SummarySummaryA retrospective study was carried out in the Maternity Hospital of Cruces (Vizcaya, Spain) over 11 years (1980–1990). Twin deliveries numbered 744 and their relative frequency was 1 in 101 deliveries. Although the total number of deliveries declined sharply during the years of study, there was an increase in the overall incidence of multiple births.The use of caesarean section for twin delivery increased during the study (P<0·05). In the first 6 years 23·1 per cent of deliveries were by caesarean section increasing to 33·9 per cent in the last 5 years.We failed to find a unique cause for these differences, but a number of factors correlated with the increase in the caesarean section rate. The increase in the incidence of a non-cephalic first twin, the decline in internal version and breech extraction of a second transverse lie twin and the rise in prematurity rates were all associated with the increase in abdominal delivery of twins.

SummarySummaryIntra-uterine growth retardation (IUGR) is the main contributory factor in the increased perinatal morbidity and mortality associated with pregnancy induced hypertension. Seventy-six primigravidae and 38 parous women with non-proteinuric hypertension (diastolic blood pressure>90mmHg) and singleton pregnancies were allocated at random to receive either labetalol or no anti-hypertensive treatment. At recruitment maternal age, blood pressure and gestational age were similar in both treatment and control groups. For primigravidae 16 per cent (7/45) of the controls delivered a baby with weight less than the 10th centile for gestational age compared with 19 per cent (6/31) in the labetalol treated group. A birth weight less than the 10th centile for gestational age occurred in 2 out of 18 multigravid controls and 4 out of 20 of the multigravid labetalol treated group. There were more admissions to the neonatal unit for the babies of labetalol treated mothers.A limited meta-analysis of the results combined with those of two other controlled trials suggests that the drug contributes to intra-uterine growth retardation (P<0·05).The possibility of labetalol having a direct adverse effect on fetal growth requires further evaluation in a larger random controlled study.

SummarySummaryThe 9-deoxo-16,16-dimethyl-9-methylene analogue of prostaglandin E, possesses many beneficial properties that makes it a potential candidate as a cervical ripening agent. Twenty pregnant women at term with unripe cervices (Bishop scores C2) were studied. A gel with 1 mg of the analogue was instilled vaginally in 10 women; the other 10 women were controls. Cervical biopsies were taken before and 6 hours after giving the gel, and at comparable times in the controls. After gel administration, the densely packed collagen bundles were broken into smaller fibrils and became loosely arranged and widely separated by abundant ground substance and clear spaces. These changes closely resemble the spontaneous physiological alterations in cervical tissues associated with ripening.

SummarySummaryNinety-one consecutive stillbirths were associated with 19 531 deliveries during 5 years in a busy district general hospital. Fifty-seven per cent of stillbirths were unexplained; the second commonest group was due to antepartum haemorrhage (16 per cent). Sixty-five per cent weighed≤2499 g and 62·5 per cent occurred between 28 and 35 weeks of pregnancy. With optimal performance by the antenatal and intrapartum care providers, patient compliance with medical advice and attendance for antenatal care, the stillbirth rate might be decreased by 41 per cent. Subclassifications to the category of unexplained stillbirths in Coleet al.'s modification of the Aberdeen classification and categorisation of the avoidability of perinatal deaths are suggested.

SummarySummaryWe prospectively recorded the frequency with which personnel performing or assisting at gynaecological and obstetric procedures came in contact with patient body fluids, to provide a basis for recommending protective precautions. In 387 of 450 instances (86 per cent; 95 per cent confidence interval 82–89 per cent) contact between patient body fluid and protected or unprotected parts of the health care worker occurred. Contact with the hand occurred in 385 instances (86 per cent). Contact with arm. body, mouth and eye occurred in 12, 4, 0·7 and 0·7 per cent respectively. Protective devices failed to prevent direct contact in 9 cases; penetrating accidents with sharp objects occurred in two cases (0·4 per cent; 95 per cent confidence interval 0·05–2 per cent). For each procedure the rate and nature of contact with body fluid is listed, and the implications concerning prevention of infection with blood borne pathogens, especially HIV, are discussed.
