SummaryThe present study describes the incidence of meiotic nondisjunction in 460 families with Down's syndrome offspring. The data are further broken down into meiosis I and meiosis II and correlated with specific parental age groups. In 22 per cent of the cases, the extra chromosome 21 was paternal in origin while the remaining 78.0 per cent of cases were due to maternal non-disjunction. A significantly higher incidence of the disjunctional event occurred in meiosis I (78.7 per cent), while second division failure was found in only 21.3 per cent of cases. There was no correlation between parental age and frequency of non-disjunctional events. Our studies revealed a higher incidence of non-disjunction among younger fathers (<35 years). The probability risk factor for Down's syndrome should be revised, since the current risk figures are based solely on maternal age. Nondisjunction in fathers is presently not well documented. The present data will be useful for genetic counsellors, medical geneticists and physicians in evaluating high risk populations.

SummaryOf 677 women who had an ultrasound examination at the booking visit by a senior house officer (junior resident) in obstetrics, 11 (1·6%) had non-viable pregnancies; three sets of twins were correctly detected. of these women, 655 had a detailed ultrasound examination at 18–20 weeks pregnant. No anomalies were detected at either scan. of the booking scans 95·1 per cent correctly assessed gestational age when compared to the detailed scan (±1 week), but below 12 weeks pregnant the risk of error was increased.

SummaryOne hundred and one pregnant women with hypertensive disorders were prospectively studied with doppler ultrasound flow velocity waveforms of the umbilical artery. Results were withheld from the obstetricians so that the test did not influence obstetric management. It was found that the resistance index of the umbilical artery was higher in patients with proteinuric pregnancy induced hypertension and in patients with chronic hypertension than in patients with non-proteinuric pregnancy induced hypertension. Patients with an abnormal resistance index were more likely to be delivered prematurely, have small for dates babies, have fulminating hypertension and have an abnormal antepartum cardiotocogram. Thirteen patients had a pathological antenatal cardiotocogram and they all had an abnormal umbilical artery flow velocity wave form when the cardiotocogram was normal.

SummarySixty patients and 32 nurses with Crohn's disease and 16 patients and 19 nurses with ulcerative colitis were asked to assess an information booklet about pregnancy and inflammatory bowel disease. of the 127 patients, 102 accepted this invitation; 87 of these found the booklet helpful. Booklets were the most favoured form for the presentation of this information and patients believed that they should be available at medical and antenatal clinics.

SummaryFollowing a cluster of patients with acute appendicitis in pregnancy in our unit, appendicitis in pregnancy over 6 1/2 years was reviewed. of 11 patients who underwent appendicectomy 8 had the diagnosis of appendicitis confirmed. The incidence of appendicitis in the pregnant population was 1:1642. The clustering of cases in late 1985 and early 1986 is unexplained. Pain was the only reliable symptom and tenderness and muscle guarding the only reliable signs. There was no mortality and no significant maternal or fetal morbidity.Early surgical intervention is mandatory and does not lead to an increased fetal loss.

SummaryThe clinical presentation and management of three cases of spontaneous subarachnoid haemorrhage occurring in pregnancy are described. All patients were delivered by caesarean section with favourable outcomes for both the mothers and their babies.Three patients in whom subarachnoid haemorrhage had occurred during pregnancy were delivered at Ashford Hospital during the summer of 1985.

SummaryAs the number of epidural anaesthetics for patients in labour increases in most obstetric units around the country the incidence of forceps and, in particular, rotational forceps delivery, also appears to increase. Prolonging the second stage of labour encourages spontaneous delivery and we describe a plan of management for such patients with epidural analgesia.

SummaryA 32 year old woman had a placental site trophoblastic tumour with a nephrotic syndrome. She had an initial serumβ-HCG of 1176miu/ml and human placental lactogen of<1μg/ml. The tumour had invaded nearly the whole thickness of the myometrium and the cervix, and formed peri-urethral nodules.She had a hysterectomy with excision of the periurethral mass and postoperative chemotherapy. PAP-positive HCG and placental lactogen were seen in some capillary loops of renal glomerulus. The patient died 4 months later after haemoptyses.
