SummaryA marked lack of parenting ability is sometimes encountered in mothers attending paediatric clinics. To identify areas where such deficiencies exist, a detailed survey of 30 primigravid women was carried out. The level of parenting was assessed by means of a scored questionnaire. Single mothers scored lower than married women and were found to be the group least likely to have attended parenting classes. Mothers scored well on aspects of parenting that had been shown to them on the postnatal ward. Many deficiencies in parenting skills were demonstrated; some of these would be readily amenable to inpatient postnatal education.

SummaryPlasmapheresis has been advocated as an additional treatment to help rhesus negative mothers with high anti-D titres during pregnancy. Although plasmapheresis may help to reduce anti-D titres the experience at Lewisham Hospital shows that the outcome of pregnancy is not significantly affected. Advances in neonatal intensive care are more likely to reduce fetal wastage due to rhesus iso-immunisation than the use of an intensive plasmapheresis programme.

SummaryTwo non-invasive radio telemetry fetal monitors were used in turn to monitor 26 ambulant patients in the early stages of labour. In order to avoid observer bias, an online computer was used to measure the quality of the recording. One of the fetal monitors gave clinically adequate results in 62 per cent of cases, the other in only 19 per cent. The proportion of patients admitted to the labour ward which we considered suitable for ambulant monitoring was estimated to be 18 per cent. The success of the monitoring was not significantly affected by the height, weight or abdominal girth. In this clinical evaluation of non-invasive telemetry we found that ambulant fetal monitoring is technically feasible.

SummaryThe mean estimated signal loss of fetal heart rate records in late pregnancy using a continuous wave Doppler ultrasound telemetry system was 14 per cent (s.e.±3·3) compared with 6 per cent (s.e.±2·3) using a pulsed Doppler ultrasound monitor (P<0·0005). During ambulant labour the mean estimated signal loss of ultrasound telemetry fetal heart rate records was 24·3±4·3 per cent in women with intact membranes and 23·1±4·0 per cent in women with ruptured membranes. More than half of this signal loss occurred as episodes of total signal loss lasting five minutes or more due to displacement of the transducer. Such episodes of signal loss during ambulation will limit the value of ultrasound telemetry unless attention is paid to maintaining optimal position of the transducer on the maternal abdomen.

SummaryUterine activity was measured in 30 primigravidae and 30 women of parity 5 or more. All were in the active phase of the first stage of spontaneous labour and were delivered spontaneously vaginally—without augmentation with oxytocin. The cumulative uterine activity integral values and the values at individual cervical dilatations were significantly lower with parity 5 or more than in primigravidae. The overall values (mean±s.d.) of the integral were 1574±468 KPa/15 min and 1139±442KPa/15 min for primigravidae and high parity women respectively. A gradual rise in uterine activity as labour advanced from 3–10 cm cervical dilatation was observed.

SummaryA retrospective survey of 239 deliveries of patients who had a previous lower segment caesarean section was carried out. Ninety-one (38 per cent) delivered vaginally and 123 (51 per cent) had an elective caesarean section. Eighty-nine (37 per cent) had X-ray pelvimetry performed. An obstetrician's perception of a Contracted pelvis on X-ray resulted in almost universal repeat caesarean section. An adequate pelvis on X-ray was found to be a poor predictor of the outcome of a trial of labour, the state of the cervix at the onset of labour giving much better prediction. The pelvic dimensions of those patients having a successful trial of labour resulting in vaginal delivery were the same as those of patients having an unsuccessful trial of labour resulting in an emergency caesarean section, but the birth weights in the latter group were greater.X-ray pelvimetry is only of limited value in the context of a trial of labour following a previous caesarean section; more attention should be paid to the state of the cervix at the onset of labour and to fetal size.

SummaryA retrospective study was performed on 4569 deliveries of normal singleton Chinese infants, in order to investigate the inter-relationships of social, physical, antenatal and intrapartum variables in the aetiology of birth asphyxia. Thirty–five variables were analysed using the technique of path analysis. Four direct and independent precursors of birth asphyxia were identified. These were placenta praevia, low birth weight, suspected fetal distress and caesarean section in the first stage of labour.
