1-19-5-85;P= 0–017) after adjustment for confounding due to intrapartum epidural analgesia. The effect of epidural analgesia was not significant (odds ratio = 1–90; 95 per cent confidence interval = 0-90-4-02;P= 0–092). Avoidance of prolonged maternal pushing during the second stage of labour by active management of labour may result in a reduction in the incidence of significant residual bladder volume postnatally.

SummarySummaryGrandmultipara are now uncommon in Western obstetric populations, and the approach to antenatal and intrapartum care in such cases is likely to be dictated by past teaching in which they would uniformly be classified as‘high risk’. Such an approach is in danger of becoming self sustaining for obstetric and medicolegal reasons. This modern retrospective study of a large population(n= 1590) of grandmultipara receiving a high standard of antenatal care provides evidence to confound this view and support a low intervention approach. A 4–8 per cent rate of primary caesarean section was achieved, with a low overall rate of only 74 per cent. An antenatal diagnosis of hypertension or diabetes and a birthweight of>4 kg were all strongly associated with an increased likelihood of abdominal delivery, and could serve as risk factors in clinical judgement.

SummarySummaryBetween March 1992 and November 1994, 48 patients with intrauterine death had labour induced, 36 with the prostaglandin analogue misoprostol and 12 with the prostaglandin dinoprostone. Both were used as a single dose in 24 hours intravaginally. The patients were all of low parity and all but one were in the third trimester. Of those getting misoprostol, 92 per cent, and of those getting dinoprostone, 67 per cent, went into spontaneous labour. Eighty-one per cent of those getting misoprostol and 67 per cent of those getting dinoprostone delivered within 24 hours without the need for further prostaglandin or oxytocin, a difference that was not significant. No differences were found in other outcome variables such as diagnosis or induction to delivery time or need for oxytocin, between the two groups. Only one maternal medical complication occurred, chorioamnionitis in a patient who had misoprostol, and in whom labour was delayed 6 days.

SummarySummaryThe causes of perinatal mortality in the University of Benin Teaching Hospital, Nigeria, 1987 to 1989, is reviewed. The perinatal mortality rates for 1987, 1988 and 1989 were 44-0, 62-7 and 71-9 per 1000, respectively. Stillbirths contributed over 50 per cent of the perinatal deaths. The perinatal mortality for unbooked patients was eight times that of the booked patients(P<0–001). The major causes were intrapartum asphyxia (20-6 per cent), prematurity (19-4 per cent) and abruptio placentae (13-9 per cent). The causes of perinatal death was unknown in 19-4 per cent. The perinatal mortality in Benin in 1989 was about six times the average in European countries for 1984. Socioeconomic factors are presumed to have played a role in the high perinatal mortality in Benin.

SummarySummaryA number of specific infections are known to cause intrauterine death in both animals and pregnant women. However amongst these, leptospirosis has seldom been recognised in this context. We describe the case of a 24 year old woman who assisted in a milking parlour on a dairy farm close to London, UK and developed leptospirosis which resulted in intrauterine death. Her initial symptoms were those of a flu like illness and two weeks after the onset of her illness at 23 weeks gestation, an intrauterine foetal death was confirmed by ultrasound. Serology demonstrated a positive IgM for leptospira at 1/160 and a microscopic agglutination antibody titre was positive at 1/5120. The infecting sera wasLeptospira hardjo. This case highlights the important consequences of leptospirosis in pregnancy and the risks of contracting the disease from cattle.

SummarySummarySevere oligohydramnios diagnosed at the time of a routine 18 week anomaly ultrasound scan is managed in a variety of ways. This is because the accuracy of ultrasound diagnosis is impaired due to the loss of the acoustic window (liquor amnii) leading to both false positive and negative diagnoses. However, although the overall prognosis is poor, survival is possible. We present four case reports which serve to highlight management inconsistency and make recommendations on how diagnosis may be improved and management facilitated.

SummarySummaryUsing a retrospective approach, twin deliveries between 1989 and 1994 at this hospital were divided into three groups. 18 cases where prostaglandin had been used to induce labour, 18 controls matched with the first group for various factors where no prostaglandin had been used, and 63 remaining cases.There were few differences in outcome between the groups. Further intervention, in particular, artificial rupture of the membranes, with or without oxytocin, was more common in group 1, 61 per cent, compared with groups 2 and 3, 23-5 per cent (p<0 01).

SummaryIntroductionOgilvie's syndrome or acute colonic pseudoobstruction is a rare but potentially lethal complication in the obstetric patient who presents with large bowel obstruction. It is more commonly seen after delivery by caesarean section. Treatment is initially conservative but surgical decompression may be required. If serious sequelae are to be avoided, early diagnosis and careful clinical monitoring of the condition is required.
