SummarySummaryThe number of low birth weight neonates (<10th centile) that had objective neonatal evidence of intra-uterine malnutrition was studied prospectively. A fetal‘weight for gestational age' centile chart nomogram was constructed retrospectively for the local population in an area of East Anglia. When the Norfolk and Norwich birth weight centiles (10th, 50th, 90th) for each week of pregnancy (29–40 weeks) were compared with national reference centile charts the local neonates were smaller by 0–250g (P<0·01). Of 418 singleton deliveries at the Norfolk and Norwich Hospital in June 1989, 42 had birth weights below the 10th centile for that population. On paediatric assessment only 17 (40 per cent) of these had morphological features of intra-uterine growth retardation. These babies were associated with more operative deliveries and special care baby unit admissions (P<0·001) than the rest of the 'small for dates' babies. The overall incidence of intra-uterine growth retardation for this population, as assessed by rational paediatric criteria, was 4·1 per cent.

SummarySummaryThere were 9387 deliveries within the study period, of which 104 (1·1 per cent) were caesarean sections for fetal distress in labour, with a singleton cephalic presentation at 37 or more weeks gestational age. Caesarean section for fetal distress was associated with a significant increase in the incidence of poor neonatal condition as defined by low Apgar scores (P<0·-001) or special care baby unit admission for asphyxia (P<0·001): but not with any increased incidence of fetal scalp blood or umbilical arterial or venous acidaemia.There was an association (P<0·05) between the interval between decision to deliver and operative delivery, and admission to the special care unit. The relative risk doubled between a 10 and a 35 minute interval. No correlation was found between the decision delivery interval and 1 or 5 minute Apgar scores, or cord arterial and venous acid-base results.A short interval from decision to delivery may be important if optimal neonatal condition is to be achieved.

SummarySummaryThe value of a single determination of plasma glucose after an overnight fast as an indicator of intra-uterine growth was examined. Fasting plasma glucose was measured at 28 weeks in 241 European and 77 Asian mothers to investigate racial differences in the sensitivity and specificity of the test. In both racial groups mothers with the lower plasma results had smaller babies, but this relationship was insufficiently strong to be of predictive clinical value in the individual patient. The relationship with birth weight appeared to be mediated by maternal size in Europeans but was independent of maternal size in Asians. Although maternal weight was greater in the Europeans, the Asians were proportionally fatter with greater triceps skinfold thickness.

SummarySummaryThree hundred and fifty consecutive grand multiparae (19·6 per cent of 1789 deliveries in 1987–1989) in lie State Hospital were reviewed. The incidence of placenta praevia. abruptio placentae, preterm rupture of the membranes and ruptured uterus were greater (P<0·05) than those for women of lower parity. The perinatal mortality rate was lower and the maternal mortality rate comparable to that of women of lower parity. The grand multipara may be at a higher risk of complications but adequate care in pregnancy, labour and puerperium can reduce serious consequences.

SummarySummaryTwo dimensional echocardiography and Doppler ultrasound was used to study haemodynamic changes in four cases of hydrops fetalis associated with nuchal cystic hygroma without other structural anomalies. Normal cardiac output was found in three of the hydropic fetuses. In the fourth fetus abnormal haemodynamic changes indicating congestive heart failure were present, probably induced by supraventricular tachycardia.
