SummaryThe changing values of whole blood viscosity and its major determinants, haematocrit, plasma fibrinogen, total serum protein, plasma viscosity and erythrocyte deformability, were followed throughout and after normal pregnancy in a serial study of 50 patients.In a cross-sectional study of the same parameters in cord blood samples from 170 fetuses at varying gestations, the haemorheological profile of fetal blood was defined. The significance of these haemorheological measurements in the changing haemodynamics of pregnancy and as a background for the study of pathological pregnancy is discussed.

SummaryOne method of investigating oxygen utilisation at tissue level is the use of the lactate-pyruvate ratio. Excess lactate is an index of anaerobic metabolism or oxygen debt. Twenty women were studied at the maternity unit of the University of Benin Teaching Hospital. Pregnant women had a slight rise in the lactate level of venous blood by the end of pregnancy. The lactate-pyruvate ratio was markedly elevated in established labour. Venous cord blood lactate-pyruvate ratio was slightly higher than the maternal ratio immediately after delivery. It is likely that the changes during labour are a consequence of muscular exercise.

SummaryThree different assays were used for the measurement of‘unconjugated oestriol’in blood samples from 108 women at 38 weeks of pregnancy. The three types of assay gave different numerical values but their clinical efficiency in the identification of intra-uterine growth retardation was identical. It is concluded that specific procedures involving extraction steps offer no clinical advantage over the simpler and more convenient direct assays.

SummarySerum placental lactogen levels were measured in serial samples from 689 women at weekly intervals from 36 to 40 weeks maturity. The mean of two serial levels of HPL was found to be a better indication of growth retardation than the first level alone. The proportion of falling levels in subjects with growth retardation was slightly greater than that of the whole population, but the observation of falling levels was of no practical clinical value.

SummaryA 10 year retrospective analysis (1968-1978) of pregnancies complicated by ovarian cysts at the Birmingham Maternity Hospital was performed. The overall incidence of this complication was 0-14 per cent. In 23 out of a total of 62 cases an ovarian tumour was an incidental finding at caesarean section or postpartum sterilisation. Ten cases presented as emergencies, 7 in the first trimester, and in 6 cases obstructed labour occurred. In 2 cases ovarian malignancy was diagnosed.It is concluded that current practice of elective removal in the mid-trimester will not significantly reduce the incidence of acute accidents but is justified to exclude possible malignancy. The high‘incidental finding’rate suggests that ovarian tumours are more common than was originally thought.

SummaryAmniocentesis was carried out in 35 pregnant diabetic patients within 72 hours before delivery. All patients had lecithin/sphingomyelin ratios of 22 or more. Four babies (114 per cent) developed respiratory distress syndrome. The remainder of the same 35 samples of amniotic fluid were used retrospectively to predict fetal lung maturity in two ways using phosphatidylglycerol. Measurement of amniotic fluid phosphatidylcholine (lecithin) and phosphatidylglycerol concentrations appears to predict fetal lung maturity more accurately than either the lecithin/ sphingomyelin ratio alone or when combined with simply recording the presence of phosphatidylglycerol.

SummaryA study of 102 pregnancies in 35 women with myotonic dystrophy has shown increased incidences of operative delivery, hydramnios, placenta praevia and placental retention as well as neonatal mortality. Four of 12 women who had major obstetric or gynaecological surgery developed severe respiratory complications. Fourteen per cent of infants were delivered within 2 hours of onset of labour and 6 per cent of women had a postpartum haemorrhage. The complication rate may not be increased in minimally affected women who largely present with eye complications in late middle age.

SummaryIn a 2-year prospective study of anticonvulsant therapy in pregnancy, 5 term breech presentations (12 per cent) were discovered in 42 patients who took anticonvulsant drugs throughout pregnancy to term. This incidence of breech presentation was statistically greater (P<0-025) than in a control hospital population (4 per cent). There was a strong correlation between maternal and cord serum drug concentrations at delivery (r= 0-95) indicating the degree of fetal exposure to the drug. No other predisposing factors were discovered and it was concluded that there is an association between anticonvulsant drug use in pregnancy and breech presentation.

SummaryAn attempt was made to predict fetal weight from ultrasonic measurements of maximum fetal abdominal circumference. It proved impossible to predict weight with sufficient accuracy to be clinically useful. If the abdominal girth was between 34 and 38 cm after 36 weeks, we could be 95 per cent confident that the baby's weight was more than 2-5 kg and less than 4 kg.
