SummaryAnxiety in pregnancy has been studied in 249 primiparous Caucasian women, using the State-Trait Anxiety Inventory. The level of anxiety was assessed on three occasions during pregnancy. The tendency to anxiety (trait) decreased from 16 to 28 weeks of pregnancy. In addition, anxiety in response to the situation (state) decreased significantly in the second trimester but increased again in the third trimester of pregnancy. Higher levels of anxiety were found during pregnancy in those women who were subsequently given oxytocin augmentation in spontaneous labour than in those who were not. In addition, high levels of anxiety were found in those who subsequently had epidural analgesia.

SummaryThe incidence of pre-eclampsia in first term pregnancies was studied in relation to whether or not they were preceded by pregnancies which ended in abortion—spontaneous or induced. Two hundred and twenty patients with previous abortion were studied along with two sets of controls, one with no previous abortion or pregnancy and one with a second term pregnancy.The incidence of pre-eclampsia was significantly lower when there had been a previous induced abortion but this was not as low as in cases with a second term pregnancy. This finding may be interpreted as supporting the idea that pregnancy exerts a protective effect against the risk of pre-eclampsia which may have an immunological basis. A mechanism is suggested to explain the apparent‘protective’effect of first pregnancies.

SummaryUltrasound examination at 19 weeks disclosed a membrane in the amniotic cavity along with a single fetus and a single placenta. A small-for-dates female baby with signs of amniotic disruption was delivered at term. There was chronic inflammation of the placenta. This presumably caused both the amniotic rupture and the severe intra-uterine growth deficit. If ultrasound examination reveals a membrane in the amniotic cavity, the possibility of amniotic rupture should be considered, and a thorough ultrasound examination of the fetus for signs of the amniotic disruption sequence should be conducted.

SummaryThe incidence of stillbirth among 6597 pregnant women who were all given fetal movement charts to use in the third trimester did not differ from that among 13705 pregnant women of whom only a very few selected high-risk mothers used fetal movement charts. These results, in contrast with previous studies, demonstrate that maternal monitoring of fetal movement does not reduce the risk of intra-uterine death.

SummaryThere were no significant changes in platelet phenolsulphotransferase M and monoamine oxidase activities in pregnant women compared with controls and women postpartum, although mean monoamine oxidase values were about 10 per cent higher during pregnancy. Platelet phenosulphotransferase P activity tended to decline during pregnancy and rose substantially postpartum. Mean values postpartum were higher than those in the pregnant group as a whole (P>0.01) and in women during the last trimester of pregnancy (P>0.01). If these platelet findings reflect changes elsewhere in the body, they might imply that women towards the end of pregnancy are more sensitive and women postpartum less sensitive to the action of drugs that are inactivated by sulphoconjugation.

SummaryIn the second stage of normal breech deliveries 35 fetuses out of 42 born with a good Apgar score had uncomplicated fetal heart rate tachycardia. Fetal heart rate tachycardia with normal baseline variability and no deceleration pattern is a physiological pattern during the second stage of breech delivery. The uncomplicated tachycardia may be due to a slight partial occlusion of the fetal cord between the fetal abdomen or thorax and the maternal pelvis. Reduction in fetal venous return causes uncomplicated fetal heart tachycardia as long as anoxia is not present, and therefore warrants no active measures as long as no other signs of fetal hypoxia appear.

SummarySerum trypsin inhibitory capacity was determined in 20 Nigerian women with pre-eclampsia and in the cord blood of their babies. Twenty other women who had spontaneous onset of labour and delivery served as controls. Trypsin inhibitory capacity was significantly lower in the cord blood of infants of pre-eclamptic mothers than in the control group (P>0.0002). The possible immunological basis for this finding is discussed.
