SummaryAmniotic fluidβ-lysin was found to have zinc-dependent bacteriostatic activity against the enteric bacilli, anaerobic Gram-negative bacilli and Gram-positive aerobic and anaerobic cocci. In contrast,β-lysin was found to exhibit bactericidal activity against the anaerobic and aerobic Gram-positive bacilli. This bactericidal activity was independent of zinc ion concentration. The clinical and biochemical implications of these observations are discussed.

SummaryFollowing induction of ovulation by clomiphene at Southampton Subfertility Clinic between 1971 and 1977. 149 pregnancies were identified and studied. Either 100 or 150 mg taken on days 2–6 induced 78 per cent of the clomiphene pregnancies. Fifty-six per cent conceived within 3 cycles of clomiphene treatment and 78 per cent within 6 cycles. In 28 per cent of cases mid-cycle oestrogen was used to improve the cervical mucus. Three per cent of pregnancies occurred when HCG had been given in addition to clomiphene.The 149 conceptions resulted in 19 per cent abortions, 75 per cent singleton pregnancies and 8 per cent multiple pregnancies. The perinatal mortality rate for singleton pregnancies (37 per 1000 births) was higher than the overall rate at Southampton General Hospital.

SummaryOf women who developed a low haemoglobin concentration in pregnancy. 84 percent had a booking ferritin concentration below 50μg/l. Two groups of patients were studied; one group had iron and folate supplements withheld. Of the unsupplemcnted patients, 18·4 per cent became anaemic (haemoglobin<10·5 g/dl). In the group given oral iron and folate. only 4·8 per cent developed anaemia. Ferritin levels in early pregnancy can be used to predict the need for iron supplements.

SummaryOccasionally, in second trimester spontaneous abortions. fetuses with a highly characteristic appearance occur (Figure 1). They arc phenotypically female. and have enormous cervical hygromas and generalised subcutaneous oedema. Those which have been successfully karyotyped have a 45, X chromosome constitution: consequently such fetuses are regarded as being a fetal form of Turner's syndrome (Singh and Carr, 1966). We report the characteristic ultrasound findings of such a fetus at 22 weeks maturity.

SummaryThe concentrations of testosterone and follic estimulating hormone (FSH) in amniotic fluid were determined in 110 pregnancies, from 11 to 22 weeks maturity. Fifty-eight of the fetuses were male and 52 were female. The two indices were compared for their value for fetal sex diagnosis in the second trimester.Mean testosterone concentration was 3·5 times higher in the presence of a male fetus. After 13 weeks, fetal sex could be predicted in 38 of the 43 subjects with a male fetus and in 34 of the 38 subjects with a female fetus. After 13 weeks, mean FSH concentration was 4·6 times higher with a female than a male fetus. Fetal sex could be predicted with FSH in 19 of the 38 subjects with a female fetus. Determination of testosterone is a valuable index and should be carried cut in the first instance. FSH should be determined in cases where the testosterone level is equivocal. This permitted diagnosis in every instance

SummaryThe possible roles of endogenous oxytocin, and spinal reflex mechanisms of the autonomic nerves were studied after the induction of abortion in the mid-trimester by uterine stretching. A complete epidural and caudal block of the afferent nerves from the uterus was performed before the insertion of laminaria tents or a balloon into the uterus, and this nerve block was maintained throughout the study. Uterine activity showed no significant difference between the nerve block and non-nerve block groups (P= 0 71), but differed significantly with increasing time after stretching (P<0 001). Plasma oxytocin values did not increase significantly in either the normal or the nerve-blocked patients.

SummaryWe report a case in which an acardiac monster presented an unusual obstetric problem. An acardiac monster was first reported by Benedetti (1533). Das (1902) classified hemicardias cimperfectly formed heart) and more specifically holocardius (absence of heart) as two major groups. This abnormality has been termed chorioangiopagus parasiticus, since this defines the entire dependency of the monster or the blood supply of its normal identical twin.

SummaryThree patients with cadaver renal transplants were managed through pregnancy. One mother and baby had a benign cytomegalovirus infection and delivered at 35 weeks; one mother developed cholestasis and delivered at 34 weeks; the third had an elective caesarean section at 37 weeks for breech presentation and hypertension. Maternal renal function did not deteriorate and all three babies did well.

SummaryWomen in late pregnancy were subjected to mild psychological stress by listening, through headphones, to a prerecorded sequence of sounds which included that of a baby crying. Maternal reaction was detected using maternal heart rate and maternal skin resistance. Coincidental changes were observed in fetal heart rate patterns. Fetal heart rate pattern changes similar to those induced by the tape-recorded sounds have been observed during‘non-stressed’antepartum monitoring when patients were disturbed by sounds significant to them.
