SummaryIn the course of 1 year, prenatal screening for neural tube defects coincidentally revealed 5 fetuses which had aberrant tissue bands or abnormalities of the cord. These cases had raised maternal serum and amniotic fluid a-fetoprotein, but were distinguished from neural tube defects by the amniotic acetylcholinesterase test. Such cases may be commoner than previously supposed, because they might be misdiagnosed on a cursory examination of the fetus. Within this group the prognosis is variable, but the risk of recurrence in future pregnancies is low.

SummaryDesmopressin is a synthetic analogue of vasopressin with a prolonged antidiuretic effect; 20μg were given intranasally to 7 normal healthy volunteers and to 80 pregnant women to assess its safety and efficacy in pregnancy as a test of renal concentrating ability. There were 27 normal pregnancies, 18 with hypertension and 35 with bacteriuria. It did not produce hypertension, oliguria or other unwanted effects. Direct comparison showed that desmopressin was more effective in assessing renal concentrating ability during pregnancy than was 15 hours of dehydration, and was comparable with published data for 24 hours of dehydration or intramuscular vasopressin.

SummaryWe report a proved case ofStreptococcus viridansendocarditis during pregnancy in a mother with a probable ventricular septal defect. She was treated with intravenous penicillin and successfully completed her pregnancy. Review of the literature shows thatS. viridansis the most commonly isolated organism in cases of infective endocarditis during pregnancy. The maternal mortality (21 per cent) is similar to the mortality reported in non-pregnant series.

SummaryIn a prospective study of 1235 pregnant women, 40 (3·2 per cent) had cytological evidence of active polyomavirus infection of the urinary tract, confirmed by virus culture, electron microscopy and serological studies in the majority of cases. The health of these 40 mothers, the course and outcome of the pregnancies and their babies' health were compared with the 1195 mothers who showed no cytological evidence of virus infection and with a subgroup of these cytologically negative women who were found to have high or rising antibody titres but no other evidence of viral reactivation. The mothers who had cytological evidence of viral reactivation had significantly more illness. both before and during pregnancy, than the cytologically negative women. and their babies were more jaundiced.

SummaryOf 38 liveborn preterm babies presenting by the breech during the 2-year period 1976–77, 10 were delivered by caesarean section. After excluding fetuses with severe congenital malformations, the subsequent course of those delivered by caesarean section was uncomplicated whereas the incidence of deaths from intracranial haemorrhage, especially in the lower weight ranges, was high amongst those delivered vaginally. Caesarean section may be the management of choice for the preterm baby presenting by the breech.

SummaryOne hundred and thirty-two twin pregnancies were prospectively studied after 30 weeks gestation. They were compared with singleton pregnancies with respect to the inter-relationship between the gestation at spontaneous labour, gestation at which perinatal mortality is lowest, somatic and biparietal diameters, plasma oestriol and placental lactogen, and pulmonary maturation. The optimum maturity for delivery in twin pregnancy appears to be between 35 and 39 weeks compared to between 37 and 41 weeks in singletons.

SummaryA study of the effect of an epidural block in 50 per cent of 580 induced primigravid and 32 per cent of 645 induced multigravid labours did not reveal any change in first stage progress of cervical dilatation. There was an increased duration of the second stage, a decreased incidence of normal vaginal delivery and a considerable increase in the proportion of patients requiring Kielland's forceps rotation in both primigravid and parous patients. In spite of this, the neonatal condition was better in the group of patients having the epidural block. The caesarean section rate in this series of low-risk patients in whom labour was induced was significantly higher than that found in patients having spontaneous onset of labour.
