Abstract.Oxygen depletion curves were recorded from the superior and central cornea with a micro‐polarographic system immediately following eyelid closure in 10 adult human subjects. Superior corneal oxygen uptake rates for the closed‐eye state ranged from 13% below to 58% above, with a mean 15% greater than equivalent central corneal oxygen uptake rates. Chronic superior corneal hypoxia induced by upper eyelid coverage when the eyes are open is therefore accompanied by a deeper hypoxia relative to the central cornea of some individuals when their eyes are closed. Abstract.Sixty‐three consecutive corneal transplantations employing banked donor material were grouped according to donor age and post mortem time, respectively, and followed at regular controls for 5 years. Twenty‐six cases (38% belonging to a risk group i.e. had earlier had corneal transplantation(s) and/or vascularized corneas) with a mean donor age of 55.8 years were compared with 37 cases (35% at risk) with a mean donor age of 75.7 years. The five‐year graft survival was 72.0% in the first group and 86.3% in the second group, however, the difference was statistically insignificant. In 27 cases (26% at risk) with a mean post mortem time of 17.4 h a five‐year graft survival of 81.0% was found compared with 80.3% for 36 cases (44% at risk) with a mean post mortem time of 40.5 hours. No difference in graft thickness was demonstrated between any of the groups, and in all cases with surviving grafts normal values for corneal thickness were achieved. Abstract.Four patients with ‘microcystic epithelial abnormality’ were studied. Photographs demonstrated biomicroscopically visible alterations taking place over months. The histopathology comprising intraepithelial basement membrane abnormalities and intraepithelial cysts is described at the light and electron microscopical level. The biomicroscopically grey‐white microcysts correspond in size to the basement membrane abnormalities. The histopathologically described cysts are much smaller. The pathogenetic mechanisms are discussed and it is suggested that breaks in the normal basement membrane are a more likely initial event than a speculative ‘primary cellular dystrophy or degeneration’. Cells grow beneath the basement membrane and lift it up into the epithelium. A subsequent intraepithelial synthesis of basement membrane material may account for part of the ultrastructural appearance. The epithelial cysts were found exclusively beneath the basement membrane. They are thought to reflect the cellular maturation which is altered by the inserted membrane. Abstract.The endothelial cell densities of corneal transplants of 30 keratoconus patients have been studied. All operations were performed by the senior author using the same technique. Preservation systems M‐K medium, MEM‐solution and K‐sol were used; ten grafts in each group. The average endothelial cell density for all 30 transplants was 1815 ± 869 cells/mm2. The mean cell density was the lowest (1128 ± 587 cells/mm2) and the mean follow‐up time longest (2 years and 9 months) in M‐K medium stored grafts. K‐sol is the newest preservation method used in our hospital. It gave the best graft endothelial cell densities (2302 ± 938 cells/mm2). The mean follow‐up time was shortest in K‐sol (8 months). Abstract.A case with severe malabsorption of fat soluble vitamins is described. The malabsorption developed after an intestinal bypass operation due to morbid obesity. Night blindness occurred as the first symptom of vitamin A deficiency. The cone visual sensory threshold was elevated about one log unit and the rod threshold abot two and a half log units. No changes of the a‐ and b‐waves of the electroretinogram (ERG) was observed. However, during the initial phase of very low serum reninol level (0.21 μmol/l) the summed amplitudes of the oscillatory potentials (OPs) were lower. After parenteral therapy with vitamin A the night blindness disappeared and the dark‐adapted rod and cone threshold sensitivity recovered to normal. However, the time‐course of rod adaptation first reached normal levels after 5 months. The amplitudes of the OPs of the ERG response returned to normal when the serum retinol level had increased close to normal. Serum retinol levels of 0.7 μmol/l or higher were always associated with normal or close to normal dark‐adapted rod sensitivity. However, a normal serum retinol level (>0.95 μmol/l) and a normal dark‐adapted rod threshold sensitivity were not always associated with a normal time‐course of the rod adaptation. It is concluded, that the maintenance dosage of vitamin A must be individualized and that patients who have undergone jejunoilea bypass surgery must be carefully monitored for vitamin A deficiency by both serum levels and dark adaptat

Abstract.In the diagnosis of corneal ulcers, bacterial cultures from scrapings, investigations on viral and fungal growth, as well as staining of smears from the lesion often lead to the correct diagnosis. We report four cases of corneal ulcer where all the methods mentioned failed to reveal the causative microorganism. After the patients had received various medical treatments without any clinical improvement, soft therapeutic contact lenses were fitted, worn overnight, and removed the next morning with sterile forceps. The contact lenses were cultured in blood and Mac Leod agars. Growth of various bacteria was then detected. The infection subsided after the antibiotic treatment was changed according to the bacterial resistance. We conclude that, in selected cases, soft contact lenses may be used for microbiological studies.

Abstract.The present report describes a case of bacterial endophthalmitis which was suppressed with topical and systemic corticosteroids for 3 1/2 months in the belief that the uveitis was a sterile reaction. Subsequent pars plana vitrectomy and microbiological work‐up disclosed an infection with coagulase‐negative staphylococci. The patient was treated with intraocular and systemic antibiotics, and the inflammation settled rapidly. It is concluded that infectious endophthalmitis should be suspected after intraocular surgery when the intraocular inflammation is unexpectedly exaggerated. A diagnosis of sterile uveitis should be made only after careful evaluation which includes a proper microbiological work
