Abstract.362 glaucomatous eyes of 213 institutionalized geriatric patients 66–100 year of age (83.9 ± 6.9 year) were studied. Of the eyes with open angle glaucoma (77%, 279/362) 51% had capsular glaucoma (CG). Of the secondary glaucomas (9%) half were neovascular glaucomas. The visual acuity (VA) of 13 patients could not be determined. More than half of the patients (114/200) were visually handicapped (VA in the better eye<0.3). 55 out of 114 patients were considered blind (VA<0.05 in the better eye). In the glaucomatous eyes the mean VA was 0.17, binocular VA for near was 0.28. 28 out of 110 eyes had Bjerrum scotoma, 21 eyes had complete arcuate scotoma and 41 eyes had a residual field in the temporal periphery. In the present study, the causes of blindness were distributed as follows: glaucoma was the only cause in 25%; cataract was one of the causes in 66% and macular degeneration in 24%. Abstract The records of all patients with simple and capsular glaucoma who visited our clinic in the period from 1.1.1972 to 31.12.1982, and who were alive at the 31.12.82, have been evaluated. In the population older than 49 years 0.54% had simple and 0.74% had capsular glaucoma, equally distributed on men and women. Abstract.Use of the various eye care resources for glaucoma patients has been studied in relation to the various age groups. The total population of Finland increases 4% between 1983–2010. The change is very different in the different age groups: a decrease in the age groups up to 44, 60% increase in the age groups over 75. The prevalence of glaucoma increases with the advancing age.7719 persons used hospital beds for glaucoma in Finland 1983. If the same percentage of patients uses hospital facilities for glaucoma in 2010 in each of the age groups the total number of hospital patients treated for glaucoma will be 10 892 in 2010. The increase is 40%.The number of patients registered for free glaucoma medication 1983 by the National Health Insurance was 28913. Abstract.The outcome of ECCE and posterior chamber lens implantation is described in 30 eyes with capsular glaucoma (CG). All eyes had glaucomatous visual field defects. 16 eyes had had preoperative argon laser trabeculoplasty, 5 glaucoma filtering surgery. The mean preoperative IOP was 17.7 mmHg and mean postoperative IOP 16.1 mmHg. Postoperative visual acuity was 0.5‐1.0 in 57 per cent of the eyes, 0.2‐0.4 in 36 per cent and 0.1 or less in 7 per cent. Six eyes showed macular degeneration. Primary change of strategy into ICCE due to subluxation of the lens was made in two eyes. In four eyes ECCE and vitrectomy had to be performed due to zonular rupture. In 16 eyes iridectomies, radial iridotomy and/or sphincterotomies were needed to facilitate the operation. Abstract.79 eyes in 71 patients with advanced glaucomatous damage (loss of at least half the visual field) have been followed for 5–18 years. At the first visit 42 eyes had an intraocular pressure of 40 mmHg or higher. During the follow‐up the visual field defect was stable in 23 eyes (29%) and progressed in 56 eyes (71%). Deterioration of the field was not dependent on the initial degree of defect. Central vision was lost in 27 eyes, and 16 eyes became amaurotic. The visual field was unchanged in 36% of the operated eyes, as against 20% of the medically treated eyes. In no case was central vision lost as as consequence of surgery. Obviously, the prognosis was better in eyes with intraocular pressures below 15 mmHg during the follow‐up (58% stable visual fields) than in the eyes with pressures higher than 15 mmHg (15% stable fields). Abstract.We studied six patients with early failure after Watson's trabeculectomy done due to chronic simple, neovascular, or chronic angle‐closure glaucoma. After operation the eyes showed a maximum intraocular pressure (IOP) between 50 mmHg and 66 mmHg and no filtering bleb. Reoperation on the 5th to 14th post‐operative day showed incarceration of ciliary body into the trabeculectomy fistula in all cases. The uveal protrusion was replaced and a deep corneoscleral block was removed in front of the scleral spur in three cases, and electrocoagulation of the anterior edges of the trabeculectomy fistula was done in other three cases. After a 4 months to 1.5 years follow‐up IOP was 12 mmHg to 18 mmHg and only two eyes needed medical therapy. Abstract.Argon Laser Trabeculoplasty (ALT) have been used in the treatment of open angle glaucoma since 1982.
