SummaryA one year population survey of 1000 women has been performed to determine the onset, occurrence and severity of pregnancy sickness and to correlate this information with a variety of biological markers in order to help explain the aetiology of this side effect of pregnancy. Of the women, 85 per cent experienced nausea and 52 per cent vomiting. Of these, 70 and 40 per cent reported daily nausea and vomiting respectively. Only 17 per cent of women who had nausea reported that their symptoms occurred exclusively in the morning, the corresponding proportion for vomiting being 31 per cent. In a third of the women the onset of pregnancy sickness occurred within four weeks of the last period and in three quarters, within six weeks; furthermore the earlier the onset of symptoms the more likely women were to suffer from frequent bouts of nausea and vomiting. Positive correlations were found with the occurrence of pregnancy sickness and age, parity and the development of food cravings. The occurrence of premenstrual symptoms, migraine and travel sickness were also positively correlated with the incidence of pregnancy sickness.

SummaryThe early amnion rupture sequence is a pattern of external deformations which may be mistaken for other more common malformations on antenatal ultrasound. Four cases are described in which antenatal ultrasound was performed, but the definitive diagnosis was made only at autopsy. Early amnion rupture is a sporadic condition which may be over-represented among second trimester abortions. It should be distinguished from those conditions which it imitates which have a higher recurrence risk.

SummarySixty cervical circlage operations, mostly by the McDonald method, were performed in 52 pregnant patients. Diagnosis of cervical incompetence was based on the history and characteristics of previous pregnancy losses and findings in the current pregnancy. Of the 60 pregnancies, 9 (15 per cent) ended in abortions; preterm labour occurred in 10 cases (17 per cent) and term deliveries in 41 cases (68 per cent). Complications following the procedure were minimal. No difference was found in pregnancy outcome with respect to the suture material used (Mersilene tape or silk suture).

SummaryA five year retrospective study of obstetric performance of sickle cell anaemia (haemoglobin SS) patients at the University College Hospital, Ibadan is presented.The study has confirmed poor obstetric performance in sickle cell anaemia patients. Although there were increased pregnancy complications, these were less than in similar studies performed previously in this hospital. This may be due to increased awareness and understanding of sickle cell anaemia. Sickle cell anaemia patients who are pregnant must be managed as high risk pregnancies in well equipped centres.

SummarySixty-eight children with developmental delay, consecutively referred to the Child Health and Monitoring Project at Ipswich Hospital had their notes and mother's obstetric notes examined.Features associated with developmental retardation included past and present obstetric complications, intrauterine growth retardation, abnormal intrapartum cardiotocography and prematurity, especially with a depressed 5 minute Apgar score. However, there was no correlation between the severity of the complications and the degree of the developmental delay.Both antenatal factors and intrapartum events play a role in the aetiology of developmental retardation in infants.

SummaryUmbilical artery Doppler waveform analysis was performed before and for 90 minutes after the intracervical administration of 0–5 mg dinoprostone (prostaglandin E2) gel. Twenty-three patients undergoing cervical ripening before induction of labour were studied. No effects on mean resistance index in the umbilical artery or fetal heart rate were demonstrated. Only one patient developed a transiently abnormally high resistance index and the concurrent cardiotocogram remained normal with a normal fetal heart rate. No other side effects on the fetus were seen.

SummaryThe hypothesis that having a 'birth plan' for labour was associated with an increased risk of operative delivery was tested by retrospectively reviewing the case notes of 124 women with normal singleton pregnancies. Sixty-two women with a birth plan were matched for age, parity, gestational age and day of delivery with 62 women without a birth plan. There was no difference in the operative delivery rate between the two groups, which was 27 per cent (24 per cent instrumental vaginal delivery and 3 per cent caesarean section) for the birth plan group and 32 per cent (27 and 5 per cent respectively) for the control group.
