SummaryPatients with a history of recurrent miscarriage were studied. Some were immunised with their partners' lymphocytes, while others immunised with their own (autologous) cells served as controls. Lymphocyte antibodies were detected in the first trimester of pregnancy in only one out of nine control patients immunised with autologous cells. Approximately half of the patients immunised with their partners' cells had lymphocyte antibodies of some kind during pregnancy, but neither lymphocyte antibodies in general or any particular specificity correlated with good or bad pregnancy outcome. Any beneficial effect of leucocyte immunotherapy is therefore not the result of lymphocyte antibody production.

SummaryThe outcome of pregnancy was reviewed in 148 patients with 158 cervical circlage operations during pregnancy. Thirty per cent of the patients had had previous cervical surgery. There were no significant complications following circlage. The fetal salvage rate after circlage was 74-1 per cent in patients with a previous fetal survival of 34-6 per cent. There was no difference in pregnancy outcome between patients who had McDonald circlage and those who had Shirodkar sutures, nor was there any difference in outcome when Mersilene tape or Mersilk was used as suture material. Cervical circlage was associated with a higher incidence of caesarean section than in the population in general, mainly due to prematurity. The fetal salvage rate and absence of complications support our liberal selection criteria for cervical circlage, even though laxity of the criteria means that a number of women without cervical incompetence are subjected to the procedure.

SummaryForty-two patients had established diabetes, 124 had gestational diabetes and 48 had impaired glucose tolerance in pregnancy. Diabetic women had an increased incidence of urinary tract infection, vaginitis, hydram-nios and ketoacidosis in pregnancy. Delivery occurred after 38 weeks in 82 per cent of patients with established diabetes and 91 per cent of gestational diabetics. The overall caesarean section rate (17-3 per cent) in the series was double that of the hospital incidence. The perinatal mortality rate was 37/1000 births, three times the overall perinatal mortality rate for the hospital. Babies of diabetic patients had increased rates of macrosomia, hypoglycaemia, hyperbilirubinaemia and birth trauma and congenital fetal anomalies (5-6 per cent compared with 1-5 per cent in the hospital population). These data emphasise the need to achieve maternal euglycaemia before conception.

SummaryTwenty-eight cases of pregnancy associated invasive carcinoma of the cervix diagnosed and treated over 10 years at the Department of Obstetrics and Gynaecology, University Medical School of Debrecen, Hungary, were analysed. Pregnancy associated cervical carcinoma had a more favourable stage distribution than the disease presenting in women who were not pregnant. The stagespecific survival was not worse than in non-pregnant cases. There was no evidence of adverse consequences when one stage Ib case was treated in the first trimester by amputation of the cervix or in two stage Ib cases where treatment was delayed until the fetus was viable. Some carefully assessed stage Ia cases seem to be suitable exemptions from radical hysterectomy.

SummaryFour cases of phaeochromocytoma presenting in late pregnancy are described. Delayed tumour resection following caesarean section was successfully employed in 2 cases. During and following delivery, maternal hypertension was easily controlled. Delayed tumour resection enables full pre-operative localisation without risk to the fetus. Furthermore, maternal physiology reverts to normal. Synchronous surgery, in the remaining 2 cases, was associated with serious peri-operative morbidity. In later pregnancy, delivery by caesarean section followed by delayed tumour resection is recommended.

SummaryWhether or not Doppler studies of the utero-placental circulation can help indicate which hypertensive pregnancies will develop other features of pre-eclampsia was investigated. A prospective, longitudinal study of utero-placental blood velocity waveform by continuous wave Doppler ultrasound in third trimester hypertensive pregnancies was conducted. Sixty patients were referred for investigation of hypertension in the third trimester of pregnancy; 25 subsequently developed other features of pre-eclampsia. Uterine blood velocity waveforms were assessed by the resistance index and the presence or absence of a dichrotic notch. Proteinuria was defined as at least a + on dip-stick testing on two occasions or>500 mg/24 hours in the absence of urinary infection.Utero-placental Doppler studies were not demonstrated to differ in the patients with proteinuric pre-eclampsia from those with non-proteinuric preeclampsia. Continuous wave Doppler studies of the utero-placental circulation in the third trimester cannot be used to guide management of hypertensive pregnancies.

SummaryA prospective controlled trial was conducted at Coventry Maternity Hospital to compare the efficacy of prostaglandin E2vaginal tablets (3 mg initially, 6 mg maximum) and prostaglandin E2vaginal gel (1 mg initially, 3 mg maximum) for ripening the unfavourable cervix and induction of labour.Two hundred patients were studied; 104 were given tablets and 96 gel. All the pregnancies were singleton and were at least 36 weeks. All patients completed a standard questionnaire using a 100 mm linear analogue scale to assess the acceptability of the two methods of induction. Scores for discomfort and pain were low and did not differ between the two groups, but patients who had previously had labour induced found the gel more acceptable than the tablets.Twenty per cent of all patients did not need amnio-tomy. Forty per cent of the table and 50 per cent of the gel patients did not need augmentation of labour with oxytocin. There was no difference in cervical ripening or induction to delivery intervals between the two groups.It appears that administration of prostaglandin E2in a gel is as efficient in a gel as it is in tablets, but only half the dose is necessary in the gel preparation as is required with tablets. This is consistent with the bioavailability of the drug from the two preparations.

SummaryThe limitations of the Apgar score are well recognised, yet it remains the only method widely used for neonatal assessment. Cord blood gas analysis is more accurate and objective, but the misconception that immediate analysis is required limits its use. This study was performed to determine the effect of storing cord blood (in a heparinised syringe at room temperature) on acid-base status. The mean change of pH at 30 min was 0-029 (95 per cent confidence limits: 0-017-0-041). Corresponding values for base deficit were 1-85 and 1-54-2-17 mmol/1. These changes were not clinically significant, and the samples represent the condition at birth. Using this simple method, analysis can be deferred up to 30 min, making it feasible in most obstetric units.
